The liver in hereditary hemorrhagic telangiectasia (Osler-Weber-Rendu disease).

The livers of four patients with hereditary hemorrhagic telangiectasia--including the original case of Osler--were examined at autopsy. Characteristic random focal fibrovascular lesions were found in all. The importance of recognizing the apparently common and seemingly benign hepatic involvement in this disease is emphasized in view of its possible confusion with more serious types of liver disease that may complicate the condition.

A Rare Case of Crystal-Storing Histiocytosis of the Breast in a Patient With Undifferentiated Connective Tissue Disease.

Crystal-storing histiocytosis (CSH) is a rare entity usually presenting microscopically as sheets of histiocytes with intralysosomal accumulation of refractile, crystalline substances. CSH is most often the result of monoclonal immunoglobulin light chain deposition in the setting of a plasma cell neoplasm or lymphoma with plasma cell differentiation. However, this condition may arise in association with autoimmune, inflammatory, infectious, and drug-related etiologies and can harbor polyclonal immunoglobulins or other crystalline materials. It usually presents in a localized fashion, with the

Delirious Mania Associated with Autoimmune Gastrothyroidal Syndrome of a Mid-Life Female: The Role of Hashimoto Encephalopathy and a 3-Year Follow-Up including Serum Autoantibody Levels.

We report the case study of a 57-year-old Caucasian female with steroid-responsive encephalopathy associated with autoimmune thyroiditis (SREAT), commonly termed Hashimoto encephalopathy (HE). This presentation includes one of the longest lasting follow-up studies of HE considering the neuropsychiatric symptoms (here delirium, mania, and EEG-slowing) and their relation to serum autoantibody levels. Antithyroid-peroxidase autoantibodies, the hallmark of autoimmune thyroiditis, were found in the serum and also in the cerebrospinal fluid. Diagnostic analyses found no evidence of limbic encephalop

[Florid glandular cystitis of the intestinal type with mucin extravasation: a lesion simulating a tumor].

A wide spectrum of glandular epitheliales metaplastic changes may be seen in the bladder. Extensive replacement of the urotelium with an epithelium resembling intestinal mucosa is recognised as a premalignant condition. However, the natural history of intestinal metaplasia of the urinary bladder and long-term outcome are unknown. We report a case of glandular cystitis with intestinal metaplasia in a young patient. Cystoscopy revealed a tumour well circumscribed measuring 4 cm on the trigone. Microscopic examination showed numerous glands lined by intestinal type epithelium without atipia, conf

Intraductal tubulopapillary neoplasms with rupture of the distal main pancreatic duct: a case report.

BACKGROUND: Intraductal tubulopapillary neoplasm (ITPN) is a rare and newly described entity defined as an intraductal, grossly visible, tubule-forming epithelial neoplasm with high-grade dysplasia and ductal differentiation without overt production of mucin. Because of its rarity, the clinical and molecular aspects of ITPN have not been fully investigated. CASE PRESENTATION: A 73-year-old woman presented to a local hospital with epigastric discomfort and pain. Abdominal multidetector-row computed tomography (MDCT) revealed a 2.5-cm hypovascular tumor in the pancreatic body with distal pancre

Kaposi Sarcoma as Initial Presentation of HIV Infection.

CONTEXT: Kaposi's sarcoma (KS), a vascular tumor that manifests as nodular lesions on the skin and to a lesser extent, the visceral organs, is the most common neoplasm encountered in human immunodeficiency virus (HIV)-infected patients. It consists of an angiosarcomatous change of not only the epithelial and mucous membrane-associated connective tissue in various sites, for example, skin, gastrointestinal system, lungs, and so on, but may also involve non-epithelial organs, such as lymph nodes. Surgical excision is the line of management for the tumor. CASE REPORT: We present one case of a 65

Relationship between paraglottic space invasion and cervical lymph node metastasis in patients undergoing supracricoid partial laryngectomy.

BACKGROUND: To investigate the relationship between paraglottic space (PGS) invasion and lymph node metastasis in patients undergoing supracricoid partial laryngectomy. METHODS: The study included 98 subjects with squamous cell carcinoma of the glottis. RESULTS: The PGS invasion rate was 60.2% (59/98), and a significant correlation was found between PGS invasion and cervical lymph node metastasis (p = .022). Fifteen of the 59 patients (25.4%) with PGS invasion, only 3 of the 39 patients (7.7%) without any evidence of PGS invasion had lymph node metastasis. Furthermore, a significant correlat

Phosphaturic Mesenchymal Tumour of the Soft Tissue in the Finger Presenting without Tumour-Induced Osteomalacia: A Case Report.

Phosphaturic mesenchymal tumour (PMT) is a rare neoplasm that can occur with or without tumour-induced osteomalacia (TIO). While PMTs commonly arise in the lower extremities or head and neck, digital involvement is rare. A 70-year-old man presented with a 12-year history of an enlarging subcutaneous mass on the dorsal aspect of his left ring finger. Imaging studies suggested an organised thrombus within a venous malformation. Marginal excision was performed, and histopathological examination revealed a lesion composed of capillaries and small vessels surrounded by spindle cells with small ovoi

Mandibular Osteoma in a Haflinger Mare: Clinical and Pathological Features.

