Normal pressure hydrocephalus: did publications alter management?

Forty six Dutch neurologists and neurosurgeons were interviewed to evaluate the clinical value of research articles on normal pressure hydrocephalus (NPH). From this survey it appears that most clinicians still limit investigations to psychometry, CSF-tap test(s), and cisternography. The main reasons for not using other techniques were: their invasiveness, technical complexity, poor availability, lack of time and doubt on their additional predictive value. There is an obvious discrepancy between the quantity of publications on NPH and their impact and their ability to assist clinicians in sele…

Common hand problems.

The principles illustrated in this brief survey are germane to all surgery. They cross whatever boundaries exist between specialties; in the hand, there are no divisions into parts for general, plastic, orthopedic, or neurosurgeons. They are essential to the success and parallel Boyes' description of Bunnell's influence in "the restoration of function, a careful detailed evaluation of the problem and a skillful manipulation of the parts, to accomplish the planned result. Solutions were sought and planned by thinking in broad terms of function, but the surgery is executed with maximum attention…

Embracing Exoscope in the Arsenal of Spine Surgeons in the Low- and Middle-Income Countries.

BACKGROUND: Exoscope (EX) is a device that combines the convenience of an endoscope with the image clarity of an operating microscope (OM) to fill the void between the two. This study aims to compare the 2-dimensional EX with OM in spine surgeries and to explore its utility and feasibility in small and peripheral hospitals of low- and middle-income countries. METHODS: Eighty-two patients with intradural spinal tumors (extramedullary and intramedullary) aged more than 18 years were included between August 2021 and August 2023. Patients with other spinal pathologies were excluded. After each ex

Neurosurgical Work during the Napoleonic Wars: George James Guthrie's Experience.

Involved in what is still considered, along with the two world wars of the 20th century, as one of the major conflicts in Europe, George James Guthrie (1785-1856) was the most famous English army surgeon of the Napoleonic wars. After treating the injured throughout the Peninsular Campaign (1808-1814), in 1815 and then in 1842 he published two major books dealing with cranial and brain injuries, among other topics. In these books, we can find, for example, an early description of the plantar reflex further described by Joseph Babinsky, accurate descriptions of the clinical signs of intracranial

Infrastructural Limitations in Establishing Neurosurgical Specialty Services in Liberia.

Introduction Liberia recently employed the first neurosurgeon in the country's history. In a country with a population of 4.7 million people and staggering rates of cranial and spine trauma, as well as hydrocephalus and neural tube defects, neurosurgery is considered a luxury. Our study documents the experience of a team of neurosurgeons, critical care nurses, scrub technicians, nurses, and biomedical engineers who carried out a series of neurosurgical clinics and complex brain and spine surgeries in Liberia. Specifically, we aim to highlight some of the larger obstacles, beyond staff and equi…

Understanding barriers to pediatric hydrocephalus management: an international survey.

PURPOSE: A pediatric neurosurgeon's approach to the treatment of hydrocephalus can vary but often includes ventriculoperitoneal (VP) shunt insertion or endoscopic third ventriculostomy (ETV). However, there are several suspected barriers to accessible hydrocephalus care, particularly regarding endoscopy. The aim of this study was to characterize neurosurgical approaches and perceived barriers to pediatric hydrocephalus treatment globally. METHODS: An online survey targeting a non-random sample of pediatric neurosurgeons was distributed via international neurosurgery mailing lists. Demographic…

Surgical management of lumbar spinal stenosis: a survey among Norwegian spine surgeons.

BACKGROUND: Symptomatic lumbar spinal stenosis (LSS) is the most common reason for lumbar surgery in the elderly. There is growing evidence that decompressive surgery offers an advantage over non-surgical management for selected patients with persistent severe symptoms. Based on treatment traditions, open laminectomy has been the gold standard surgical treatment, but various other surgical and non-surgical treatments for LSS are widely used in clinical practice. Therefore, we conducted a survey study to capture potential diversities in surgeons' management of LSS in Norway. METHODS: All spine

[Multiple splenic abscesses associated with normotensive hydrocephaly].

