Sexuality and sexual health: constructs and expressions in the extended Middle East and North Africa.

The extended Middle East and North Africa (EMENA) region is the world region with the second youngest population, where globalization, migration, information technology, and political changes are contributing to the shaping of sexuality and sexual behaviors. Understanding the various sociocultural, demographic and public health dimensions of sexual and reproductive health of young people is fundamental to understanding the pattern of sexual behavior and the burden of sexually transmitted infections (STIs), including human papillomavirus-related diseases. New norms and forms of marriage have em

Community-based approaches and partnerships: innovations in health-service delivery in Bangladesh.

In Bangladesh, rapid advancements in coverage of many health interventions have coincided with impressive reductions in fertility and rates of maternal, infant, and childhood mortality. These advances, which have taken place despite such challenges as widespread poverty, political instability, and frequent natural disasters, warrant careful analysis of Bangladesh's approach to health-service delivery in the past four decades. With reference to success stories, we explore strategies in health-service delivery that have maximised reach and improved health outcomes. We identify three distinctive

Time trends of ulcer mortality in Europe.

BACKGROUND & AIMS: The aim of the present study was to follow the time trends of ulcer disease in a representative sample of European countries and assess whether the most recent behavior of peptic ulcer still fits the overall pattern governed by an underlying birth-cohort phenomenon. METHODS: Mortality data from 6 countries between 1921 and 2004 were analyzed, including Denmark, France, Germany, The Netherlands, Spain, and Switzerland. The age-specific death rates of gastric and duodenal ulcers from each individual country were plotted as period-age and cohort-age contours. RESULTS: The dat

Paediatric traumatic brain injury in South Africa: some thoughts and considerations.

PURPOSE: This paper endeavours to describe the socio-political and economic backdrop to the assessment and management of traumatic brain injury (TBI) in children in South Africa. OUTCOMES: The argument put forward is that huge disparities in socio-economic status, diverse socio-cultural influences, and widely differing educational systems and educational attainment which characterize the country's population, render the management of TBI different from approaches adopted in developed countries. The main causes of TBI can be attributed to the high rates of violence in the country, as well as a

Institutionalizing Health Literacy as a Governance Mechanism in Low- and Middle-Income Countries: A Transformative Roadmap from the MENA Region.

Health literacy (HL) is increasingly recognized not merely as an individual asset, but as a structural determinant of health equity and a critical governance instrument for sustainable health systems. While frameworks for "health literate organizations" are well-established in high-income contexts, low- and middle-income countries (LMICs) face similar systemic barriers-including centralized governance, fragile social trust, and resource constraints-that require distinct implementation roadmaps. In this perspective we argue that for health systems in the Middle East and North Africa (MENA) regio

Health Inequalities in Crisis Times: Questions for Global Health Governance.

Health inequalities continue to blight populations creating a global social justice challenge. Despite continued action by policy makers and practitioners, they are recurrent and stubborn, with evidence of exacerbation in many territories. Driven by extractive, capitalist economic and political logics that have organised societies with systemic power imbalances, inequalities are so entrenched they appear ahistorical. Recent global crisis of disease, finance and conflict have intensified challenges, with outcomes worst for those with the least power and resources. Impacts are compounded where i

Governance for planetary health equity-the Planetary Health Equity Hothouse project.

BACKGROUND: Planetary health equity (PHE) is defined here as equitable good health in a stable Earth system. PHE is arguably in crisis. Human-made climate change is damaging global populations through hotter temperatures, wildfires, and more severe and frequent storms, flooding, and landslides. A tsunami of health inequities will result from this, as pre-existing health conditions and inequities in living and working conditions ensure that socially disadvantaged groups and people in low-income and middle-income countries are disproportionately affected by climate change. Despite evidence of th

The crisis of health in a crisis ridden region.

The overall picture of the Arab world today is dire; no Arab country is free from political strife, foreign intervention, or the threat of terrorism. Continuing instability and the failure of national governments to uphold the rule of law and deliver on development are tearing apart the fabric of Arab societies. Internally, Arab regimes' agendas are shaped by security and their makeup by kinship and loyalty, pushing people to seek refuge within their primordial loyalties. From the outside, foreign meddling in the name of "democracy" is deepening the Arab world's instability, therefore providin

Care for low back pain: can health systems deliver?

Low back pain is the leading cause of years lived with disability globally. In 2018, an international working group called on the World Health Organization to increase attention on the burden of low back pain and the need to avoid excessively medical solutions. Indeed, major international clinical guidelines now recognize that many people with low back pain require little or no formal treatment. Where treatment is required the recommended approach is to discourage use of pain medication, steroid injections and spinal surgery, and instead promote physical and psychological therapies. Many healt

Evaluating blood transfusion services in a low- and middle-income country: implications for quality assurance and patient safety.

