The "Opercular" Approach to Orbital Reconstruction after Orbital Exenteration Following Rhino-Orbital-Cerebral Mucormycosis: A Novel Method.

Background  Rhino-orbital cutaneous mucormycosis (ROCM) leading to orbital exenteration can be debilitating functionally as well as psychosocially. Orbital reconstruction following exenteration for mucormycosis has centered on volume filling with local, regional, or free flaps. This case series is built on the original idea of a bilayered operculum at the orbital inlet. Materials and Methods  The opercular approach comprises an inner layer of hinged orbital mucosa and an outer layer of nasofacial flap in most cases, or alternatives, for lining or cover. Results  Eight cases of ROCM treated usi…

Correlation between increased orbital volume and enophthalmos and diplopia in patients with fractures of the orbital floor or the medial orbital wall.

Fractures of the orbital floor and medial orbital wall (blowout fractures) are common midface injuries. Diagnostic methods and treatment options have improved over recent years, due to threedimensional image processing and the use of patient-specific implants. Nonetheless, the indications for orbital reconstructive surgery are still controversial. Previous authors have reported a linear correlation between post-traumatic increases in orbital volume and enophthalmos and diplopia. Post-traumatic increases in orbital volume are often considered to be predictive of long-term symptoms; therefore, i…

Orbital Adherence Syndrome following the Use of Titanium Precontoured Orbital Mesh for the Reconstruction of Posttraumatic Orbital Floor Defects.

Orbital blowout fractures are a common occurrence following orbital trauma. Depending on the size of the defect and the contents that have herniated or incarcerated, possible sequelae include enophthalmos, diplopia, dystopia, and entrapment. Surgical intervention aims to prevent or alleviate this through the use of a bone graft or an alloplastic implant to reconstitute the continuity of the orbit. However, in doing so, the implant itself may result in the unexpected adherence of the periorbita, resulting in orbital adherence syndrome. We present two cases of orbital adherence syndrome followin…

Bacterial orbital infection leading to orbital exenteration: a case series.

Orbital exenteration (OE) secondary to infection is rare. This case series reports 3 cases and aims to elucidate the possible underlying etiopathogenesis. A retrospective data analysis was performed of patients who underwent an OE and subsequently received an orbital prosthesis from the Dutch Institute for Facial Prosthetics between 2017 and 2024. Only cases secondary to infection were included. Of the 44 cases treated, only 3 were infection related. An odontogenic infection was the underlying cause in 2 cases, a periorbital/orbital necrotizing fasciitis due to a skin infection in the third ca…

IgG4-related Orbital Disease Presenting as an Orbital Apex Lesion.

IgG4-related disease is an uncommon multisystem inflammatory disease characterized by tissue infiltration by IgG4 plasma cells, extensive fibrosis, and sclerosing inflammation. Diffuse orbital involvement extending to the orbital apex and cavernous sinus has been reported, but a solitary well-circumscribed lesion at the apex has not been previously reported. Herein, we report a unique case of IgG4-related orbital disease presenting as a solitary well-circumscribed orbital apex lesion causing subacute visual decline in a 45-year-old male. MRI demonstrated bilateral lacrimal gland and infraorbit…

Orbital Lymphoma Presenting As Recurrent Orbital Cellulitis: A Diagnostic Challenge.

Orbital cellulitis is an infection of the orbital tissue behind the orbital septum. We report a case of a 71-year-old Malay lady, a passive smoker for 20 years, presenting with recurrent orbital cellulitis at the same site. She initially presented with right periorbital swelling, redness, and reduced vision worsening over two weeks, along with a firm swelling over the right temple. CT of the brain and orbit revealed a homogenous mass extending from the right sphenoid bone to the right orbit. The initial diagnosis was right sphenoid meningioma or metastasis. Her symptoms improved after 10 days …

Targeted orbital intervention in the management of sino-orbital mucormycosis cases.

