Corneal grafts at St John Eye Hospital, Jerusalem, January 2001-November 2002.

AIM: To compare a cohort of corneal graft patients in east Jerusalem with one in Sweden, concerning diagnosis, sex, patient age, preoperative visual acuity in both eyes, and type of operation. METHODS: Standard forms developed for the Swedish Corneal Transplant Register were used for data collection at the time of operation. RESULTS: In east Jerusalem, keratoconus accounted for 51% of the grafts compared with only 27% in Sweden and the male:female ratio was reversed. There were very few patients with endothelial disease. The Palestinian patients had overall worse visual acuity both in the ey

Large recession nystagmus surgery in albinos: effect on acuity.

PURPOSE: To evaluate the effect of large muscle recessions on visual acuity in albinos with nystagmus. METHODS: This is a retrospective, unmasked, chart review of 12 patients with ocular or oculocutaneous albinism demonstrating nystagmus who underwent four horizontal rectus muscle retroequatorial recessions. Pre- and postoperative visual acuity were measured. RESULTS: Best corrected visual acuity improved postoperatively by two or more Snellen lines in 7 of 12 patients (58%). Subjective improvement of vision and nystagmus was noted by patients and examiners. Follow up was performed between 3

Extracapsular cataract extraction in proliferative diabetic retinopathy.

PURPOSE: To establish the visual prognosis, prevalence of complications, and optimal strategy for management of proliferative diabetic retinopathy (PDR) in isolated extracapsular cataract extraction (ECCE). METHOD: This is a retrospective review of 56 patients with PDR who underwent ECCE with lens implantation. Results were compared with 64 patients with background diabetic retinopathy (BDR) operated on during the same time period. RESULTS: Final visual acuity (67% > or = 20/40) was better in BDR eyes compared with PDR eyes (21% > or = 20/40; P < 0.001). In eyes without maculopathy, 94% with

An unusual presentation of pseudotumor cerebri.

A 25-year old grossly obese woman presented with decreased visual acuity. She had bilateral total (internal and external) ophthalmoplegia, a left-sided seventh cranial nerve palsy, and florid bilateral papilledema. The diagnosis of pseudotumor cerebri was made on the basis of high cerebrospinal fluid pressure and the absence of other abnormalities on systemic, neurologic, and neuroradiologic examination. The patient responded favorably to corticosteroid treatment; however, bilateral optic atrophy occurred with persistent loss of visual field and acuity.

The association between myopic shift and visual acuity outcome in pediatric aphakia.

PURPOSE: The advent of intraocular lens implantation after pediatric cataract surgery necessitates an increased understanding of refractive development. The significant variation in rate and amount of refractive change among eyes, both aphakic and pseudophakic, is well recognized, although the causes of such variation remain unclear. The purpose of this study was to determine if a correlation exists between the rate of refractive growth (RRG) and visual acuity outcome in pediatric aphakia. METHODS: Multicenter, retrospective observational case series. One hundred and twenty-five eyes of 85 pa

[Persistent foveal retinal detachment after successful rhegmatogenous retinal detachment surgery by scleral buckling].

We report the case of a phakic patient with late visual recovery after successful surgery of macula-off retinal detachment, using the scleral buckling procedure. Visual acuity remained low because of the persistence of subfoveal fluid despite the reattachment of peripheral retina. The amount of subfoveal fluid decreased slowly and final visual acuity finally recovered. We used optical coherence tomography (OCT) to quantify the remaining subfoveal fluid. Persistence of submacular fluid can explain late visual recovery after surgical treatment of retinal detachment.

Improvement of acquired pendular nystagmus by gabapentin: case report.

Acquired Pendular Nystagmus (APN) may cause distressing visual symptoms in patients who are already suffering a severe general disease. Averbuch-Heller et al. conducted the first double-blind controlled study on treatment for APN. They showed that gabapentin substantially reduces pendular nystagmus and significantly increases visual acuity in the majority of patients. We present a patient with APN due to multiple sclerosis who suffered severe oscillopsy and reduction of visual acuity and who substantially benefited from a trial treatment with this agent.

Acuity profile perimetry: description of technique and preliminary clinical trials.

