Spontaneous bacterial peritonitis (SBP) is an infection of the peritoneal fluid in the absence of an obvious intra-abdominal source. It is most commonly diagnosed in patients with cirrhotic ascites, although it has been described in other syndromes as well. The organisms most frequently cultured from the peritoneum are those of intestinal flora; however, there are cases which have all the features of SBP, but remain culture negative. This article discusses 7 cases of SBP in patients with ventriculoperitoneal shunts; a combination which has previously not been described. The most significant fe…

UNLABELLED: This is a clinical series of 83 cases, admitted in the Surgical Department of Fundeni Hospital between 1988-1996 of severe acute peritonitis in which scheduled reoperations (at least one planned reoperation at 24-48 hours after the first operation) were performed. The main criteria for scheduled reoperations were; unresolved source of contamination, acute peritonitis older than 48 hours and the presence of multiple system organ failures. 63 cases (76%) were postoperative peritonitis. 203 planned reoperations were performed (minimum: 1, maximum: 10, mean: 2,4 reoperations per patien…

A 30 year old man presented with an initial clinical picture limited to low back pain. Increasing severity of the pain after one month led to his admission to a surgical department and the diagnosis of a large right retroperitoneal abscess. Several hours before the proposed operation for extraperitoneal surgical drainage of the abscess through a lumbar approach, his condition was complicated by the intraperitoneal rupture of the abscess. The resulting peritonitis and the retroperitoneal abscess were treated through a laparotomy and the postoperative course was uncomplicated. A literature revie…

A boy aged five months was admitted to a hospital elsewhere on account of failure to thrive for three weeks and intermittent pyrexia. X-ray revealed dislocation of the right hip. After attempted reduction, the infant developed peritonitis. Laparotomy revealed pus which originated from purulent coxitis draining through a defect at the base of the acetabulum. Arthrocentesis should be performed in children with acute coxitis and high fever. If pus or cloudy joint fluid is obtained, the hip joint should be opened and drained at the same session. Neglected purulent coxitis results in destruction of…

Recent reports have suggested that focal hyperechoic abdominal masses detected during the second trimester may represent a normal variation in fetal intestinal development that is transient in nature and not associated with pathologic conditions. The patient described here had second-trimester ultrasonic findings of fetal meconium peritonitis without ascites, polyhydramnios, or other anomalies. Subsequent ultrasound examinations at 22, 30, and 36 weeks demonstrated no change in the abdominal appearance. At birth, this preterm male infant had clinical symptoms of congenital cytomegalovirus infe…

Gonococcal peritonitis occurred after a tubal ligation. Falk's postulated benefit of cornual resection as a preventive measure for recurrent infection does not extend to isthmic interruption. Pelvic inflammatory disease should be considered when a sterilized woman presents with an acute abdomen.

Spontaneous bacterial peritonitis occurred on 44 separate occasions in 43 patients during a five year period, including 27 culture positive and 17 probable cases of spontaneous bacterial peritonitis. Alcoholic liver disease was the underlying cause of 72% of cases. Of the 27 culture positive cases, Escherichia coli was the most common isolate (14 cases), followed by Klebsiella pneumoniae (three cases), group G streptococci (three cases), group B streptococci (two cases) and one case each of five other organisms. Bacteremia occurred in 50% of cases and was the same as the peritoneal isolate 88%…

The inflammatory response which is similar in all forms of peritonitis was recorded by determining the levels of parameters shown to represent the activation state of plasmatic and cellular systems as well as the inhibitory capacity of the plasma. In a selected series of patients with different underlying diseases, blood sampling was started at the time of admission when the clinical finding of an acute abdomen led to emergency laparotomy. Depending upon the duration of the illness and the severity of the peritonitis, a significant increase in fibrinopeptide A and of C3a could be detected with…

Intraluminal meconium calcifications are a rare cause of neonatal abdominal calcifications and can easily be misinterpreted as meconium peritonitis. We report three patients with anorectal anomalies, rectourethral fistula, and intraluminal calcified meconium. Intestinal stasis and mixing of urine and meconium may be predisposing factors for the calcification of meconium. Intraluminal calcifications appear as discrete punctate flecks within the distribution of the bowel, in contrast to meconium peritonitis, where the calcifications are linear and plaque-like, occurring anywhere in the abdominal…

The first reported case occurred of acute peritonitis secondary to a wound infection after minilaparotomy sterilization. The patient's dramatic presentation required a complete exploratory laparotomy.

The effect of prophylactic intraperitoneal aminated glucan on the survival rate and formation of adhesions and abscesses was investigated in rats with acute and subacute peritonitis, respectively induced by cecal perforation and ileal ligation. A significantly reduced mortality was found in both forms of peritonitis. Pretreatment by aminated glucan also significantly reduced the number of abscesses and peritoneal adhesions. An about threefold increase in peritoneal macrophages in aminated glucan-treated rats compared to controls was noted. In vitro studies, using 32P-labelled Escherichia coli,…

The majority of patients admitted to the hospital with left lower quadrant peritonitis and suspected acute diverticulitis are treated empirically without colon radiography either for the duration of their hospital course, or until such time as it is deemed safe to perform a barium enema to confirm this diagnosis. Unlike barium, a water-soluble contrast agent such as Gastrografin can be used safely and accurately immediately on hospitalization. Early establishment of an accurate diagnosis can help eliminate complications, morbidity, mortality, and lengthy hospitalizations. Seventy-one patients …

Intrauterine contraceptive devices (IUD) have been associated with the induction of chronic anaerobic endometritis as well as superimposition of exogenous sexually transmitted diseases and unilateral tubo-ovarian abscesses. We report an unusual case of spontaneous bacterial peritonitis in a patient with ascites and an IUD. Bacteriological data support the concept that the source of infection was a chronic mixed endometritis induced by prolonged use of an IUD (16 years). In the presence of ascites, a localized endometritis with its reservoir of bacteria provided the medium for systemic disease.
