Identification of an oculomotor biomarker of preclinical Huntington disease.

The authors examined oculomotor function to identify a biomarker of disease progression in genetically confirmed preclinical and early clinical Huntington disease (HD). Initiation deficits of voluntary-guided, but not reflexive, saccades were characteristic of preclinical HD. Saccadic slowing and delayed reflexive saccades were demonstrated in clinical but not preclinical HD. Saccadic measures provide biomarkers of disease progression in both preclinical and early clinical stages of HD.

[Clinical and MRI findings of patients with internuclear ophthalmoplegia].

Internuclear ophthalmoplegia (INO) is a distinctive ocular motor disorder resulting from dysfunction of the medial longitudinal fasciculus, which lies in the pontine tegmentum. We retrospectively analyzed clinical and magnetic resonance imaging (MRI) findings for four consecutive patients with internuclear ophthalmoplegia who were treated in our hospital. The causes of the disease were cerebral infarction in three cases and multiple sclerosis in one case. Vertigo and facial nerve palsy were associated in three cases and one case, respectively. MRI studies visualized an ischemic lesion in the r

Lateral gaze disturbance due to cerebral microbleed in the medial lemniscus in the mid-pontine region: a case report.

A 72-year-old man who was attending the outpatients clinic in our hospital because of multiple lacunar cerebral infarctions and mild mental disturbance developed an inability to gaze to the left side voluntarily. MR imaging revealed a new cerebral microbleed in the medial lemniscus of the right side in the mid-pontine region. The finding in this case suggests that the pathways from the frontal eye field descend at least to the level of the mid-pontine region ipsilaterally, and that the pathways then contralaterally decussate and terminate in the paramedian pontine reticular formation. Cerebral

[Clinical signs of dysthyroid orbitopathy].

Dysthyroid ophthalmopathy is an inflammatory disease of the orbit, combined or not with thyroid disorders. However, pure ophthalmologic forms exist, and the diagnosis can be made by the ophthalmologist. The diagnosis is made by examining eyelid malposition, oculomotor disorders, inflammatory signs, proptosis measurement, and by the search for optic nerve pain. We discuss the NOSPECS classification, which can evaluate the severity and progression of the disease.

Vertebro-basilar syndromes causing oculo-motor disorders.

PURPOSE OF REVIEW: All functional classes of eye movements require exquisite coordination between cortical, basal ganglia and brainstem centers involved in ocular motor control. Vertebrobasilar stroke may produce a wide spectum of isolated or combined eye-movement disorders. The intent of this article is to summarize the curent knowledge of eye-movement disorders occurring in infarcts involving the different arterial territories of the vertebrobasilar circulation. RECENT FINDINGS: In the last few years there has been an improvement in our understanding of pathophysiological mechanisms that ar

Internuclear ophthalmoplegia and cerebellar ataxia: report of one case.

Deep hemispheric or brainstem small infarcts can lead to atypical lacunar syndromes. Unilateral internuclear ophthalmoplegia (INO) and cerebellar ataxia has not been reported previously. A 57-year-old hypertensive female presented with bilateral appendicular and left truncal cerebellar ataxia and right INO. Cranial MRI showed a right paramedian infarct of lacunar size located in the tegmentum of caudal mesencephalon. At this level the involvement of medial longitudinal fascicle (MLF) led to right INO and the lesion of brachium conjunctivum caused the bilateral cerebellar ataxia. Ipsilateral in

Effect of steroid and high-dose immunoglobulin therapy on opsoclonus-myoclonus syndrome occurring in neuroblastoma.

The authors describe a case of an 8-month-old boy with opsoclonus-myoclonus syndrome (OMS) and coincident unresectable neuroblastoma (NB). He achieved a complete remission for NB after 6 courses of standard-dose chemotherapy without significant neurological improvement despite the use of steroids and high-dose immunoglobulin (HIG), administered separately. Only the combined treatment withthese two drugs induced a complete disappearance of neurological symptoms. On the basis of this experience, the authors suggest the association of steroids plus HIG for the treatment of OMS in patients not res

[Pretectal syndrome caused by a plaque of multiple sclerosis].

