Evolution of Delivery Room Management for Meconium-Stained Infants: Recent Updates.

BACKGROUND: The approach to intrapartum and postnatal management of an infant born through meconium-stained amniotic fluid (MSAF) in the delivery room (DR) has changed several times over the last few decades, leading to confusion and anxiety among health care providers (nurses, nurse practitioners, respiratory therapists, midwives, and physicians). This article provides state-of-the-art insight into the evidence or lack thereof for the changes in guidelines. PURPOSE: To discuss the evidence for evolution of DR management of vigorous and nonvigorous infants born through any type of MSAF. METH

Using the RETAIN Tabletop Simulator as a Summative Assessment Tool for Neonatal Resuscitation Healthcare Professionals: A Pilot Study.

Background: Frequent and objective summative assessment of neonatal healthcare providers is important to ensure high-quality care to patients during neonatal resuscitation. Currently, neonatal resuscitation providers are only individually assessed using an at-home online multiple-choice questionnaire. While simulation-based assessment is preferred, resource constraints limit its widespread uptake. An alternative approach to simulation-based summative assessment is needed. Simulation-based serious games may provide a solution. Objective: The aim of this study was to examine if individual perfor

Neonatal jaundice detection in low-resource Mexican settings: possibilities and barriers for innovation with mobile health.

BACKGROUND: Neonatal jaundice is a common condition that can lead to brain damage and disabilities when severe cases go undetected. Low- and middle-income countries often lack accurate methods for detecting neonatal jaundice and rely on visual assessment, resulting in a higher incidence of adverse consequences. Picterus Jaundice Pro (Picterus JP), an easy-to-use and affordable smartphone-based screening device for the condition, has demonstrated higher accuracy than visual assessment in Norwegian, Philippine and Mexican newborns. This study aimed to identify the barriers and facilitators to im

A Novel Method for Administering Epinephrine during Neonatal Resuscitation.

OBJECTIVE: This study aimed to determine if prefilled epinephrine syringes will reduce time to epinephrine administration compared with conventional epinephrine during standardized simulated neonatal resuscitation. STUDY DESIGN: Timely and accurate epinephrine administration during neonatal resuscitation is lifesaving in bradycardic infants. Current epinephrine preparation is inefficient and error-prone. For other emergency use drugs, prefilled medication syringes have decreased error and administration time. Twenty-one neonatal intensive care unit nurses were enrolled. Each subject engaged i

Predictive Value of C-Reactive Protein/Albumin Ratio for Necrotizing Enterocolitis in Preterm Infants With Neonatal Sepsis.

BACKGROUND: Necrotizing enterocolitis (NEC), a critical neonatal digestive disease, is recognized as a contributing factor to neonatal sepsis. However, the role of the C-reactive protein/albumin ratio (CAR) in diagnosing neonatal sepsis within the context of NEC remains under-researched. AIM: Our objective was to evaluate the clinical utility of the C-reactive protein/albumin (CRP/ALB) ratio in predicting neonatal sepsis at the NEC diagnosis stage. STUDY DESIGN: In this retrospective analysis, we evaluated 148 NEC cases, utilizing data extracted from electronic medical records. The relations

High-risk pregnancy and neonatal care.

The delivery of high-risk maternal and neonatal homecare has been shown to be a cost-effective model of care delivery. The homecare management of preterm labor, placenta previa. PIH, and hyperemesis gravidarum has proven to be a safe interventional strategy with successful clinical outcomes. Studies of home uterine activity monitoring and phototherapy have demonstrated that the use of advanced technologic devices is spearheading the drive to reduce hospital lengths of stay, enabling the safe return of the patient to the home setting, under the watchful supervision of the high-tech homecare nur

Differential Effects of Breed and Nursing on Early-Life Colonic Microbiota and Immune Status as Revealed in a Cross-Fostering Piglet Model.

