Neurogenic Bladder-Induced Stone in a Pelvic Kidney of a Caudal Regression Syndrome Patient: Management of a Complex Case.

Percutaneous nephrolithotomy (PCNL) is a difficult treatment for treating kidney stones, especially when there are orthopedic or skeletal abnormalities. Here, in a 19-year-old male, we describe a two-step PCNL with a case of caudal regression syndrome (CRS) and a pelvic kidney, with an extremely deformed neurogenic bladder on intermittent catheterization. Our conclusion is that PCNL may be done safely with minimum morbidity in patients with caudal regression syndrome by utilizing adult equipment for heavy stone burdens, allowing full and rapid stone removal.

Advanced Management of Complex Transplant Lithiasis: Low Lithotomy and Boot Stirrups Technique.

BACKGROUND Transplant lithiasis may be rare but poses significant risk to the renal graft function of the recipient. Immediate management is necessitated upon first detection, to prevent further complications. CASE REPORT We report 2 cases of transplant lithiasis that were not treated immediately upon first detection. The first patient was a 42-year-old man that received a living-donor kidney from his mother, which was complicated by ureterovesical anastomotic stenosis, BK polyomavirus infection, and oliguria. He had a renal stone and 4 ureteric stones in his right allograft. The second patien

Delayed migration of embolized coil with large renal stone formation: a rare presentation.

Delayed bleeding following percutaneous nephrolithotomy (PCNL) usually occurs due to development of the pseudoaneurysm which can be successfully managed with coil embolization. However very few cases of such complications have been reported in the literature. Here we are reporting a case of delayed post-PCNL bleeding that occurred in a 53-year-old diabetic patient operated on for renal stone. Computed tomography scan revealed a presence of the pseudoaneurysm in the segmental branch of right renal artery, which was successfully managed with coiling. Patient remained asymptomatic for the next 9

Extraosseous bone formation in the renal pelvis.

PURPOSE: We evaluated the frequency and features of extraosseous bone formation in the kidneys of patients undergoing percutaneous nephrolithotomy. MATERIALS AND METHODS: Percutaneous nephrolithotomy was performed in 621 patients at our institution between 1997 and 2006. In 21 cases metaplastic bone arising from the urothelium was observed. Clinical, radiographic and histopathological features of this group were studied in detail. RESULTS: Patient age ranged from 7 to 40 years (median 11). Extraosseous bone formation was identified in 7 right (33%) and 14 left (67%) kidneys. In all cases ext

Aid to percutaneous renal access by virtual projection of the ultrasound puncture tract onto fluoroscopic images.

BACKGROUND AND PURPOSE: Percutaneous renal access in the context of percutaneous nephrolithotomy (PCNL) is a difficult technique, requiring rapid and precise access to a particular calix. We present a computerized system designed to improve percutaneous renal access by projecting the ultrasound puncture tract onto fluoroscopic images. MATERIALS AND METHODS: The system consists of a computer and a localizer allowing spatial localization of the position of the various instruments. Without any human intervention, the ultrasound nephrostomy tract is superimposed in real time onto fluoroscopic ima

Retrograde Intrarenal Renal Surgery versus Supine Mini Percutaneous Nephrolithotripsy in Treatment of Renal Pelvic Stones Less than 2 cm, Randomized Clinical Study.

OBJECTIVE: Retrograde intrarenal surgery (RIRS) is the first-line treatment for kidney stones between 1 and 2 cm and can serve as an alternative to percutaneous nephrolithotomy for larger stones in high-risk patients, including those with bleeding disorders, obesity, renal congenital abnormalities, or solitary kidneys. We aimed to compare the safety and efficacy of supine mini-percutaneous nephrolithotomy (PCNL) versus RIRS for the treatment of renal pelvic stones less than 2 cm. METHODS: This prospective, randomized, double-blinded study included 50 patients aged >18 years, of both sexes, wi

Robotic Pyelolithotomy, Extended Pyelolithotomy, Nephrolithotomy, and Anatrophic Nephrolithotomy.

INTRODUCTION: We are a reporting on the indications, techniques, and limitations of robotic surgery in the management of renal stones disease. Robotic surgery is a good tool to manage large kidney and ureteral stones, particularly in patients with anatomic anomalies. We describe three techniques in managing staghorn kidney stones: robotic anatrophic nephrolithotomy, robotic pyelolithotomy, and robotic nephrolithotomy. MATERIAL AND METHODS: Robotic pyelolithotomy (RP) is ideal for patients with large renal pelvis and partial staghorn stone with a wide extra-renal pelvis. Robotic nephrolithotom

Safety and efficacy of retrograde intrarenal surgery in primary and residual renal calculi in children.

BACKGROUND: Minimal invasive approach is the current standard of care in the management of pediatric renal calculi. Current guidelines are clear with extra corporal shock wave lithotripsy (ESWL) and percutaneous nephrolithotomy (PCNL) for stone size less than and greater than 20 mm respectively. Although retrograde intrarenal surgery (RIRS) is well established in adults but literature on its role, safety and efficacy in children is sparsely available. OBJECTIVE: To share our experience of RIRS and its outcome in a pediatric population in both primary and residual calculi of size less than 20

Jackstones in the renal pelvis: A rare calculus.

Jackstones are a rare subtype of calculi, characterised by radiating spicules. We describe a case of multiple jackstones in the urinary upper tract. A 53-year-old man was referred for evaluation of left flank pain. Imaging revealed a left pelvic-ureteric junction obstruction and several left renal calculi. A left supine percutaneous nephrolithotomy was performed and two jackstone calculi were extracted. The presence of multiple jackstones in the renal pelvis should prompt for evaluation for obstruction and complete stone clearance should be achieved to avoid recurrence.

