[Balloon compression for the treatment of trigeminal neuralgia].

The authors present a retrospective study of 121 patients treated with balloon compression of the rootlets behind the Gasser ganglion from 1995 to 2007. The inclusion criteria were drug-resistant idiopathic trigeminal neuralgia. The authors described the surgical technique and compared their results with results from the literature. The following parameters were compared: technical success, pain relief and recurrence, and complications. Balloon compression is considered in the literature to be a safer procedure than other percutaneous surgeries, especially for postoperative sensitive disorders…

Glossopharyngeal neuralgia as onset of multiple sclerosis.

OBJECTIVES AND METHODS: Glossopharyngeal neuralgia is a painful condition, affecting the ninth cranial nerve, rarely described in the course of multiple sclerosis. Here we describe a case of multiple sclerosis presenting with glossopharyngeal neuralgia. RESULTS AND DISCUSSION: We suggest the presence of demyelinating areas at the nerve root entry zone as principal trigger mechanism.

[Treatment of herpes zoster and postherpetic neuralgia].

Herpes zoster, a biphasic disease with different kinds of pain is a challenge for pain therapy. The acute illness with mixed pain (nociceptive - neuropathic) requires antivirals for risk patients and pain treatment according to rules of acute pain therapy. For treatment of postherpetic neuralgia (PHN) an example of neuropathic pain, antidepressants, anticonvulsants and topical lidocaine are today the first line therapeutics, further opioids are of a certain therapeutic value.

Successful treatment of trigeminal neuralgia with milnacipran.

Paroxysmal pain in a 64-year-old woman diagnosed with trigeminal neuralgia disappeared with the administration of carbamazepine, but carbamazepine had to be discontinued because of intolerable lassitude and liver dysfunction. Afterward, the paroxysmal pain reoccurred, and depressive symptoms appeared. Milnacipran was then administered at a dosage of 50 mg/d for 2 months, and the paroxysmal pain and depression disappeared completely. Carbamazepine is the drug of first choice for trigeminal neuralgia, but the present results suggest that milnacipran is worth investigating for patients who do not…

Management of herpes zoster and postherpetic neuralgia.

Patients with herpes zoster experience severe pain and potential lasting complications such as postherpetic neuralgia, ophthalmic disease/damage, and, rarely, skin complications (eg, infection of rash area). Treatment for acute zoster aims to accelerate healing, control pain, and, when possible, reduce the risk of complications. Early intervention with antivirals can accelerate rash healing, reduce rash severity, and reduce the risk of some complications. The addition of corticosteroids to antiviral medication may further alleviate short-term zoster pain, but is associated with an increased ri…

Topical lidocaine for the treatment of postherpetic neuralgia.

BACKGROUND: The cause of postherpetic neuralgia is damage to peripheral neurons, dorsal root ganglia, and the dorsal horn of the spinal cord, secondary to herpes zoster infection (shingles). In postherpetic neuralgia, peripheral neurons discharge spontaneously and have lowered activation thresholds, and exhibit an exaggerated response to stimuli. Topical lidocaine dampens peripheral nociceptor sensitisation and central nervous system hyperexcitability, and may benefit patients with postherpetic neuralgia. OBJECTIVES: To examine the efficacy and safety of topical lidocaine in the treatment of …

Nonidiopathic trigeminal neuralgia: report of a case.

Trigeminal neuralgia, which may involve one or more branches of the trigeminal nerve, is considered to produce one of the most severe types of pain. As it may be associated with other conditions, in some cases, the patient is subjected to unnecessary treatment. A case of nonidiopathic trigeminal neuralgia associated with internal carotid aneurysm, is presented.

[Patient careers. Facial pain and neuralgias].

The correct diagnosis in patients with facial pain is essential to avoid erroneous diagnostic and therapeutic measures. Trigeminal neuralgia is easily recognized by the typical history given by the patient. Treatment of first choice is carbamazepine. When pharmacological treatment fails, surgical options are available. Tolosa Hunt syndrome is judged to be caused by a granulomatous infiltration in the cavernous sinus, treatment consists in corticosteroids. Painful craniomandibular disorders are often mistaken for 'atypical facial pain'. Atypical facial pain, now called 'persistent idiopathic fa…

Evaluation of surgical procedures for trigeminal neuralgia.

Trigeminal neuralgia is a type of facial pain that is difficult to treat. The pain can be excruciating and debilitating. The wide range of treatments currently used for trigeminal neuralgia is ample evidence that there is no simple answer to how it should be managed. This review will evaluate the current surgical procedures used for the treatment of trigeminal neuralgia. A critical analysis of the evidence-based studies to date was done to evaluate and compare the efficacy of the different surgical procedures. Arguments for and against the use of surgery for trigeminal neuralgia are presented.…

Pathophysiology of trigeminal neuralgia: the ignition hypothesis.

There are no satisfactory animal models of trigeminal neuralgia, and it is difficult to obtain essential data from patients. However, trigeminal neuralgia presents with such idiosyncratic signs and symptoms, and responds to so distinctive a set of therapeutic modalities, that scientific deduction can be used to generate likely hypotheses. The ignition hypothesis of trigeminal neuralgia is based on recent advances in the understanding of abnormal electrical behavior in injured sensory neurons, and new histopathologic observations of biopsy specimens from patients with trigeminal neuralgia who a…

Reduction of postherpetic neuralgia in herpes zoster.

BACKGROUND: Persons 50 years of age and older are not only at increased risk of developing herpes zoster, they are also more likely to suffer the long-term morbidity of postherpetic neuralgia (PHN). PHN is pain persisting after the rash of herpes zoster has healed. PHN affects at least 40% of all herpes zoster patients over age 50 and over 75% of herpes zoster patients over age 75; PHN is the single most common neurologic condition in elderly patients. OBJECTIVE: The objective of this review is to evaluate interventions that may reduce or even eliminate PHN. No single therapy has been consist…

Amitriptyline and fluphenazine in the treatment of postherpetic neuralgia.

BACKGROUND: Postherpetic neuralgia (PHN) is a vexing problem occurring in 10 to 20 percent of people with from herpes zoster (shingles). Anecdotal reports show that fluphenazine enhances the effects of amitriptyline for the treatment of PHN. The aim of this study was to determine, in a controlled manner, whether this was the case. METHODS: In a double-blind placebo-controlled study, 49 patients with PHN were randomly assigned to four treatment groups: Group 1, amitriptyline; Group 2, amitriptyline and fluphenazine; Group 3, fluphenazine; Group 4, a placebo. An active placebo was used to mimic…
