Reduction in spread of excitation from current focusing at multiple cochlear locations in cochlear implant users.

Channel interaction from a broad spread of excitation is likely to be a limiting factor in performance by cochlear implant users. Although partial tripolar stimulation has been shown to reduce spread of excitation, the magnitude of the reduction is highly variable across subjects. Because the reduction in spread of excitation is typically only measured at one electrode for a given subject, the degree of variability across cochlear locations is unknown. The first goal of the present study was to determine if the reduction in spread of excitation observed from partial tripolar current focusing s

Effects of age and hearing loss on stream segregation based on interaural time differences.

The effect of interaural time differences (ITDs) on obligatory stream segregation for successive tone bursts was investigated for older listeners with normal hearing (ONH) and hearing loss (OHL), by measuring the threshold for detecting a rhythmic irregularity in an otherwise isochronous sequence of interleaved "A" and "B" tones. The A and B tones had equal but opposite ITDs from 0 to 0.5 ms. For some of the ONH listeners, the threshold increased with increasing ITD, but no OHL listener showed an effect of ITD. It is concluded that hearing loss reduces the potency of ITDs in inducing obligator

Effect of contralateral acoustic stimulation on cochlear tuning measured using stimulus frequency and distortion product OAEs.

OBJECTIVE: To study whether a change in cochlear tuning, measured using OAEs, could be detected due to contralateral activation of the efferent system using broadband noise. DESIGN: Cochlear tuning measures based on SFOAE phase gradients and SFOAE-2TS 'Q' were used to test this hypothesis. SFOAE magnitude and phase gradient were measured using a pure-tone sweep from 1248 to 2496 Hz at 50 dB SPL. 2TS curves of SFOAE were recorded with a suppressor frequency swept from 1120 to 2080 Hz at 50 dB SPL. DPOAE f2-sweep phase gradient was also obtained to allow comparisons with the literature. All thr

Interactions in the cochlea between air conduction and osseous and non-osseous bone conduction stimulation.

Since air-conducted (AC) and clinical (mastoid) bone-conducted (BC) sounds interact in the cochlea (e.g. pitch, cancellation, masking, beats), it has been thought that both AC and BC stimulations lead to a mechanical wave in the cochlea. However, there are also "non-osseous" forms of BC, i.e. auditory sensation produced when the clinical bone vibrator is applied to "non-osseous" soft tissue sites. In the present study, such "non-osseous" sites were identified (e.g. eye, cheek, neck) and they interacted with AC and osseous BC (pitch matching, beats, masking), indicating that all of these forms

Visual awareness of objects and their colour.

At any given moment, our awareness of what we 'see' before us seems to be rather limited. If, for instance, a display containing multiple objects is shown (red or green disks), when one object is suddenly covered at random, observers are often little better than chance in reporting about its colour (Wolfe, Reinecke, & Brawn, Visual Cognition, 14, 749-780, 2006). We tested whether, when object attributes (such as colour) are unknown, observers still retain any knowledge of the presence of that object at a display location. Experiments 1-3 involved a task requiring two-alternative (yes/no) respo

Event-related potentials to overlapping shapes: effects of saliency and interference.

Visual perception is often challenged by various difficulties that act concomitantly and whose respective impacts may therefore be hard to distinguish. We used event-related potentials to dissociate the impact of target saliency, generated by occlusion, from that of interference produced by incongruent nontargets. In one block, the target (a square) partially occluded another square tilted by 45 degrees. This nontarget square interfered only to a small extent with target perception. In another block, the target was the occluded stimulus, and interference from the nontarget was substantial. Blo

Backward masking: evidence of reduced subcortical amygdala engagement in autism.

Recent data suggest that subthreshold presentation of emotional information is relayed to the amygdala along subcortical pathways. We examined the effect of backward masked neutral and anxious faces on the social decisions of a group of high functioning children with autism ages 7-13 years and matched controls. Participants were asked to select the friendliest of two faces, one of which was associated with the subthreshold (33 ms) presentation of an anxious face (A/N) and the other a subthreshold neutral face (N/N). Neutral paired faces were selected more often than A/N paired faces by both gr

Speech recognition in noise: estimating effects of compressive nonlinearities in the basilar-membrane response.

OBJECTIVES: This experiment was designed to estimate effects of cochlear nonlinearities on tonal and speech masking for individuals with normal hearing who have a range of quiet thresholds. Physiological and psychophysical evidence indicates that for signals close to the characteristic frequency (CF) of a place on the basilar membrane, the normal growth of response of the basilar membrane is linear at lower stimulus levels and compressed at medium to higher stimulus levels. In contrast, at moderate to high CFs, the basilar membrane responds more linearly to stimuli at frequencies well below th

The performance-perceptual test and its relationship to unaided reported handicap.

OBJECTIVE: Measurement of hearing aid outcomes is necessary for demonstration of treatment efficacy, third-party payment, and cost-benefit analysis. Outcomes are usually measured with hearing-related questionnaires and/or tests of speech recognition. However, results from these two types of test often conflict. In this paper, we provide data from a new test measure, known as the Performance-Perceptual Test (PPT), in which subjective and performance aspects of hearing in noise are measured using the same test materials and procedures. A Performance Speech Reception Threshold (SRTN) and a Percep

The use of psychophysical tuning curves to explore dead regions in the cochlea.

OBJECTIVE: "Dead regions" are regions in the cochlea with no functioning inner hair cells (IHCs) and/or neurons. Amplification (using a hearing aid) over a frequency range corresponding to a dead region may not be beneficial and may even impair speech intelligibility. The objective of this article is to illustrate the use of psychophysical tuning curves (PTCs) as a tool for investigating dead regions and to illustrate the variety of audiogram configurations that can be associated with dead regions. We explore the influence of signal level and signal frequency to test the hypothesis that the fr

Does perception of biological motion rely on specific brain regions?