A 11-year-old, Haflinger breed mare developed a slowly growing mass, protruding on the ventral aspect of the mandible. On palpation, the lesion was pedunculated, very firm, cold and not painful. Radiographic examinations showed a radiopaque, multi-lobular, "mushroom-like" lesion, which arose from the mandibular cortex without involving the bone marrow cavity. Thereafter, the mass was excised for cosmetic purposes. Microscopically, the lesion mainly consisted of bone trabeculae, covered by a thick layer of connective tissue resembling the periosteum. History and clinical findings allowed the te

Mast Cell Association with the Microenvironment of a Phosphaturic Mesenchymal Tumour Secreting Fibroblast Growth Factor 23.

Background: Phosphaturic mesenchymal tumours secreting fibroblast growth factor 23 (hereinafter referred to as FGF23+ PMT) are rare neoplasms that can cause hypophosphataemic osteomalacia, owing to excessive FGF23 production. Mast cells (MCs) play a key role in tumour biology by modulating proliferative activity of atypical cells, resistance to innate and acquired immunity, angiogenesis, and metastatic behaviour. However, MCs associated with FGF23+ PMT have not previously been investigated. This study, to our knowledge, is the first to characterise features of the tumour microenvironment throu

Biochemical evaluation of X-linked hypophosphatemia and tumor-induced osteomalacia: insights into diagnosis and management.

INTRODUCTION: X-linked hypophosphatemia (XLH) and tumor-induced osteomalacia (TIO) are characterized by alterations in phosphate metabolism due to elevated levels of fibroblast growth factor 23 (FGF23). These conditions cause significant morbidity due to chronic hypophosphatemia and resulting musculoskeletal disorders. OBJECTIVE: This study aims to provide clinical strategies for supporting the diagnosis and management of the biochemical profile of patients with XLH and TIO, addressing key considerations beyond the hypophosphatemia and hyperphosphaturia commonly observed in these conditions a

Extraskeletal myxoid chondrosarcoma: A study of 17 cases focusing on the diagnostic utility of INSM1 expression and presenting rare morphological variants associated with non-EWSR1::NR4A3 fusions.

Extraskeletal myxoid chondrosarcoma (EMC) is a rare sarcoma of uncertain lineage. Insulinoma-associated protein 1 (INSM1) has recently been described as a highly specific and sensitive immunohistochemical marker for EMC. The goal of this study was to evaluate the diagnostic significance of INSM1 immunohistochemistry in EMC. Furthermore, correlations between molecular and morphological findings were performed. Sixteen of 17 EMC cases were stained with the INSM1 antibody. Tumors with at least 5% INSM1-positive cells and any staining intensity were considered positive. Molecular testing was succe

Interim Analysis of a Phase 2 Open-Label Trial Assessing Burosumab Efficacy and Safety in Patients With Tumor-Induced Osteomalacia.

Patients with tumor-induced osteomalacia (TIO), an acquired paraneoplastic condition characterized by osteomalacia due to hypophosphatemia, exhibit a similar clinical picture to those with X-linked hypophosphatemic rickets/osteomalacia (XLH). The human monoclonal anti-fibroblast growth factor 23 (FGF23) antibody burosumab (KRN23) increases serum phosphate and improves bone turnover, fracture healing, pain, and physical function in XLH patients by inhibiting circulating FGF23; thus, burosumab is expected to be an effective treatment for TIO. We report here an interim analysis of a multicenter,

Rituximab: Uses in Dermatology.

Rituximab is an anti-CD20 monoclonal antibody with considerable potential in dermatology due to an increase in off-label indications. Chronic graft-versus-host disease and pemphigus vulgaris are two of the most promising indications for off-label use of rituximab. It is a generally safe alternative that should be considered when traditional therapy with corticosteroids or immunosuppressants has failed or caused significant intolerance. Currently, rituximab is only FDA-approved for treatment of follicular and diffuse large B-cell non-Hodgkin's lymphoma, rheumatoid arthritis, chronic lymphocytic

Ossifying Fibroma in a Patient With Oculocerebrocutaneous (Delleman) Syndrome.

Delleman syndrome is a rare neurocutaneous disorder characterized by congenital anomalies affecting the eye, skin, and central nervous system. This disorder was first raised as a distinct syndromic entity by Delleman and Oorthuys in 1981 under the term "oculocerebrocutaneous syndrome" (OCCS). Since then, fewer than 40 cases have been reported. All cases have been sporadic, and no patient with an abnormal karyotype has been reported. Although the etiology is still unknown, several theories have been advanced. Most of the tissues affected by OCCS are neural crest derivatives, with a striking hom

[Anti-FGF23 antibody therapy for patients with tumor-induced osteomalacia].

Tumor-induced osteomalacia (TIO) is a disease caused by fibroblast growth factor 23 (FGF23) secreted from the causative tumor. This disease is cured by complete surgical removal of the tumor. However, there are several difficult cases in which the responsible tumors cannot be found, are incompletely removed, or relapse after the surgery. Anti-FGF23 antibody is being studied as a novel therapy for FGF23-related hypophosphatemic diseases. The efficacy of anti-FGF23 antibodies were confirmed using a murine model of X-linked hypophosphatemic rickets (XLHR) , which is the most common heritable form
