In a cohort of HIV-infected patients of sub-Saharan origin we describe the incidence of metabolic syndrome, insulin resistance, and lipodystrophy after 3 years of combined antiretroviral therapy, and model the 10-year risk of cardiovascular diseases, while taking into account environmental factors. This is a multinational, prospective cohort study conducted in HIV outpatient clinics from four tertiary care centers set in France and Côte d'Ivoire. The participants were HIV-infected, treatment-naive patients eligible to start antiretroviral treatment and were of sub-Saharan African origin. The m

BACKGROUND: People with long-term physical conditions are more likely to experience distress, depression, or anxiety. Physical-mental comorbidity is associated with lower quality of life, poorer clinical outcomes, and increased mortality than physical conditions alone. People of South Asian origin are the largest minority group in the UK, and more likely to have long-term conditions (LTCs) such as diabetes and heart disease.
AIM: To explore how men of South Asian origin with LTCs understand and experience emotional distress as well as the experiences of GPs supporting them.

BACKGROUND: Escherichiacoli are the reservoirs of antimicrobial resistance (AMR) genes. Shiga toxin-producing E. coli of animal origin can cause severe life-threatening infections in man.
OBJECTIVES: The present study investigated the susceptibility of poultry-origin E. coli isolates to Clinical and Laboratory Standard Institute (CLSI)-recommended antibiotics and their Shiga toxin profiles.
MATERIALS AND METHODS: A total of 141 samples comprising cloacal swabs and water and litter samples were investigated. Antimicrobial susceptibility profiles of the recovered isolates were determined for 1

CONTEXT: The protean mucocutaneous manifestations of HIV and the resultant opportunistic infections are well documented. Genital dermatoses can be either venereal or nonvenereal in origin. As the presence of HIV infection greatly increases the chances of acquiring another sexually transmitted pathogen, these are often presumed to be venereal in origin.
AIMS: The aims of the study were to record the different morphologies of genital skin lesions in seropositive patients and to classify them as venereal or nonvenereal in origin.
SETTINGS AND DESIGN: This was an observational study undertaken i

Osteoclasts are multinucleated giant cells that resorb bone, ensuring development and continuous remodelling of the skeleton and the bone marrow haematopoietic niche. Defective osteoclast activity leads to osteopetrosis and bone marrow failure1-9, whereas excess activity can contribute to bone loss and osteoporosis10. Osteopetrosis can be partially treated by bone marrow transplantation in humans and mice11-18, consistent with a haematopoietic origin of osteoclasts13,16,19 and studies that suggest that they develop by fusion of monocytic precursors derived from haematopoietic stem cells in the

Recent randomized trials focused on gene expression-based determination of the cell of origin in diffuse large B-cell lymphoma could not show significant improvements by adding novel agents to standard chemoimmunotherapy. The aim of this study was the identification of a gene signature able to refine current prognostication algorithms and applicable to clinical practice. Here we used a targeted gene expression profiling panel combining the Lymph2Cx signature for cell of origin classification with additional targets including MYC, BCL-2 and NFKBIA, in 186 patients from 2 randomized trials (disc

The glucose kinase superfamily, which includes enzymes such as hexokinases and glucokinases, plays a central role in energy metabolism across all domains of life. This study explores their evolutionary origins, functional diversity, and adaptation to ecological niches, tracing their journey from early life forms to modern organisms. Recent advances reveal how these enzymes have diversified, with some retaining broad specificity, while others evolved high substrate specificity, reflecting the metabolic demands of their environments. By integrating phylogenetic, structural, and functional analys

INTRODUCTION: Massive parenchymal hemorrhage of the thyroid gland is very rare. Some of these can reach a life-threatening level.
CASE PRESENTATION: A 70-year-old female patient approached the emergency department with swelling and redness on her neck after a routine dialysis session. In the neck computed tomography obtained, there was a massive hematoma originating from the thyroid gland parenchyma. The hematoma was causing airway compression. We performed thyroid artery embolization and within days, hematoma dimensions and compression effect disappeared without surgical treatment.

There is a need for a safe and effective oral treatment for cutaneous and visceral leishmaniasis. Miltefosine is the first oral drug that is efficacious against different forms ofleishmaniasis, however it is not equally effective against all Leishmania species. Miltefosine is an alkylphosphocholine, originally developed for the treatment of cancer. The mechanism of action is probably based on interference with the synthesis and degradation of parasitic membrane lipids. Little is known about the pharmacokinetics ofmiltefosine; an important characteristic is its long elimination half-life of sev

Human emotions are hypothetic constructs based on psychological and physiological data. According to the psychoevolutionnist theories, all emotions derive from a set of discrete basic emotions, common to human and animals, genetically determined. Basic emotions are thus considered as physiological processes based on specific neuronal circuits. On the contrary, for appraisal and social theories, emotions are psychological processes resulting from the cognitive appraisal of the stimulus-event for the well-being and objectives of the subject, and are of social origin. They develop during life, es

