[A case of tsutsugamushi disease which occurred in south western Shikoku].

We report a case of tsutsugamushi disease found in south western Shikoku. A 64-year-old male who lived in Towa Village in Kochi, developed a fever and headache on April 6, 1994, and was admitted to Uwajima City Hospital on April 15, with a ten-day history of illness. He had an eschar on the right anterior side of the breast and an enlargement of the right axillary lymph node, without a rash. Laboratory data showed mild liver injury and atypical lymphocytes with 6% in peripheral blood. After his blood was drawn for rickettsial isolation, the minocycline was administered. His symptoms improved r

Rickettsia tsutsugamushi in chiggers (Acari: Trombiculidae) associated with rodents in central Thailand.

Chiggers were collected from rodents trapped at two military bases located 10 km apart in central Thailand. One site was swampy and nearly treeless and the other site was well-drained and partially wooded. Although 13 species of chiggers were collected, only three species were found to be positive for Rickettsia tsutsugamushi (Hayashi) Ogata using a direct fluorescent antibody test: Blankaartia acuscutellaris Walch (7.3% infected), Leptotrombidium delinese Walch (3.1% infected), and an undescribed species of Ascoschoengastia near A. indica Hirst (1.2% infected). This was the first record of R.

Isolation of Rickettsia tsutsugamushi antigenically different from Kato, Karp, and Gilliam strains from patients.

Rickettsia tsutsugamushi strains from three recent patients of Tsutsugamushi disease in Niigata Prefecture were isolated primarily in mice and then in L cell cultures. By this procedure, low virulent strains to mice, as well as high virulent ones, could be isolated and cultivated serially in L cell cultures, suggesting the usefulness of L cells for isolation of this species of rickettsia. Each newly isolated strain was identified as a member of R. tsutsugamushi from the results of cross immunological tests and morphological observation. On the other hand, it was recognized that one of these ri

Scrub typhus 1980.

Scrub typhus is a widespread and at times serious infection in Asia. If results from central Malaysia can be applied, it appears to be economically important. Diagnosis is often difficult and treatment prone to fail if short courses of antibiotics are used. Prophylaxis is the key area of research with the development of a vaccine being the ultimate goal.

Different allotypic susceptibility of mice to Rickett-sia tsutsugamushi.

Some lines of mice and random-bred mice show different susceptibility to infection with Rickettsia tsutsugamushi, displayed mainly in the development of overt infection. The latter is most markedly manifested in random-bred and Swiss mice, causing high death rates and massive accumulation of rickettsiae in the peritoneum. Black mice respond to inoculation with asymptomatic infection with no microscopically detectable rickettsiae. The infection is accompanied in either case by immunological response and persistence of the agent in mice. One of the mechanisms of resistance to R. tsutsugamushi in

From eschar to diagnosis: A case report on scrub typhus causing multi-organ failure in a returning traveller.

Scrub typhus, caused by Orientia tsutsugamushi, is a rickettsial infection transmitted by mite bites, often underdiagnosed in travellers from endemic regions. This case report describes a 65-year-old female with hypertension who developed severe scrub typhus after a trip to Sri Lanka. She presented with fever, myalgia, headache, fatigue and a scabbed lesion. Initial tests showed neutrophilia, lymphocytopenia and elevated liver enzymes. Malaria was ruled out, and empiric treatment with intravenous Tazocin was initiated. On day 5, she developed Clostridium difficile infection, requiring a switch…

Touching the invisible: exploring intracellular host-pathogen interactions through multisensory art.

The microscopic world of intracellular bacteria is rarely communicated to non-scientists. By participating in the Sensory Science Exhibition, held at St Catharine's College, University of Cambridge as part of the Cambridge Festival, we sought to address this problem by creating a 3D mammalian cell with model bacteria, including Salmonella enterica, Chlamydia trachomatis and Orientia tsutsugamushi. By hijacking eukaryotic host cellular machinery and avoiding detection, these bacteria orchestrate their survival and replication within host cells. This tactile display aimed to guide participants t…

Integration of IgM ELISA and 56 kDa gene PCR in management of pediatric acute encephalitis syndrome associated with scrub typhus.