Acuity profile perimetry is a technique that allows visual acuity properties to be rapidly measured at any location in the visual field. This report describes our initial clinical trials with acuity profile perimetry, as performed in combination with standard static perimetry. Preliminary results from patients with cataracts, amblyopia, and central serous retinopathy show, in general, a much greater deficit for acuity profiles than for static sensitivity profiles. In contrast, patients with optic neuritis or other optic neuropathies display approximately equivalent losses for static and acuity

Factors associated with low-acuity hospital admissions in a public safety-net setting: a cross-sectional study.

BACKGROUND: Given system-level focus on avoidance of unnecessary hospitalizations, better understanding admission decision-making is of utility. Our study sought to identify factors associated with hospital admission versus discharge from the Emergency Department (ED) for a population of patients who were assessed as having low medical acuity at time of decision. METHODS: Using an institutional database, we identified ED admission requests received from March 1, 2018 to Feb 28, 2019 that were assessed by a physician at the time of request as potentially inappropriate based on lack of medical

Online Questionnaire with Fibromyalgia Patients Shows Negative Correlations between Disease Severity and Adherence to Mediterranean Diet.

Fibromyalgia (FM) is a multidimensional disorder in which intense chronic pain is accompanied by a variety of psychophysical symptoms that impose a burden on the patients' quality of life. Despite the efforts and the recent advancement in research, FM pathogenesis and effective treatment remain unknown. Recently, the possible role of dietary patterns and/or components has been gaining attention. The current study aimed to investigate a potential correlation between adherence to the Mediterranean diet (MedDiet) and FM severity in a sample of Italian FM patients. An online survey was designed, c

Examining the Relationship between Systemic Immune-Inflammation Index and Disease Severity in Juvenile Idiopathic Arthritis.

Juvenile Idiopathic Arthritis (JIA), the leading childhood rheumatic condition, has a chronic course in which persistent disease activity leads to long-term consequences. In the era of biologic therapy and tailored treatment, precise disease activity assessment and aggressive intervention for high disease activity are crucial for improved outcomes. As inflammation is a fundamental aspect of JIA, evaluating it reflects disease severity. Recently, there has been growing interest in investigating cellular immune inflammation indices such as the neutrophil-to-lymphocyte ratio (NLR) and systemic im

Biomarkers to monitor the prognosis, disease severity, and treatment efficacy in coronary artery disease.

INTRODUCTION: Coronary Artery Disease (CAD) is a prevalent condition characterized by the presence of atherosclerotic plaques in the coronary arteries of the heart. The global burden of CAD has increased significantly over the years, resulting in millions of deaths annually and making it the leading health-care expenditure and cause of mortality in developed countries. The lack of cost-effective strategies for monitoring the prognosis of CAD warrants a pressing need for accurate and efficient markers to assess disease severity and progression for both reducing health-care costs and improving p

Serum Appetite-Regulating Hormone Levels in Cystic Fibrosis Patients: Influence of the Disease Severity and the Type of Bacterial Infection-A Pilot Study.

Cystic fibrosis (CF) belongs to the most common inherited diseases. The severity of the disease and chronic bacterial infections are associated with a lower body index, undernutrition, higher number of pulmonary exacerbations, more hospital admissions, and increased mortality. The aim of our study was to determine the impact of the severity of the disease and the type of bacterial infection in 38 CF patients on the serum level of appetite-regulating hormones including leptin, ghrelin, neuropeptide Y, agouti-signaling protein, proopiomelanocortin, kisspeptin, putative protein Y, and α-melanocyt

Association of heme-oxygenase 1, hemopexin, and heme levels with markers of disease severity in COVID-19.

Heme-oxygenase 1 (HO-1) is an enzyme with well-known anti-inflammatory and antioxidant properties, whose levels have been previously associated with disease severity in the context of sterile and infectious diseases. Moreover, the heme/HO-1 pathway has been associated with prothrombotic changes in other diseases. Accordingly, the potential of modulating HO-1 levels for the treatment of COVID-19 was extensively speculated during the COVID-19 pandemic, but very few actual data were generated. The aim of our study was to explore the association of HO-1, heme, and hemopexin (HPX) levels with COVID