IMPORTANCE: Heterotopic glial tissue is a rare, benign congenital anomaly that can present as a fibrous mass with or without overt signs of neural tissue origin, such as overlying alopecia, pigmentation, or a "hair collar" sign. These lesions are typically found in the midline area, though they have been found laterally involving the mastoid and middle ear regions. As demonstrated by this case report, a lateral scalp lesion may represent glial tissue even if fine-needle aspiration does not demonstrate neural findings.
OBSERVATIONS: This single case report represents a 7-month-old white infant

INTRODUCTION: Psychological disorders are strong predictors of life expectancy and have an impact on quality of life. Autosomal dominant polycystic kidney disease (ADPKD) is frequently diagnosed before the onset of subjective symptoms. Similar to other disorders of genetic origin, ADPKD may be a source of remarkable psychological discomfort. One way of coping with emotional distress is its suppression, and this could be measured. Our study aimed to provide data on the acceptance of illness, the emotional suppression of anger, and both anxiety and depression as well as satisfaction with life in

BACKGROUND: One of the most important aspects for patients undergoing rectal cancer surgery is quality of life, which is closely related to postoperative sexual, urinary, and bowel functions. To preserve these functions, surgeons need to pay special attention to the fascial planes and autonomic nerve plexuses.
OBJECTIVE: The aim of this study is to describe the locations of autonomic nerves in critical areas and to demonstrate the correct surgical planes for protecting these nerves during total mesorectal excision.
DESIGN AND SETTINGS: Macroscopic and microscopic surgical dissections were pe

Hemoptysis is the expectoration of blood from the tracheobronchial tree. It is commonly caused by bronchiectasis, chronic bronchitis, and lung cancer. The expectorated blood usually originates from the bronchial arteries. When hemoptysis is suspected, it must be confirmed and classified according to severity, and the origin and cause of the bleeding determined. Lateral and AP chest X-ray is the first study, although a normal chest X-ray does not rule out the possibility of malignancy or other underlying pathology. Multidetector computed tomography (MDCT) must be performed in all patients with

Embryonic development in hymenopteran parasitoids exhibits remarkable diversity, often associated with shifts in life history strategies such as the transition from ecto- to endoparasitoidism. In particular, reduction or loss of yolk has been linked to profound modifications of early developmental processes, including cleavage patterns and the formation of extra-embryonic membranes (EEMs). In this study, the embryogenesis of Encarsia lutea (Hymenoptera: Aphelinidae), a common whitefly parasitoid, was investigated using light and fluorescence microscopy. Eggs and developing embryos were collect

BACKGROUND: Who we live with in later life significantly influences the daily care support we receive, potentially moderating chronic disease trajectories. For immigrants, this relationship is further complicated by cultural preferences for certain living arrangements. This study examines the differential role of living arrangements in chronic disease accumulation among native-born and immigrant older adults in Europe.
METHODS: Using data from the Survey of Health, Ageing and Retirement in Europe (2006-2022), we analyze how living arrangements moderate the relationship between age, migration

INTRODUCTION AND HYPOTHESIS: The aim of this study is to compare a modified inside-out transobturator procedure with its original counterpart [inside-out transobturator (TVT-O)] for the treatment of female stress urinary incontinence (SUI).
METHODS: A prospective, randomized trial in women suffering from SUI was used. The modified procedure consisted of a shorter tape whilst the scissors or guide no longer perforated the obturator membrane. The primary outcome was the resolution of subjective and objective SUI at 1 year. Secondary outcome measures included adverse events, quality of life meas

Alzheimer's disease (AD) is a chronic neurodegenerative disorder associated with cognitive impairment and later dementia among the elderly. Mounting evidence shows that adverse maternal environments during the fetal development increase the risk of diseases later in life including neurological disorders, and suggests an early origin in the development of AD-related dementia (ADRD) in utero. In the present study, we investigated the impact of antenatal hypoxia and fetal stress on the initiation of AD-related pathology in offspring of 5xFAD mice. We showed that fetal hypoxia significantly reduce

BACKGROUND: Low transsphincteric fistulas less than 1/3 of the sphincter complex are easy to treat by fistulotomy with a high success rate. High transsphincteric fistulas remain a surgical challenge. Various surgical procedures are available, but recurrence rates of these techniques are disappointingly high. The mucosal flap advancement is considered the gold standard for the treatment of high perianal fistula of cryptoglandular origin by most colorectal surgeons. In the literature a recurrence rate between 0 and 63% is reported for the mucosal flap advancement. Recently Armstrong and colleagu

BACKGROUND: Helicobacter pylori (H. pylori) is the most common chronic bacterial infection in humans. The risk of acquiring H. pylori is related to socioeconomic status and living conditions early in life. Treatment regimens must consider local antibiotic resistance patterns. Adventist Health White Memorial Hospital serves a predominantly indigent population in east Los Angeles with a large number of immigrants from South and Central America. Data regarding the prevalence and resistance of H. pylori in this population is scant.
AIM: To evaluate the prevalence and resistance of H. pylori and c