We report a 20-year-old woman who presented with pretectal syndrome. She was well until September 12, 1991 when she noted an onset of difficulty in focusing her eyes. On neurologic examination, she showed convergence-retraction nystagmus and restriction of vertical eye movements more in the upward direction. A cranial CT scan revealed no abnormality. An oligoclonal band was detected in CSF. An MRI using Hitachi MRH-500 (0.5 T) revealed an abnormal high signal intensity lesion at the ventral area of the midbrain aqueduct, and another small lesion in the temporal white matter on the left. In add

Opsoclonus in malaria: the first report in the literature.

The first report in the literature of opsoclonus in malaria was presented. A 24-year-old woman had a two week history of high fever and lassitude. Physical examination revealed no specific neurological sign on admission. Ring form and gametocytes of Plasmodium falciparum were found in blood smear. Combined treatment of quinine and tetracycline were prescribed. She developed classical abnormal eye movement of opsoclonus and hand tremor on the following day, which gradually subsided on the tenth hospital day. Computed brain tomography and cerebrospinal fluid study were normal. She was discharged

Horner Syndrome in Giant Cell Arteritis: Case Series and Review of the Literature.

BACKGROUND: Giant cell arteritis (GCA) is a systemic inflammatory vasculitis that affects medium- and large-sized arteries and can result in permanent vision loss. In rare instances, Horner syndrome has been noticed at the time of GCA diagnosis, although the mechanism of both diagnoses occurring at the same time is not entirely understood. We reviewed 53 charts of all patients diagnosed with biopsy-proven GCA in tertiary neuro-ophthalmology practice to find patients who presented with new onset of Horner syndrome at the time of GCA diagnosis. METHODS: Two patients with biopsy-confirmed GCA wh

Vision Problems and Reduced Reading Outcomes in Queensland Schoolchildren.

PURPOSE: To assess the relationship between vision and reading outcomes in Indigenous and non-Indigenous schoolchildren to determine whether vision problems are associated with lower reading outcomes in these populations. METHODS: Vision testing and reading assessments were performed on 508 Indigenous and non-Indigenous schoolchildren in Queensland, Australia divided into two age groups: Grades 1 and 2 (6-7 years of age) and Grades 6 and 7 (12-13 years of age). Vision parameters measured included cycloplegic refraction, near point of convergence, heterophoria, fusional vergence range, rapid a

Joubert Syndrome and related disorders.

Joubert syndrome (JS) and related disorders (JSRD) are a group of developmental delay/multiple congenital anomalies syndromes in which the obligatory hallmark is the molar tooth sign (MTS), a complex midbrain-hindbrain malformation visible on brain imaging, first recognized in JS. Estimates of the incidence of JSRD range between 1/80,000 and 1/100,000 live births, although these figures may represent an underestimate. The neurological features of JSRD include hypotonia, ataxia, developmental delay, intellectual disability, abnormal eye movements, and neonatal breathing dysregulation. These may

Visual facial grasp.

Some patients with degenerative neurological diseases have a release of the vestibular-ocular reflex (VOR), as detected by passive head movement during visual fixation on a moving target ("doll's eyes"maneuver). However, a positive doll's eyes sign might be induced by other defects and the purpose of this article is to describe a new ocular sign of cortical dysfunction, the visual facial grasp. We observed three patients, one with progressive supranuclear palsy (PSP), another with probable Alzheimer's disease (AD) and a third with cortico-basal degeneration (CBD) all of whom appeared to demons

[The Oculomotor, Trochlear, and Abducens Nerves].

Saccade and vestibular eye movements, which direct and maintain gaze towards an object, respectively, are produced by different neural mechanisms. The oculomotor, trochlear, and abducens nerves work together to move the extraocular muscles to achieve these eye movements. Recognizing eye movement disorders that occur with disorders of each nerve is useful towards the diagnosis and localization of the insulting lesion. Disorders in the central neural mechanisms of saccade and vestibular eye movements also cause abnormalities in eye movements, making it necessary to understand the eye movement ab
