Spontaneous vaginoperitoneal fistula as a rare complication of carcinoma ovary: a case report.

BACKGROUND: Spontaneous vaginoperitoneal fistula formation in a case of carcinoma ovary is a very rare occurrence and has never been reported. CASE PRESENTATION: A 55-year-old postmenopausal lady presented with complaints of abdominal distention and mass coming out of the vagina for the last 10 days. On examination, she had tense ascites, uterovaginal prolapse and hard, fixed mass felt anteriorly on per-rectal examination. Biochemical investigations and radiological imaging suggested advanced stage ovarian neoplasm. She was planned for neoadjuvant chemotherapy. During the second cycle of chem

Primary Tumors of the Sacrum: Imaging Findings.

The diagnosis of sacral neoplasms is often delayed because they tend to remain clinically silent for a long time. Imaging is useful at all stages of the management of sacral bone tumors, i.e., from the detection of the neoplasm to the long-term follow-up. Radiographs are recommended as the modality of choice to begin the imaging workup of a patient with known or suspected sacral pathology. More sensitive examinations, such as Computerized Tomography (CT), magnetic resonance (MRI), or scintigraphy, are often necessary. The morphological features of the lesions on CT and MRI help orientate the d

Relationship between diagnostic imaging features and prognostic outcomes in gastrointestinal stromal tumors (GIST).

Gastrointestinal stromal tumors (GISTs), the most frequent mesenchymal neoplasms of the gastrointestinal tract, are a relatively recently described entity. GISTs can occur across any age but are more common in patients older than 50 years. GISTs most commonly are in the stomach (60-70%), followed by the small intestine (20%-30%); they also rarely occur in the abdominal cavity, such as in the mesentery, the omentum and the retroperitoneum. Contrast-enhanced multi-detector computed tomography (MDCT) is the most largely used imaging modality for the localization, characterization and staging of G

[Myelophthisis and kasabach merrit syndrome as initial manifestation of splenic angiosarcoma].

Primary splenic angiosarcoma is an extremely agressive and rare neoplasm. Manifestations as bone marrow invasion and coagulation disorders have been reported isolatedly. A 26 years-old woman presented with abdominal pain; several anemia and thrombocytopenia associated to leukoerythroblastic reaction were found in the laboratory. Consumpion coagulopathy signs and microangiopathy as schistocytes, prolonged prothrombine time, decreased fibrinogen and increased D dimer were also present. Imaging findings included a lobulated, enlarged spleen, with spontanously hyperdense areas, and heterogeneous n

[Reevaluation of the term "carcinoid"--how to deal with this misnomer].

In the present review, the malignant entity of so-called carcinoid is discussed from three points of view: [I] The earliest extraglandular(intramucosal) invasion of neoplastic cells following a focal penetration of the basement membrane of a gastric gland in the form of a "budding phenomenon", confirmed on multiple consecutive serial sections. [II] Reevaluation of the original diagram of the submucosal tumor, that is a result of invasion from the original site in the mucosa. [III] Analysis of human materials on metastasis rates at the stage of submucosal-nodule formation in the stomach and rec

Neoplasms of the fallopian tube and broad ligament: a selective survey including historical perspective and emphasising recent developments.

Recent contributions on selected aspects of the pathology of tubal and paratubal neoplasms are reviewed after a brief historical perspective. The most important, considering both frequency and clinical importance, are those pertaining to tubal carcinoma. These can be grouped as follows: (1) recognition that endometrioid carcinoma is the second most common carcinoma of the tube, after serous, and other variants of müllerian carcinoma are rare; (2) expansion of knowledge about the morphological spectrum of tubal endometrioid carcinoma; (3) modifications of the FIGO staging system; (4) heightened

Supracricoid partial laryngectomy as a primary treatment for carcinosarcoma of the larynx.

Carcinosarcoma of the larynx is uncommon. When it does occur; its clinical features resemble those of sarcomatoid carcinoma, and its biologic behavior is similar to that of malignant mesenchymal neoplasms. We describe 2 cases of carcinosarcoma of the glottis. The tumors were staged as T3N0M0 and T2N0M0. Both patients were treated with supracricoid partial laryngectomy with cricohyoidoepiglottopexy. Eight months postoperatively, 1 of the 2 patients experienced a recurrence of the sarcomatous component of the tumor, and he underwent a total laryngectomy. The other patient remained free of diseas

Clinicopathological consultation. Lymphoepithelial carcinoma of the larynx hypopharynx, and trachea.

Lymphoepithelial carcinoma of the larynx, hypopharynx, and trachea is a rare neoplasm composed of large, poorly differentiated, nonkeratinized cells intermingled with small nonneoplastic lymphocytes and plasma cells. It is histologically similar to its more common counterpart occurring in the nasopharynx. In contrast to nasopharyngeal carcinoma, most cases have not been associated with Epstein-Barr virus (EBV), although rare cases have been reported to be EBV-positive. The diagnosis often requires immunohistochemistry or electron microscopy for confirmation. The neoplasm seems to behave in a f

Papillary serous carcinoma of the retroperitoneum.

