OBJECTIVES: Partial or total degeneration of the vestibulocochlear anastomosis at its takeoff from the saccular ganglion and regenerating efferent neural buds under the cochlear outer hair cells (OHCs) have been found in the temporal bones of human patients with a history of vestibular neuritis (VN). We sought to test whether VN has any functional impact on the ipsilateral OHCs by means of transient evoked otoacoustic emission (TEOAE) testing.
METHODS: We retrospectively analyzed prospectively collected TEOAE data of 28 patients (19 female, 9 male; age range, 21 to 60 years; median age, 42.5

Auditory brainstem responses (ABRs) were studied in 15 adults with suspected normal pressure hydrocephalus (NPH) before and after shunting. The patients were divided into Groups A (shunt-ineffective) and B (shunt-effective). The pre- and postoperative ABRs of each patient were compared with those of 20 normal volunteers, and the relationships between ABRs and certain clinical findings were investigated. Preoperatively, nine patients (60%) showed prolonged central conduction time (CCT) (interpeak latency of wave I-V or neural-axonal conduction time) relative to the mean control value plus 2 SD.

The projections of the peripheral and intracerebral portions of the hamster terminal nerve were examined with lesion and immunocytochemical techniques. After transection, proximal processes of the terminal nerve accumulate luteinizing hormone-releasing hormone-immunoreactive material, while the distal processes disintegrate and are no longer stained. It thus becomes possible to determine the direction of conduction of terminal nerve axons. The results of transection at the level of the cribriform plate, along the olfactory bulb, and in the ventral forebrain were all consistent in indicating a

This study aims to advance our mechanistic understanding of electrical stimulation of dorsal root ganglia (DRG) for treating chronic pain. While DRG stimulation has shown moderate clinical success in managing certain types of chronic pain, the underlying neural mechanism remains inconclusive, hindering the further development of the technology to treat a broader range of chronic pain symptoms and benefit a larger patient population. In this study, we conducted computational simulations in the NEURON simulation environment to assess the neuromodulatory effect of DRG stimulation on action potent

BACKGROUND AND PURPOSE: Distal sensory polyneuropathy (DSPN) is a common complication of Diabetes Mellitus (DM). Patients often experience disturbing neuropathic symptoms such as numbness and burning. These symptoms can also be seen in prediabetic patients who do not meet the diagnostic criteria for DM. The sural nerve responses are evaluated as standard in electrophysiologic technique. However, the sural nerve may not always show abnormalities in those with mild neuropathy. Therefore, different nerves can be studied to detect mild neuropathy. Our study aimed to investigate both the superiorit

BACKGROUND: Carpal tunnel syndrome (CTS) is a common compressive neuropathy that is diagnosed primarily through clinical symptoms and electrodiagnostic testing. However, the lack of a gold standard and the varied accuracy of nerve conduction studies (NCS) present challenges in diagnosis and treatment decisions.
OBJECTIVES: This study aimed to evaluate the efficacy of different measurements of NCS in patients with CTS, analyze the postoperative changes in NCS, and identify factors associated with surgical outcomes.
MATERIALS AND METHODS: A total of 565 patients (1077 hands) presenting with CT

BACKGROUND: In hereditary transthyretin (ATTRv) amyloidosis, axonal neuropathy is the main pathologic basis of polyneuropathy. However, whether nerve conduction slowing resembling chronic inflammatory demyelinating polyneuropathy (CIDP), observed in some patients, reflects primary demyelination or secondary changes related to axonal damage remains unclear.
METHODS: Electrophysiologic findings were compared between 96 patients with V30M ATTRv amyloidosis from nonendemic areas and 175 patients with nutritional or alcoholic axonal neuropathies. Myelinated fiber density was assessed in sural nerv

BACKGROUND: Finger extension weakness could be a presentation of either posterior interosseous nerve (PIN) syndrome or multifocal motor neuropathy (MMN). However, there is a delay in the diagnosis of MMN in cases with a selective radial weakness, as they are frequently misdiagnosed as PIN.
OBJECTIVE: To analyze which variables in nerve conduction studies could aid in the early diagnosis of MMN.
METHODS: We reviewed charts of patients with diagnoses of MMN or PIN syndrome, from 2014 to 2022, in a single Brazilian reference center. Electrophysiological parameters included in the analysis were

