UNLABELLED: An 89-year-old man presented with right elbow pain after a low-energy fall. At age 2 years, he sustained an untreated distal humeral fracture dislocation, which resulted in progressive remodeling and formation of a functional pseudoarticulation. Despite permanent deformity, he maintained stable functional motion for decades and performed heavy manual labor. Imaging revealed a displaced distal humeral fracture through the chronically remodeled elbow. Open reduction and internal fixation with a variable-angle locking plate was performed, while the pseudoarticulation was intentionally

BACKGROUND: Fracture-related infection (FRI) and subsequent persistent infections significantly affect fracture surgeries. We aimed to develop a precise, personalized risk calculator to assist orthopedic surgeons in evaluating perioperative FRI risk.
METHODS: Data from 36,087 patients across two medical centers and four regional hospitals were analyzed. We assessed 29 risk factors, including patient characteristics, comorbidities, fracture location, and surgical variables using multivariable logistic regression. Each factor was weighted based on its regression coefficient. Discrimination and

Background Radial neck fractures in children are rare injuries, representing 1 to 5% of all elbow pediatric fractures. Most of them are non-displaced or slightly displaced and treated conservatively. Severely displaced or angulated radial neck fractures (Judet type III and IV fractures or O'Brien type III radial neck fractures) requires surgical treatment. Objective To study the clinical and radiological outcomes of fractures following closed or open reduction and percutaneous intramedullary stabilization of the displaced radial neck fracture in children. Method There were 24 children with dis

It is well-known that late median nerve neuropathy can occur after open reduction and internal fixation (ORIF) of distal radius fracture (DRF). The current study investigated the predictive factors of late median nerve neuropathy after ORIF with a volar locking plate for DRF. The authors retrospectively reviewed 712 patients who underwent ORIF using a volar locking plate after DRF at 3 medical institutions between 2006 and 2017. Thirty-seven (5.2%) patients developed late median nerve neuropathy at a mean of 8.25±3.47 months (range, 3-19 months) after surgery. The radiographic data of 37 patie

INTRODUCTION: Open reduction with internal fixation (ORIF) and intramedullary nailing (IMN) have similar union rates for treating humerus shaft fractures, but IMN leads to increased incidence of shoulder impingement and reoperation. The difference in 30-day readmission rate and length of stay (LOS) between these procedures is unknown. The objective of the study was to compare 30-day unplanned readmissions and length of stay between humeral shaft fractures fixed with either ORIF or IMN.
METHODS: The nationwide readmissions database (NRD) was queried for patients with humeral shaft fractures tr

OBJECTIVE: High-grade pediatric supracondylar humerus fractures are commonly treated with closed reduction and internal fixation with percutaneous pinning. When this fails, open reduction followed by internal fixation is the widely accepted procedure of choice. Use of a lateral external fixator was recently described as an optional procedure, but evidence is scarce.
METHODS: We investigated the outcomes of upper limbs treated by either open reduction with internal fixation or closed reduction and external fixation.
RESULTS: Twenty-one patients completed the long-term follow-up; 11 underwent

This study prospectively evaluated closed reduction (CR) outcomes in non-displaced, non-dislocated high-condylar and condylar-head fractures (Class VI after Spiessl and Schroll) and open reduction and internal fixation (ORIF) of displaced (Class III) or dislocated (Class V) fractures. Thirty-eight patients with 54 fractures (16 (42%) with bilateral fractures, 14 (37%) CR, 24 (63%) ORIF) were enrolled in a 1 year follow-up that 18 patients with 33 fractures completed. Condylar translation in Class VI fractures recovered to 11 mm for vertical opening, 8mm for protrusion and 10 mm for mediotrusio

Distal radius fractures are common in elderly patients and often lead to functional impairment. This study compared closed reduction with external fixation (EF) and open reduction with locking plate fixation (LP) in elderly patients. A retrospective analysis was conducted on 80 elderly patients with unilateral distal radius fractures treated in our hospital from 2021 to 2024. Patients were divided into a closed reduction and EF group (n = 40) and an open reduction and locking plate internal fixation group (n = 40), according to the surgical procedure. General demographic and fracture-related d

BACKGROUND: Periprosthetic femoral fractures are associated with significant morbidity, mortality, social and economic cost. The incidence of these fractures is expected to increase with an ever-growing elderly world-population. The complex nature and varied pattern of these injuries requires a range of specialized surgical techniques and tools. Fixation alone is being increasingly regarded as the preferred method of addressing these fractures, even in cases where the femoral stem is unstable, showing favourable outcomes overall when compared to a fix-and-replace approach. Lateral plate fixati

Fractures of the mandibular condyle account for a significant proportion of mandibular fractures. The specific functional loads require particular specifications for the implant design used for open reduction and internal fixation of such fractures. The clinical and radiographic outcomes in patients treated using a single rhombic 3D condylar fracture plate for open reduction and internal fixation at a single institution, and who fulfilled the inclusion and exclusion criteria, are presented. The primary outcome variables were: occlusion, maximum interincisal distance and mandibular excursion at

BACKGROUND: Acetabular fractures in geriatric patients are challenging due to poor bone quality and increased risk of postoperative complications. While open reduction and internal fixation (ORIF) remains a common approach, combining ORIF with total hip arthroplasty has emerged as an alternative and is referred to as the combined hip procedure (CHP). This study aimed to compare clinical, radiological, and functional outcomes between CHP and ORIF in patients aged over 65 years.
METHODS: In this prospective, randomized, single-blind clinical trial, aged > 65 years with acetabular fractures were