Nursing mother and breed can differently regulate early-life microbiota succession in pigs. However, it remains unclear whether they affect gastrointestinal microbiota and immune status, which are critical for early-life gut health. Here, an interspecific cross-fostering piglet model was employed by fostering neonatal Yorkshire and Meishan piglets to the same or another breed of sows. Jejunal and colonic microbiotas and mucosal immune parameters were analyzed at postnatal days 14 (preweaning) and 49 (postweaning). Nursing mother affected 10 genera in the colon and 3 minor genera in the jejunum

Ethical issues in the delivery room: resuscitation of extremely low birth weight infants.

The neonatal team attending high-risk deliveries is often faced with difficult ethical decisions concerning aggressive cardiopulmonary resuscitation for infants of 22-24 weeks gestational age (GA). These decisions are often based on the ethical principles of beneficence, nonmaleficence, justice, futility, autonomy, quality of life, and best interests and legal rights of the infant. Three delivery room recommendations are discussed along with their legal and ethical considerations. The perinatal and neonatal intensive care nurse must understand the moral, ethical, legal, and professional respon

[The assistance of the ventilated infant: role of the nurse in the management of the endotracheal tube].

One of the principal duties of a staff nurse working in a NICU is the assistance of the ventilated infants. The management of the endotracheal tube is a key moment in the neonatal care and it is fundamental to prevent the ventilator induced lung injury and the change in the perfusion (especially in the brain) that may determine an impairment of gas exchange. Therefore, the role of the staff nurse is to guarantee that the tube remains correctly placed and firmly fixed in order to avoid tubes displacement and accidental extubations. High production of bronchial secretion may lead to a tube obstr

Congenital bilateral choanal atresia.

Eight neonatal patients with bilateral congenital choanal atresia were treated by endonasal perforation with a curved trocar. In two of these the atresia plate was perforated under direct vision using a 0 degree Hopkins rod endoscope. A stenting technique is described, the aim of which is to provide good fixation and easy postoperative nursing care. Of the cases, seven did not require further treatment. A single case required further dilatation on one side. The trans-nasal endoscopic technique and the described method of stenting is a safe, quick and efficient procedure for the correction of c

Optimising neonatal bubble continuous positive airway pressure: A Somaliland quality initiative.

BACKGROUND: Continuous positive airway pressure (CPAP) therapy is the standard of care for neonatal respiratory distress and improves survival when implemented in low-resource settings. Clinical audits at the Borama Regional Hospital (BRH) Neonatal Intensive Care Unit (NICU) revealed multiple barriers to effective CPAP, including insufficient pressure, a lack of neonatal-sized nasal prongs, and patient interface challenges. AIM: Improve respiratory distress by increasing effective CPAP delivery for neonates 30 days of age from 52% to 90% in 6 months. SETTING: Single-centre referral hospital

Task-shifting for lactation support to enhance mother's own milk use in neonatal intensive care unit: a quality improvement initiative.

Mother's own milk improves outcomes for sick and preterm neonates, yet its use in many neonatal intensive care units (NICUs) remains suboptimal. This quality improvement study aimed to increase proportionate consumption of expressed breast milk (EBM) in the NICU and exclusive (EBF) and any breastfeeding (ABF) at discharge by ≥20% above baseline within 12 weeks using sequential Plan-Do-Study-Act (PDSA) cycles and sustain gains thereafter. Root-cause analysis identified poor role clarity in lactation support. In the first implementation phase, neonates receiving <50% EBM were identified during t

Inter-observer reliability in reading amplitude-integrated electroencephalogram in the newborn intensive care unit.

AIM: Amplitude-integrated electroencephalogram (aEEG) is widely used in neonates to detect electrical seizure and predict outcome following hypoxic ischaemic encephalopathy and other encephalopathy. Because accurate interpretation is important for clinical decision-making and family counselling, inter-observer reliability is a major concern. We aimed to evaluate inter-observer reliability in the interpretation of aEEG tracings in a neonatal intensive care unit. METHODS: Three neonatal intensive care unit clinicians with different levels of experience in reading aEEG independently reviewed and