INTRODUCTION: Many countries from South-East Asia reported an epidemic of sino-orbital mucormycosis (SOM), otherwise a rare disease, during the coronavirus disease 2019 pandemic. SOM, a potentially fatal disease, is typically treated with orbital exenteration and systemic antifungals after metabolic stabilization. There is no clear evidence of survival benefit of exenteration in the literature, and thus, there have been attempts at globe conserving treatments like orbital infusion after limited debridement and intraorbital injections with Amphotericin B (IOAB). METHODS: We conducted a prospec…

Orbital Aging: A Computed Tomography-Based Study of 240 Orbits.

BACKGROUND: Previous studies have attempted to explain age-related changes to the orbit in isolation, often producing conflicting results. The authors used highly accurate imaging software to analyze computed tomographic scans to characterize changes related to age objectively. METHODS: In this case-control study, patients seen in an ear, nose, and throat clinic were screened for study entry. Male and female participants were divided into two age groups (20 to 30 years and 60 to 75 years). Primary outcomes included measurement of bony orbital dimensions, volume of soft tissues (muscle and fat…

Unusual association of orbital tumour and rhino-orbital-cerebral mucormycosis.

In developing tropical countries, rhino-orbital-cerebral mucormycosis has been a cause of severe morbidity and mortality during the COVID-19 pandemic. Classically, it develops as an aggressive angioinvasive destruction of nasal, orbital and cerebral involvement. Blindness is a major disabling complication. The association of mucor in cancer is linked with immunosuppression caused by radiation and/or chemotherapy. In this case report, we tried to explore the diverse possibilities of neck swelling, nasal discharge, ocular swelling and dimness of vision in a teenage boy. Rhabdomyosarcoma is a rar…

To implant or not to implant: emergency orbital eviscerations with primary orbital implants.

BACKGROUND/OBJECTIVES: To evaluate the outcomes of orbital evisceration with primary implant placement in acutely infected/inflamed eyes, using implant exposure/extrusion as a surrogate of success. To contextualise this with previously published literature. SUBJECTS/METHODS: A retrospective case series of all patients with acutely infected/inflamed eyes undergoing urgent orbital evisceration with primary implants, at a British tertiary centre between January 2006 and August 2018. A systematic literature review of orbital eviscerations with primary implant placement in acute endophthalmitis/in…

Orbital Blowout Fracture and Orbital Emphysema caused by Nose Blowing.

A 76-year-old man presented with 1 day history of eyelid swelling and decreased vision on the left side. History reveals nose blowing the night prior to the occurrence of the symptoms. Initial examination of the left eye showed decreased visual acuity, increased intraocular pressure, and limited ocular motility with presence of air bubbles under the bulbar conjunctiva. Imaging studies showed medial orbital wall fracture with severe emphysema. The patient was observed for a week and no active surgical intervention was performed. Thereafter, spontaneous resolution of symptoms was observed. Eyeli…

Overview of tools for the measurement of the orbital volume and their applications to orbital surgery.

There are numerous applications in craniofacial surgery with orbital volume (OV) modification. The careful management of the OV is fundamental to obtain good esthetic and functional results in orbital surgery. With the growth of computer-aided design - computer-aided manufacturing (CAD-CAM) technologies, patient-specific implants and custom-made reconstruction are being used increasingly. The precise measurement of the OV before surgery is becoming a necessity for craniofacial surgeons. There is no consensus on orbital volume measurements (OVMs). Manual segmentation of computed tomography (CT)…

IgG4 related orbit disease - An unusual cause of an orbital mass.

IgG4-related disease is a newly described systemic autoimmune and allergic disease, characterized histologically by a fibroinflammatory response with IgG4 plasma cells. It was initially described as affecting the pancreas, but commonly involves the head and neck region as well. While a biopsy is essential for definitive diagnosis, cross sectional imaging may be the initial modality which may suggest this entity. We describe a case of pathologically proven IgG4 related disease which highlights some key radiologic features seen in this entity. Our case highlights some key radiological features o
