Civil forensic psychiatry - Part 2: specific issues.

OBJECTIVES: This paper describes the main areas of civil forensic psychiatry (FP) and the skills required by psychiatric experts. Some specific areas of civil FP are discussed, including tort law reform, reliability of psychiatric evidence, contentious psychiatric disorders, and the many domains of civil FP. CONCLUSIONS: Civil FP is an important sub-specialty component of forensic psychiatry that requires greater emphasis in the training and continuing education of psychiatrists. A process of accrediting psychiatrists as having competency in advanced civil FP may be of value.

A plea for the diagnosis of hypochondriacal personality disorder.

Hypochondriacal personality disorder is identified as a separate condition by cluster analysis of data from a structured interview for assessing personality disorders, the Personality Assessment Schedule. Close examination of the patients identified as having hypochondriacal personality disorder suggests that they have common characteristics, including excessive preoccupation with the maintenance of health, distorted perception of minor symptoms so that they are elevated to the status of major disease, and frequent medical consultations and disciplines fringing on medicine. In a study of 1000

Partners in crime: A 21-year cohort comparison of people who commit serious crimes together with those who act alone.

BACKGROUND: Perpetrators who act together violently occur frequently in police and media discussions, but are rarely the focus of forensic psychiatric research. AIMS: We aimed to characterise people who act together when committing a serious crime and to map the frequency of such crimes over 21 years in Finland. METHODS: Data for the study were retrieved from the national database of forensic psychiatric examinations for the period 2000-2020, with reports on file for nearly all people charged with serious criminal offences in the country. Index cases were defined as those with two or more pe

[Psychiatric and somatic morbidity of patients with suspected multiple chemical sensitivity syndrome (MCS)].

Multiple chemical sensitivity (MCS) or idiopathic environmental intolerance (IEI) is understood as an acquired disorder with multiple recurrent symptoms that cannot be traced to any well-known medical or psychiatric condition and is associated with diverse environmental influences that are well tolerated by the majority of people. In a prospective study, we investigated 120 consecutive patients admitted a university-based outpatient department for environmental medicine during 1 year. Apart from routine medical examination and special toxicological diagnostic procedures, a structured clinical

[Descriptive approach to latent forms of schizophrenia].

The computerized medical file, used in routine work in an Adult Psychiatry University-Hospital Unit enabled us to select 113 cases among 1,000 consecutive hospitalizations, the diagnosis of which could possibly lead to schizophrenia. These cases which we named "paraschizophrenic states" are linked to DSM III-R criteria of borderline (27 cases), schizoid (40 cases) or schizotypical (15 cases) personalities, schizophreniform trouble and unspecified psychotic trouble (17 cases), brief reactional psychosis (14 cases). We selected 196 cases of schizophrenia in the same cohort of hospitalized patien

The effect of personality on placebo response in panic patients.

Twenty-eight patients who participated as a placebo group in a treatment study of panic disorder were studied to determine the effect of personality on study completion and outcome. Those subjects who completed only 3 weeks had significantly more pathological personality traits than those who continued in the study. For those continuing beyond 3 weeks, there were negative correlations between the paranoid and borderline personality traits and a global outcome measure. Spontaneous panic attacks were not affected by personality.

Homelessness among individuals with psychotic disorders hospitalized for the first time: findings from the Suffolk County Mental Health Project.

OBJECTIVE: Although data suggest that homelessness among persons with severe mental disorders is both distressing and common, several important epidemiologic questions remain unanswered. This study reports on the occurrence of homelessness in a quasi-representative sample of persons newly hospitalized with psychotic disorders. The authors also compared rates of homelessness in different diagnostic groups and among groups with differing symptom profiles. METHOD: The study was based on data from 237 first-admission patients hospitalized at 10 of the 12 inpatient facilities in eastern Long Islan

Clinical study of the relation of borderline personality disorder to Briquet's syndrome (hysteria), somatization disorder, antisocial personality disorder, and substance abuse disorders.

