In a 34-year-old man with an acute traumatic spondylolisthesis (ATS) of the axis, the presenting physical sign was severe torticollis associated with reactive spasm of the right sternomastoid muscle. Computed axial tomography (CAT) was useful both in assessing the nature of ATS (which involved more displacement of the axis fracture and adjacent soft-tissues on the right side of the neck) and in ascertaining reduction, realignment, and healing. The reduction was sustained by halo-vest immobilization.

The effect of dissipative mechanical loads on spastic gait has been studied, to evaluate the feasibility of using mechanically damped orthoses to effect functional improvements in the gait of spastic patients. In the treatment of unstable comminuted and intra-articular fractures of the lower end of the radius, we have found that is is essential to reconstitute the normal anatomical relationship of the radius to the carpus and to the ulna. This is possible by performing an open reduction and by maintaining the fractured fragments by Kirschner wires and adding a tension band. Patients treated thus are reported. There have been no complications and the patients regained full function of the wrist without unsightly deformity.

The spectrum of patellofemoral disorders ranging from chondromalacia of the patella to degenerative arthritis is a difficult, often vexing problem. Many methods of treatment, both surgical and nonsurgical, have been used without uniform or predictable success. The infrapatella strap, a new concept in nonsurgical management of patellofemoral pain, can be effective in 77% of patients. Its simplicity and comfort, patients' acceptance, and the apparent lack of contraindications when it is worn as instructed justify further clinical trials.

Constricting devices placed on the penis to improve sexual performance present a challenge to urologists. Various metallic and nonmetallic objects have been described -- plumbing cuffs, bull rings, wedding rings, plastic or rubber rings -- which usually could be removed by the combination of medical and operative treatment (decompression of the penis and glandular puncture) and craftsmen's techniques. We describe the case of a titanium thumb ring of 2 mm thickness which only could be removed by using an electric cutting tool.
