Benefit of degenerative posterior longitudinal ligament removal during anterior decompression in cervical spondylotic myelopathy.

It remains controversial whether degenerative posterior longitudinal ligaments should be removed during anterior decompression procedures for cervical spondylotic myelopathy. Few data are available from studies that have compared removing and retaining the degenerative posterior longitudinal ligament. The goal of this retrospective study was to evaluate the benefit of degenerative posterior longitudinal ligament removal during such operations. Clinical data on 130 patients with confirmed degenerative posterior longitudinal ligament who underwent anterior cervical decompression surgery were ret

Medical malpractice litigation after total shoulder arthroplasty: a comprehensive analysis based on the Westlaw legal database.

INTRODUCTION: Orthopedic surgery is one of the most common subspecialties subject to medical malpractice claims. Although total shoulder arthroplasty (TSA) is associated with favorable patient outcomes and relatively low complication rates, surgeons performing this procedure may be subject to malpractice litigation leading to significant economic and psychological burden on the provider. The purpose of this study is to characterize and describe malpractice claims against orthopedic surgeons performing TSA using the Westlaw legal database. METHODS: The Westlaw legal database was queried for al

Injury to endoscopic personnel from tripping over exposed cords, wires, and tubing in the endoscopy suite: a preventable cause of potentially severe workplace injury.

BACKGROUND The endoscopy unit before remediation may be a high-risk area for slip and fall injuries due to a large number of exposed above-the-floor wires in the endoscopy rooms, dimmed lighting during endoscopic procedures, and staff inattention to obstacles due to preoccupation with the endoscopic patient. AIM To describe a novel, previously unappreciated workplace hazard to endoscopic personnel: Exposed wires in the endoscopy unit.METHODS This study is a retrospective review of 110,000 endoscopic procedures performed during the last 5 years at an academic, teaching hospital with a high-volu

Arthroscopic repairs of triangular fibrocartilage complex tears.

Technical advancements in arthroscopic wrist procedures have improved our knowledge of normal and abnormal intraarticular wrist function. Triangular fibrocartilage complex (TFCC) tears from trauma injuries are a common source of ulnar-sided wrist pain. Fortunately, the TFCC is a structure that can be evaluated and treated arthroscopically with results that are comparable to open surgical procedures. Successful arthroscopic repairs of TFCC tears depend on a coordinated team effort between perioperative nurses, orthopedic surgeons, nurse practitioners, and occupational hand therapists, as well a

Indications for magnetically controlled growing rods have evolved over time.

PURPOSE: The purpose of this study is to determine the trends in use of magnetically controlled growing rods (MCGRs) over time for the treatment of scoliosis. METHODS: All patients treated with MCGRs were identified through a multi-center pediatric spine database from 2014 to 2021. MCGR use over time was analyzed with respect to individual variables within the dataset including demographics, etiology, primary vs conversion surgeries, and major curve magnitude. RESULTS: A total of 1,404 patients treated with MCGRs were identified. MCGR usage grew quickly until 2017 and then steadily declined

From Arthroereisis to Osteoereisis: A Nomenclature Proposal for a Misnamed Surgical Procedure.

The surgical procedure currently known as arthroereisis has been performed for decades as a treatment for recurrent talotarsal dislocation sequela in both pediatric and adult populations, yet its name has never been formally challenged despite being etymologically and anatomically inaccurate. The term derives from the Greek arthro (joint) and ereisis (to lift or support), implying joint-level action that does not reflect the primary mechanism of the procedure. The sinus tarsi implant-and the family of interventions grouped under this designation-acts not on a joint surface as its primary mecha

You've Got Some Nerve: Initial Management of Iatrogenic Nerve Injuries.

Orthopaedic surgery has a higher risk of iatrogenic nerve injury than other surgical specialties. The initial management depends on the etiology, which requires early recognition and an appreciation for the injury. Three perspectives are given to guide the surgeon through the initial management of these devastating complications. An orthopaedic surgeon with a knowledge of case law emphasizes the importance of clear documentation and frequent communication with the patient. An anesthesiologist discusses the improvements in regional anesthesiology and the risks inherent in motor-sparing blocks.