OBJECTIVE: The criteria for borderline personality disorder seem to select patients with very high rates of Briquet's syndrome (hysteria), somatization disorder, antisocial personality disorder, and substance abuse disorders. This study was undertaken to determine whether systematic assessment of patients with borderline personality disorder would reveal characteristic features of that condition which would distinguish it from these other disorders. METHOD: Eighty-seven white female patients (75 in St. Louis and 12 in Milan, Italy) who had borderline personality disorder according to both the

Psychiatric emergencies.

Psychiatric disorders are common in medical inpatient and outpatient populations. As a result, internists commonly are the first to see psychiatric emergencies. As with all medical problems, a good history, including a collateral history from relatives and friends, physical and mental status examination, and appropriate laboratory tests help establish a preliminary diagnosis and treatment plan. Patients with suicidal ideation usually have multiple stressors in the environment and/or a psychiatric disorder (i.e., a major affective disorder, dysthymic disorder, anxiety or panic disorder, psychot

Self and other directed violence on a closed acute-care ward.

Violence to self and to others on a locked, acute-care unit is reported for a three-year period. Drug abuse correlated strongly with both types of violence. Self-violence appeared to be a more predictable event due to its diagnostic specificity, history of prior violence and timing of the episodes. Assaults occurred more randomly demonstrating only a mild association with schizophrenia, but not with other psychotic disorders.

Can antiepileptic drugs unmask a susceptibility to psychiatric disorders?

Psychiatric adverse events have been associated with rapid titration and high doses of most antiepileptic drugs (AEDs). However, several AEDs can cause such events at any dose. These agents include barbiturates, topiramate, levetiracetam and zonisamide. A search of the literature reveals that psychiatric adverse events associated with these AEDs seem not to occur at random. Rather, they are most likely to be reported by patients who have an inherent vulnerability to psychiatric disorders; that is, those with a personal or family history of psychiatric illness. In a recent study by Mula et al.,

Dependency and self-criticism in bipolar and unipolar depressed women.

OBJECTIVES: The dependency and self-criticism scores of women with histories of either unipolar disorder (N = 74) or bipolar disorder (N = 20) or no psychiatric history (N = 24) were compared to determine the influence of state depression on these personality dimensions. DESIGN: All women completed the Depressive Experiences Questionnaire and the Beck Depression Inventory. Patients were divided into currently depressed and remitted groups. RESULTS: Currently depressed women with unipolar disorder were more self-critical and dependent than non-psychiatric controls; women with unipolar disorde

Stroke mortality in patients with mental health disorders; a cohort study from the Spanish hospital discharge records.

INTRODUCTION: Evidence on the clinical outcomes of stroke for patients with psychiatric disorders is poor. It is also unclear if the prognosis of stroke has improved over the last two decades for those with mental health conditions. The aims of this study are first, to compare the hospital mortality between 2019 and 2023 of stroke for patients with and without depression, anxiety, schizophrenia, bipolar, or personality disorders, and second, to investigate whether the stroke mortality in 2019-2023 is lower than in 2005-2009, for those with psychiatric conditions. METHODS: Data from all stroke

Associations between plasma glucose and DSM-III-R cluster B personality traits in psychiatric outpatients.

Associations between personality traits, measured with the Karolinska Scales of Personality, the Impulsiveness subscale from the Impulsiveness, Venturesomeness and Empathy (IVE) Inventory, and with self-assessed personality traits and disorders (SCID-II Screen Questionnaire), and plasma insulin, glucagon and glucose, respectively, were explored in a sample of 101 psychiatric outpatients of both sexes. No relationships between the peptide hormones and personality measures were found. However, fasting glucose values, which were all essentially within the normal biological variation, were signifi

Development of scales to screen for eight common psychiatric disorders.

The present study was designed to create a group of scales from the items on the Symptom Checklist 90 (SCL-90) to identify common psychiatric diagnoses. Subjects were 1457 adult psychiatric outpatients who completed the Symptom Checklist-90 and a structured diagnostic interview at the time of their initial evaluation. A combination of rational and empirical test construction methods was used to create the SCL-90 Diagnostic Scales, item sets that identify eight common psychiatric conditions: major depression, bipolar disorder, schizophrenia, antisocial personality disorder, somatization disorde