We describe a retroperitoneal neoplasm in an 11-year-old girl which had a light microscopic appearance identical to that of papillary serous carcinoma of the ovary. There was no evidence of ovarian involvement. Immunohistochemical staining for amylase was positive within the cytoplasm of tumor cells. Since amylase is a marker for serous ovarian tumors, this finding supports the belief that "ovarian-type" neoplasms that occur at ectopic locations are essentially identical to their ovarian counterparts. We believe they originate from metaplasia of mesothelium. Our findings support the concept th

Revisions to the TNM Staging of Lung Cancer: Rationale, Significance, and Clinical Application.

Lung cancer remains the leading cause of cancer-related mortality worldwide. To formulate effective treatment strategies and optimize patient outcomes, accurate staging is essential. Lung cancer staging has traditionally relied on a TNM staging system, for which the International Association for the Study of Lung Cancer (IASLC) has recently proposed changes. The revised classification for this eighth edition of the TNM staging system (TNM-8) is based on detailed analysis of a new large international database of lung cancer cases assembled by the IASLC for the purposes of this project. Endoscopic ultrasound in cancer staging.

BACKGROUND: Endoscopic ultrasound (EUS) represents one of the most significant developments in endoscopy over the past 20 years. It allows highly detailed assessment of the gastrointestinal wall layers as well as to visualize extraluminal structures such as the mediastium and retroperitoneum. METHODS: The literature was reviewed to assess the role of EUS in cancer staging. RESULTS: EUS is an integral part of the staging of many upper gastrointestinal cancers as well as rectal and lung cancer and has been shown to be cost-effective. It can be used to confirm malignancy in suspicious lesions a

Lung cancer: update on staging classifications.

The AJC for Cancer Staging has improved the TNM system for lung cancer by breaking up the large category of advanced disease into several groups with different treatment implications. Radiologists should be familiar with this system so that they can direct the imaging part of the workup efficiently. The ATS classification of regional lymph nodes also has been updated both to take into account the growing recognition of superior diaphragmatic nodes in lung cancer and to change the supraclavicular lymph nodes from distant metastases to thoracic nodes (in keeping with the revised TNM system). Use

Unusual CT findings in urachus carcinoma with an extensive and atypical pattern of metastasis. A case report.

Carcinoma of the urachus is very rare. Usually it has a typical appearance on computed tomography, with calcifications in a midline supravesical mass, but advanced stages of this neoplasm require malignancy evaluation that is not easy to establish. We report a case of a urachal tumor where CT scan did not properly assess the response to chemotherapy, while it did show an uncommon metastatic localization in the laterocervical soft tissues.

The pattern of p14ARF expression in primary and metastatic human endometrial carcinomas: correlation with clinicopathological features and TP53 pathway alterations.

OBJECTIVES: Alterations of p53 pathway (p14(ARF)/MDM2/p53) play a crucial role in the development and progression of various human neoplasms, including endometrial carcinoma (EC). The aim of the current research was to examine the p14(ARF) expression pattern in primary ECs and corresponding metastatic lesions. MATERIALS AND METHODS: We studied 47 primary ECs and corresponding metastatic lesions applying immunohistochemistry and investigated the relationship between p14(ARF) overexpression and clinicopathological variables of carcinoma as well as TP53 alterations. RESULTS: Protein expression

Laryngeal squamous cell carcinoma in HIV-positive patients: lack of association with human papillomavirus infection.

OBJECTIVES: Neoplasms associated with human papillomavirus (HPV) infection occur at increased frequency in patients with HIV infection/AIDS. Although laryngeal squamous cell carcinomas (LSCCs) in HIV-positive patients are uncommon, a higher incidence of this malignancy in HIV-positive patients than in the general population has been reported. As a proportion of LSCCs are associated with HPV in the general population, the clinicopathological features of a series of LSCCs developing in HIV-positive patients were evaluated to investigate the possible relationship with HPV infection, and infection

From the archives of the AFIP: a comprehensive review of fetal tumors with pathologic correlation.

Fetal tumors are a diverse group of neoplasms, which are unique in their histologic characteristics, anatomic distribution, and pathophysiology. The biologic behavior of tumors in the fetus may differ dramatically compared with that of the same tumor detected later in life. Teratomas are the dominant histologic type and constitute the majority of both extracranial and intracranial neoplasms. Although often histologically mature, they may prove lethal because of their location and metabolic demands on the fetus. Large solid tumors may lead to cardiovascular compromise and hydrops fetalis.