Orthopedic management in Duchenne muscular dystrophy.

This article provides a comprehensive overview of the orthopedic management strategies in Duchenne muscular dystrophy (DMD). DMD manifests with severe muscle wasting, respiratory insufficiency, and cardiomyopathy, with initial symptoms emerging in early childhood. By the age of 10-12 years, most patients require wheelchairs due to muscle degeneration, which often leads to spinal deformities and joint contractures. The absence of dystrophin causes the breakdown of the dystrophin glycoprotein complex, causingmuscle degeneration, necrosis, and fibrosis. Current treatments focus on symptom managem

Os Trigonum Syndrome: A Cause of Posterior Ankle Pain.

BACKGROUND: Os trigonum syndrome represents a cause of posterior ankle pain that is predominantly seen in athletes. The os trigonum ossicle forms from a secondary ossification center of the talus and is located at its posterior aspect in an interval between the posterior lip of the tibial plafond and calcaneus. The os trigonum ossicle is often an incidental finding and asymptomatic. However, repetitive plantarflexion and push-off maneuvers can cause symptoms and lead a patient to pursue orthopedic care. MATERIALS AND METHODS: A review of the literature was conducted using the PubMed search en

Intraoperative Challenges in Hand Surgery.

A wide array of intraoperative issues can arise during surgery involving the hand and upper extremity. An understanding of the common pitfalls within hand surgery may help practicing hand surgeons circumvent such issues. Within this manuscript, we first identify problems with the increasingly popular technique of wide-awake local anesthesia no tourniquet (WALANT). Achieving appropriate hemostasis and anesthetic can be bothersome, especially for procedures proximal to the distal palmar crease. We discuss our local anesthetic timing and concentrations to help mitigate such issues, as well as oth

The magnetic field strength and the force distance dependency of the magnetically controlled growing rods used for early onset scoliosis.

Magnetically controlled growing rods (MCGR's) have revolutionized the treatment of early-onset scoliosis (EOS) because painless lengthenings can be done in the outpatient clinic without anesthesia. Untreated EOS leads to respiratory insufficiency and reduced life expectancy. However, MCGR's have inherent complications like non-functioning of the lengthening mechanism. We quantify an important failure mechanism and give advice on how to avoid this complication. The magnetic field strength was measured on new/explanted rods at different distances between the external remote controller and the MC

Predictive analysis of the scoliotic curve using a subject's 3D model.

A predictive analysis of the conservative scoliosis treatment is necessary, in which a 3D model of an optimal treatment algorithm is a basic part in the design of a prosthetic corset. Since CAD technology has proven to be very useful in the field of prosthetics and orthotics, we used an open-source software to plan the correction of the scoliotic curve on a virtual model of the subject's torso. The shape of the scoliosis was simplified by means of a directional polygon, which was drawn in a reverse manner depending on the directional arcs of the scoliotic curve. The resulting scoliosis correct

Early-onset scoliosis.

PURPOSE OF REVIEW: The aim of this review is to provide the reader with the most updated available information so that it can be helpful in the approach of patients with early-onset scoliosis (EOS). RECENT FINDINGS: While confirming the efficacy and safety of classic techniques for the treatment of EOS such as traditional growing rods or Mehta casting, recent research suggests that there is room for improvement with less invasive techniques. SUMMARY: The most important goal when treating patients with EOS should be to promote rib cage expansion and lung development. Different techniques have

Hemostatic cover in orthopedic surgery.

Bleeding into joints, known as hemarthrosis, is the most common kind of bleeding experienced by patients with hemophilia. Repeat bleeds into the same joint lead to the so-called hemophilic arthropathy. Patients with this condition tend to require surgery earlier and most frequently than the general population. Successful hemostasis is essential to carry out such procedures. Thanks to the advances made in the treatment of hemophilia, most surgical techniques can be performed safely and reliably. The present review shall focus on the international recommendations related to the performance of th