Perception of biological motions plays a major adaptive role in identifying, interpreting, and predicting the actions of others. It may therefore be hypothesized that the perception of biological motions is subserved by a specific neural network. Here we used fMRI to verify this hypothesis. In a group of 10 healthy volunteers, we explored the hemodynamic responses to seven types of visual motion displays: drifting random dots, random dot cube, random dot cube with masking elements, upright point-light walker, inverted point-light walker, upright point-light walker display with masking elements

Auditory dysfunction in occupational noise exposed workers.

Occupational noise-induced hearing loss (NIHL) is well known to be an epidemiologically relevant problem. The subjects affected with NIHL show alteration of hearing thresholds as well as a worsening of the cochlear analysis functions and, usually, an impaired speech discrimination in presence of background noise. The study has evaluated the relationships between hearing threshold and equivalent exposure lever per day (Lepd), age and working seniority in a homogeneous sample of occupationally noise exposed workers. Three subgroups were also selected to study the most important cochlear function

Lessons learned from the regulatory alignment in ketamine, esketamine and arketamine clinical trials: A cross-sectional analysis of protocols from ClinicalTrials.gov.

Ketamine and its enantiomers, esketamine and arketamine, have emerged as promising treatments for treatment-resistant depression (TRD). This cross-sectional study evaluates the regulatory alignment of 40 clinical trials listed on ClinicalTrials.gov, focusing on the methodological challenges. The study highlights methodological inconsistencies, particularly around the challenges of functional unblinding caused by ketamine's dissociative effects, addressing expectancy bias, and the inadequate safety monitoring practices in many trials. A notable concern is the variability in blinding techniques,

Learning to adapt: Dynamics of readaptation to geometrical distortions.

The visual system can adapt to optical blur, whereby the adapted image is perceived as sharp. Here we show that adaptation reduces blur-induced biases in shape perception, with repeated adaptations (perceptual learning), leading to unbiased perception upon re-exposure to blur. Observers wore a cylindrical lens of +1.00 D on one eye, thus simulating monocular astigmatism. The other eye was either masked with a translucent blurred lens (monocular) or unmasked (dichoptic). Adaptation was tested in several repeated sessions with a proximity-grouping task, using horizontally or vertically arranged

Developmental plasticity after right hemispherectomy in an epileptic adolescent with early brain injury.

OBJECTIVES: The authors present the case of an adolescent affected with refractory epilepsy due to a neonatal ischemic infarction of the right medial cerebral artery. Hemiplegic since the first months of life, she began presenting motor partial seizures associated with drop attacks at 4.5 years; these were initially well controlled by antiepileptic drugs, but at 10 years seizures appeared again and became refractory. Thus, at 14 years and 10 months, she was submitted to a right hemispherectomy that made her rapidly seizure free. In the post-surgical follow-up lasting 5 years, neuropsychologica

Glottal closure, transglottal airflow, and voice quality in healthy middle-aged women.

Seventeen healthy women, 45 to 61 years old, were examined using videofiberstroboscopy during phonation at three loudness levels. Two phoniatricians evaluated glottal closure using category and ratio scales. Transglottal airflow was studied by inverse filtering of the oral airflow signal recorded in a flow mask (Glottal Enterprises System) during the spoken phrase /ba:pa:pa:pa:p/ at three loudness levels. Subglottal pressure was estimated from the intraoral pressure during p occlusion. Running speech and the repeated /pa:/ syllables were perceptually evaluated by three speech pathologists rega

Effects of Talker Sex Differences on Binaural Summation in Cochlear Implant Users and Normal Hearing Listeners.

The benefit of binaural summation ("summation") is often measured in co-located steady-state noise; summation for competing speech is less understood. The goal of the present study was to evaluate how the target-masker acoustic properties and binaural listening mode affect summation for speech-in-speech recognition. Recognition of target speech in two-talker speech maskers was measured with each ear alone and with both ears in bilateral CI users (BiCI), CI users with asymmetric hearing loss (AHL), CI users who were single-sided deaf (SSD), and normal-hearing listeners (NH). Target and masker s

Effects of hearing acuity on psychophysiological responses to effortful speech perception.

In recent studies, psychophysiological measures have been used as markers of listening effort, but there is limited research on the effect of hearing loss on such measures. The aim of the current study was to investigate the effect of hearing acuity on physiological responses and subjective measures acquired during different levels of listening demand, and to investigate the relationship between these measures. A total of 125 participants (37 males and 88 females, age range 37-72 years, pure-tone average hearing thresholds at the best ear between -5.0 to 68.8 dB HL and asymmetry between ears b

Signal-in-noise detection across the lifespan in a mouse model of presbycusis.

The auditory system is constantly tasked with detecting acoustic cues in complex auditory environments. Difficulty hearing speech in noise, largely a result of energetic masking, is a major communication complaint of the elderly, which impacts a third of the global population over 65. The neural mechanisms responsible for processing sound in background noise and subsequently achieving release from energetic masking remain obscure. Furthermore, the senescence of signal-in-noise detection is poorly understood, a phenomenon which could have a myriad of clinical implications. We tested over 300 CB

Head shadow enhancement with low-frequency beamforming improves sound localization and speech perception for simulated bimodal listeners.

Many hearing-impaired listeners struggle to localize sounds due to poor availability of binaural cues. Listeners with a cochlear implant and a contralateral hearing aid - so-called bimodal listeners - are amongst the worst performers, as both interaural time and level differences are poorly transmitted. We present a new method to enhance head shadow in the low frequencies. Head shadow enhancement is achieved with a fixed beamformer with contralateral attenuation in each ear. The method results in interaural level differences which vary monotonically with angle. It also improves low-frequency s