BACKGROUND: Carpal Tunnel Syndrome (CTS) is the most common compressive neuropathy, accounting for 90% of all neuropathies. Its prevalence ranges from 3.8%-7.8% in the population. The gold standard for its diagnosis is the neurophysiological study (85% sensitivity and 95% specificity), with the disadvantage of being invasive, complex and expensive, which means an increase in cost and time for the diagnosis of the disease. The main objective of this diagnostic test evaluation study is to investigate the value of ultrasound in the diagnosis of CTS, and among the secondary objectives, to establis

We study numerically the dynamics of conduction blocks using a detailed electrophysiological model. We find that this dynamics depends critically on the size of the paced region. Small pacing regions lead to stationary conduction blocks while larger pacing regions can lead to conduction blocks that travel periodically towards the pacing region. We show that this size-dependence dynamics can lead to a novel arrhythmogenic mechanism. Furthermore, we show that the essential phenomena can be captured in a much simpler coupled-map model.

Diabetic Polyneuropathy (DPN) is a common debilitating complication of Diabetes Mellitus (DM), causing pain which may progress to numbness and motor involvement. DPN may lead to diabetic foot ulcers and possible amputation. Thus, DPN affects the patients' quality of life and poses a financial burden if not managed early. Nerve conduction studies (NCS) have been used as an objective method for diagnosis of DPN. Neuromuscular Ultrasound (NMUS) is a more comfortable tool used to complement NCS in diagnosis of many diseases. The study aimed to investigate NMUS as a possible screening tool for DPN.

OBJECTIVES: Patients presenting with musculoskeletal symptoms are fairly common in any neurologist practice, their presentation raises concern of underlying radiculopathy as a cause of their symptoms. The role of electrodiagnostic study has been variable in such patients. Creating a diagnostic tool will be of most help to provide physicians of predictive value of such tests before ordering and help guide them in finding the most appropriate test.
METHODS: Retrospective chart review study of 444 patients older than 18 years. The patients were referred to the electrodiagnostic laboratory with m

A 36-year-old man presented with rapidly progressive lower-limb weakness that reached its nadir within 16 h of the onset. The initial nerve conduction study (NCS) revealed no significant abnormalities except for a decrease in F-waves. Intravenous immunoglobulin therapy was administered, resulting in a gradual improvement in muscle strength. Follow-up NCS revealed findings consistent with axonal damage, and subsequent testing revealed positivity for immunoglobulin G-class anti-GM1 antibodies. The patient was finally diagnosed with acute motor axonal neuropathy, a variant of Guillain-Barré syndr

Spinal cord injury (SCI) is a severe neurological disease resulting in the formation of a harsh microenvironment that hinders neural regeneration. By preserving the components and structure of natural tissues, the decellularized extracellular matrix (dECM) mimics the neural regeneration microenvironment and provides the biochemical signals and structural support for neural regeneration. However, the regenerated neurons lack the ability to autonomously restore neural communication. In this study, we assemble the conductive polymer poly(3,4-ethylenedioxythiophene) (PEDOT) within spinal cord-deri

The blink reflex was studied in four patients with neurinoma of the acoustic nerve and in two patients with trigeminal neurinoma. It was abnormal in all cases except in one neurinoma of the acoustic nerve inside the canal. The jaw reflex, studied in four cases, showed unilateral impairment in two cases of trigeminal neurinoma and one case of acoustic neurinoma; it was normal in the case of acoustic neurinoma in the canal. The blink reflex and jaw reflex merit inclusion among the further tests conducted in cases of suspected neurinoma of the trigeminal or acoustic nerves. However, they are of l

Group-level voxelwise statistical analyses of lesion-deficit relationships have been used to implicate brain structures critical for specific aspects of human cognition and behavior. Current approaches fail to account for the role of fiber tract disruptions in causing deficit, and confound cortical damage with damage to fibers of passage. Here, we develop a framework, Generalized Lesion-Symptom Mapping (GLSM), to integrate fiber tract information from DTI-based tractographic atlases in tractwise and voxelwise lesion-deficit analyses. First, we used the geniculo-calcarine system as a validation