Synchronous melanosis of upper and lower urinary tract.

Urothelial melanosis is an exceptionally rare diagnosis, with less than 25 cases being reported in the literature. Melanosis of the urothelium is characterized by abnormal melanin deposition within tissues, producing a black, velvety appearance to the urothelial mucosa. We present a 67-year-old male undergoing cystoscopy during a routine percutaneous nephrolithotomy (PCNL), who was found to have diffuse bladder melanosis extending up the ureter and into the renal pelvis. To our knowledge, this is the first reported case of synchronous melanosis of upper and lower urinary tract.

[The role of micro-percutaneous nephrolithotripsy in the treatment of kidney stones].

BACKGROUND: Urolithiasis is one of the most common diseases of the urinary system, accounting for up to 50% of the capacity of urology departments in the Russian Federation. Modern minimally invasive technologies, including micro-percutaneous nephrolithotomy (PCNL), offer opportunities to optimize surgical strategy; however, the indications for its use remain controversial. AIM: To perform a comparative analysis of the outcomes of micro-percutaneous procedures versus other minimally invasive interventions in patients with kidney stones up to 2 cm in size. MATERIALS AND METHODS: The study inc

RIRS using a flexible and navigable suction ureteral access sheath and thulium fiber laser in a patient taking anticoagulants with multiple large kidney stones. Clinical case.

Percutaneous nephrolithotomy is one of the modern methods for treating kidney stones larger 2 cm. When anticoagulation therapy cannot be stopped, retrograde intrarenal surgery (RIRS) is recommended. We report a case of RIRS using flexible and navigable suction ureteral access sheath (FANS-UAS) and Thulium fiber laser (TFL) in a patient with multiple large kidney stones who could not be stopped on anticoagulant therapy. The surgery was successful and without complications. RIRS using FANS-UAS and TFL in this group of patients has high efficacy and safety.

Asymptomatic emphysematous pyelitis-a rare clinical entity.

A 62-year-old asymptomatic woman with diabetes was referred to the urology department from nephrology due to deterioration in renal function with accompanied right-sided hydronephrosis on ultrasound. CT imaging subsequently revealed a right-sided staghorn calculus and a significant volume of gas in the right collecting system from the kidney to the distal ureter, in keeping with emphysematous pyelitis. She was admitted and managed with antibiotics and insertion of right nephrostomy in the first instance, followed by percutaneous nephrolithotomy to definitively manage the stone. The patient rem

When is the indication of percutaneous chemolysis justified?

Persistent residual stone fragments after extracorporeal shockwave lithotripsy (ESWL), percutaneous nephrolitholapaxy (PNL) as well as pyelo- and nephrolithotomy represent a great problem in the treatment of stones. The choice of therapy for patients with insufficient renal drainage, poor renal function, and a high anesthetic risk also remains quite difficult. Between 1991 and 1993, 5 patients underwent percutaneous antegrading chemolysis with 'Suby G' solution. Two patients, presenting struvite and apatite stones, were free of stones afterwards. In 1 patient, where stone size could be observe

Ultrasound-Guided Access in a Malrotated Kidney: Optimally Safe and Effective.

Background: Management of renal calculi in a patient with kidney malrotation can be difficult because of complexity and alteration of collecting system anatomy. Pyelolithotomy, whether open, laparoscopic, or robotic, has been shown to be an effective method of stone removal in this patient population; however, it is not always ideal because of its invasiveness and increased morbidity. Ideally, a percutaneous approach may be less invasive, and if feasible, it can optimize patient safety and stone-free status. Case Presentation: Here we present a case of a 68-year-old Caucasian female who presen

Evaluation of outcomes in kidney stone management: stone-free rate vs. cost vs. quality of life: a systematic review from EAU endourology.

BACKGROUND: Nephrolithiasis is a prevalent and highly recurrent pathology that affects up to 15% of the global population. The three main treatment modalities of nephrolithiasis are extracorporeal shock wave lithotripsy (ESWL), ureteroscopy (URS), and percutaneous nephrolithotomy (PCNL), however they differ in efficacy, invasiveness, and cost. Although stone-free rate (SFR) remains the traditional measure of success, a comprehensive evaluation should integrate economic and patient-centered outcomes. OBJECTIVE: To compare stone-free rate (SFR), cost, and quality-of-life (QoL) outcomes for patie

Development and validation of the UCSS score, a novel method to predict septic shock after PCNL.

OBJECTIVE: To develop an objective and easily recognizable model to predict septic shock following percutaneous nephrolithotomy (PCNL). SUBJECTS AND METHODS: First, we identified differences between 431 patients who underwent PCNL with or without septic shock. These data were used to develop existing models and examine their improvement. Multivariate analysis was applied to identify risk factors of septic shock after PCNL based on the scores allocated to the PCNL postoperative test indicators. Finally, we developed a predictive nomogram using the selected factors and compared its performance

[Endovascular treatment of traumatic and iatrogenic intrarenal arterial lesions by microcoil embolization].

Selective embolization is the best treatment for intrarenal arterial lesions due to trauma or percutaneous procedures with non-controlled or recurrent haematuria. Three male patients, aged 20-70 (mean 49 years), were recently treated in our institution by means of arterial embolization with microcoils. Two patients presented a pseudo-aneurysm and an arterio-venous fistula secondary to percutaneous nephrolithotomy and one patient presented an isolated pseudo-aneurysm due to trauma. In the 3 cases, haematuria (associated with retroperitoneal haemhorrage in one case) was not controlled and requir