OBJECTIVES: To identify the potential target genes for detection of Orientia tsutsugamushi (OT) in pediatric acute encephalitis syndrome (pAES). METHODS: DNA was extracted from whole blood of 100 pAES cases having tested positive (n = 41) and negative (n = 59) for scrub typhus (ST) by IgM ELISA. These samples were subjected to standard PCR for 56 kDa, 47 kDa, 16 s rRNA, groEL, traD genes and the newly identified 27 kDa gene. RESULTS: Among the selected gene targets, 56 kDa demonstrated its superiority for OT detection over the other tested genes. The presence of OT was confirmed via PCR targ…

Atrial Fibrillation in Scrub Typhus: A Series of Four Cases.

Scrub typhus, also known as bush typhus, is an acute febrile zoonosis caused by Orientia tsutsugamushi, transmitted by the bite of chigger mite. Patients with scrub typhus can have many different presentations such as acute hearing loss, interstitial pneumonitis, acute respiratory distress syndrome, myocarditis, pericarditis, meningoencephalitis, acute renal failure, acute hepatic failure, and septic shock. The occurrence of multi-organ dysfunction is responsible for high mortality seen in scrub typhus patients. Cardiovascular involvement can also occur in the form of arrhythmia, which leads t…

Management of Central Nervous System Infections, Vientiane, Laos, 2003-2011.

During 2003-2011, we recruited 1,065 patients of all ages admitted to Mahosot Hospital (Vientiane, Laos) with suspected central nervous system (CNS) infection. Etiologies were laboratory confirmed for 42.3% of patients, who mostly had infections with emerging pathogens: viruses in 16.2% (mainly Japanese encephalitis virus [8.8%]); bacteria in 16.4% (including Orientia tsutsugamushi [2.9%], Leptospira spp. [2.3%], and Rickettsia spp. [2.3%]); and Cryptococcus spp. fungi in 6.6%. We observed no significant differences in distribution of clinical encephalitis and meningitis by bacterial or viral …

Severe Fever with Thrombocytopenia Syndrome Virus Infection or Mixed Infection with Scrub Typhus in South Korea in 2000-2003.

Severe fever with thrombocytopenia syndrome is a tick-borne viral disease, with a high mortality rate that was first reported in China in 2009. Scrub typhus is an acute febrile illness caused by Orientia tsutsugamushi, a bacterium transmitted to humans through chigger mite bites. Severe fever with thrombocytopenia syndrome and scrub typhus are endemic to South Korea. To investigate evidence of severe fever with thrombocytopenia syndrome virus (SFTSV) infection or mixed infection with scrub typhus in South Korea, we examined 2,329 sera samples collected from patients presenting from November 1,…

Infectious causes of encephalitis and meningoencephalitis in Thailand, 2003-2005.

Acute encephalitis is a severe neurologic syndrome. Determining etiology from among ≈100 possible agents is difficult. To identify infectiou … or probable etiologies, the leading pathogens were Japanese encephalitis virus, enteroviruses, and Orientia tsutsugamushi. No samples were positive for chikungunya, Nipah, or West Nile viruses; Bartonella henselae; or malaria parasites. Although a broad range of infectious agents was identified, the etiology of most cases remains unknown.

Severity predictors in eschar-positive scrub typhus and role of serum osteopontin.

We prospectively evaluated severity predictors in terms of host, microorganism, and treatment factors in 153 eschar-positive scrub typhus patients. Severity was assessed with the Acute Physiology and Chronic Health Evaluation (APACHE) II score (< 10 versus ≥ 10) and predefined criteria of severe complications. Genotypes of Orientia tsutsugamushi were determined. Independent risk factors for severity (APACHE II score ≥ 10) were old age, diabetes mellitus, serum osteopontin > 100 ng/mL, and a group of underlying diseases (congestive heart failure, cerebrovascular disease, chronic liver disease, …

Acute respiratory distress syndrome in scrub typhus.

Scrub typhus is a mite-borne infectious disease caused by Orientia tsutsugamushi. Acute respiratory distress syndrome (ARDS) is a serious complication of scrub typhus. This study retrospectively reviewed the medical records of 72 patients diagnosed with scrub typhus from January 1998 to August 2006 in Kaohsiung Chang Gung Memorial Hospital in Taiwan. Eight of 72 scrub typhus patients with ARDS were included in the study; the other patients without ARDS were used as controls. The mortality rate for the scrub typhus patients with ARDS was 25%. The eight patients seldom had underlying diseases. I…