BACKGROUND: Blood transfusions save millions of lives, but errors in the transfusion process can lead to serious adverse outcomes. There is a need to deliver this service using safe technologies and robust quality systems. In Palestine, the political situation requires full readiness for any emergency and attention to the availability of safe and sufficient blood units. The purpose of this study was to assess blood transfusion services (BTSs) throughout the West Bank, Palestine, to identify key gaps, pinpoint areas of strength, and direct timely enhancements to the safety and quality of the bl

The Cholera Epidemic of 1907 and the Formation of Colonial Epidemic Control Systems in Korea.

It was in 1907 when Korea was annexed by Japan in the field of health care systems as the Gwangje Hospital, Uihakgyo the National Medical School and the Korean Red Cross Hospital were merged into the colonial Daehan Hospital, and massive cholera epidemic controls by the Japanese Army were enforced. However, despite their importance, the cholera epidemic of 1907 in Korea and preventive measures taken at that time have not yet been studied extensively as a single research subject. The purpose of this paper is to contribute to a more concrete and broader understanding of the Korea-Japan annexatio

Social science research in malaria prevention, management and control in the last two decades: an overview.

In the recent past, considerable progress has been made in understanding how human behavior and social organization, macro- and micro-level economic processes, and health and political systems affect responses to malaria at global, national, community, household, and individual levels. Advances in malaria-related social, behavioral, economic, evaluation, health systems, and policy (social science) research have resulted in improvements in the design and implementation of malaria prevention, management and control (PMC) strategies. Indeed, the past two decades chronicle dramatic advances in the…

Work system factors influencing physicians' screen sharing behaviors in primary care encounters.

OBJECTIVE: While the use of electronic health records (EHRs) in primary care has increased dramatically, its potential benefits need to be considered in light of potential negative impacts on physician-patient interactions and the increase in physician cognitive workload. This study aims to understand work system factors contributing to physicians' use of the EHR as a communication tool during primary-care encounters. METHODS: We interviewed 14 primary care physicians on their use of EHRs as a communication tool in patient visits. A qualitative content analysis guided by the work system model

Rudolf Nieuwenhuys (11 June 1927-4 November 2024): a scholarly life.

This collective eulogy by colleagues, co-authors and friends is a tribute to the work and life of Rudolf Nieuwenhuys. 'Neurofascination' is an apt label for his scholarly life in the sciences from the start in 1955 until his last days in 2024. In addition, he had a broad interest in Roman and Gothic architecture, the history and politics of the twentieth century, religion and the music of Johann Sebastian Bach. Extensive discussions on one or more of these topics often led to long-lasting friendships, some of which inform the following pages. Rudolf is remembered for his highly didactical and

Cannabinoids for major neurocognitive disorder: case report and literature review.

INTRODUCTION: Major neurocognitive disorder (MNCD) affects millions of people worldwide. However, the pharmacological options for its management are limited, ineffective and frequently associated with severe adverse reactions. CASE REPORT: An 85-year-old man with history of multiple chronic brain injuries (alcohol-use disorder, haemorrhagic stroke, brain trauma, chronic use of benzodiazepines) developed an MNCD, reaching 7 points on the Reisberg Global Deterioration Scale. He had minimal response to antidepressants, antipsychotics and anticholinergic medications. After the use of mother tinct

[Institutional care for frail elderly from Weimar to Bonn (1924-1961)].

The article deals with the development of institutionalized care and its overall discourse concerning the elderly sick people in Germany from 1924 to 1961. This period of time embraces an ambivalent process of modernization that falls short of any unilinear success story. Neither politics nor the medical sciences had the impact to make the nursing homes catch up with the advanced hospitals. They became low-grade institutions within the national welfare system. These homes evolved from poor-law houses with no specialized care whatsoever. The chronically ill and infirm old people emerged during

Nuclear Medicine in IRAQ: From a Pioneering Past to Future Progress.

This review article explores the trajectory of nuclear medicine in Iraq from its pioneering establishment in 1956 to its re-emergence as a cornerstone of precision medicine. The foundation of the Iraqi Atomic Energy Commission facilitated early regional training and domestic isotope production, fostering a self-sufficient clinical healthcare system. However, decades of conflict, sanctions, and isolation ensued in between 1980 and 2003 led to a profound systemic collapse and the erosion of both technical capacities and human resource continuity. Post-2003 reconstruction catalyzed a profound tra

Could home management of COVID-19 cases alleviate strain on the fragile healthcare system in Damascus, Syria: a prospective cohort study.

The global impact of COVID-19 was devastating, particularly in low-income countries. Conflict zones like Syria, already plagued by political and socioeconomic instability, faced additional hardships, resulting in a fast collapse of the healthcare system. The overwhelmed system necessitated the implementation of home-based management as a practical solution to alleviate strain on hospitals. This study documents the efforts of a volunteer team during the initial COVID-19 wave in Damascus, Syria (August-September 2020) in implementing management through 770 home visits and evaluates its outcomes