The paper contains a concise review of the current literature on the splenic abscesses and presents an unusual association of multiple splenic abscesses with normotensive hydrocephaly to an elderly patient, treated by ventriculo-cardiac shunting and splenectomy as an one stage procedure performed by a combined surgical team (neurosurgeon, vascular surgeon and general surgeon), with a favourable outcome. As far as the authors know, this is the first communication of multiple splenic abscesses associated with adult hydrocephaly treated by a simultaneous surgical approach.

Mechanisms of visual loss in papilledema.

The term "papilledema" indicates swelling of the optic discs secondary to increased intracranial pressure. Papilledema can be caused by an intracranial mass lesion or by other factors. Visual symptoms frequently accompany papilledema, which can lead to permanent visual loss if left untreated. Starting with an illustrative case, the authors review the pathophysiology of the visual signs and symptoms of papilledema. They also briefly review potential treatment options, focusing on the role of the neurosurgeon in the treatment of patients with papilledema.

Suction Pressure Control Valve for Microneurosurgery.

A suction pressure control valve (SPCV) for microneurosurgery is designed to place control of safe suction pressure in the hands of an operating neurosurgeon. Tumor tissue needs more negative suction pressure as compared to normal brain tissue, cranial nerves, and vessels in the neurosurgical operating field. The authors present the details of a prototype of a simple and inexpensive device for dynamic adjustment of negative suction pressure by the surgeon or the assistant while working on different tissue types during microneurosurgery.

Cadaveric findings of persistent fetal trigeminal arteries.

Persistence of fetal intracranial arteries is rare. We review this anatomical literature regarding the persistence of the trigeminal artery and discuss the findings in two cadaveric cases both found to harbor this vessel. This anomaly should be considered by the clinician and anatomist who view imaging of the skull base. Additionally, the relationships between such an artery and other structures within the cavernous sinus should be understood by the neurosurgeon who operates in this region, e.g., transsphenoidal approaches to the skull base.

The costovertebral angle.

Because the anatomy of the costovertebral angle is complex and often unfamiliar to the operating thoracic surgeon, surgical procedures performed in that area must be performed by a surgical team rather than by individual surgeons, and such a team usually includes either an orthopedic surgeon or a neurosurgeon. This is the case, for example, of Pancoast's tumors invading the roots of the brachial plexus or the spine itself where the help of an orthopedic surgeon is invaluable not only to achieve a complete resection but also to prevent catastrophic complications.

Normal-pressure hydrocephalus: twelve years experience.

The authors report on twelve years experience with so-called normal pressure hydrocephalus. If care is taken to make a correct diagnosis and give adequate treatment with a shunt, hydrocephalic dementia has a good prognosis. Such cases should be distinguished from other syndromes, particularly senile dementia and Alzheimer's disease. The occurrence of a syndrome of dementia should compel every neurologist and neurosurgeon to carry out careful investigations in order to determine the favourable time for possible treatment.

Nosocomial transmission of a strain of Staphylococcus aureus causing toxic shock syndrome.

A strain of Staphylococcus aureus producing toxic shock syndrome toxin-1 was repeatedly isolated from the nares of a neurosurgeon. This strain was identical to strains cultured from two of his patients who developed toxic shock syndrome after laminectomy. The relatedness of the isolates was shown by Southern blot hybridization analyses using chromosomal transposons as probes. This approach should be considered, in addition to standard bacteriologic techniques, as an effective method to analyze the relatedness of nosocomial isolates.

[Neurosurgical indications in craniofacial injuries].

The cerebral complications of cranio-facial trauma require urgent neurosurgical assessment and intervention. Apart from such cases, the cooperation of the neurosurgeon is also necessary when the anterior part of the base of the skull must be explored. This may be necessary either in the presence of a cerebrospinal fluid leak with rhinorrhoea or because the reduction and fixation of orbito-facial fractures requires an intracranial approach. Early treatment, at the same time, of all cranial, basal and orbito-facial lesions is recommended.
