Peripheral nerve disorders are caused by a range of different aetiologies. The range of causes include metabolic conditions such as diabetes, obesity and chronic kidney disease. Diabetic neuropathy may be associated with severe weakness and the loss of sensation, leading to gangrene and amputation in advanced cases. Recent studies have indicated a high prevalence of neuropathy in patients with chronic kidney disease, also known as uraemic neuropathy. Immune-mediated neuropathies including Guillain-Barré syndrome and chronic inflammatory demyelinating polyradiculoneuropathy may cause significan…

BACKGROUND AND AIMS: Immune cells and circulating inflammatory proteins play crucial roles in chronic inflammatory demyelinating polyneuropathy (CIDP) pathogenesis. However, the causal associations between these factors and CIDP remain unclear. Herein, we aimed to explore these associations using a two-sample, two-step Mendelian randomization (MR) approach.
METHODS: Two-sample MR analysis was conducted using genome-wide association studies data to assess links between immune cells, inflammatory proteins, and CIDP. Data on 731 immune cell traits were obtained from a cohort of 3757 Sardinians,

BACKGROUND: Chronic inflammatory demyelinating polyneuropathy (CIDP) is the most common chronic immune-mediated polyneuropathy. Early and effective immunomodulatory treatment is essential to prevent long-term disability. Efgartigimod, a human IgG1 Fc fragment, has recently been approved for adult CIDP, but its safety and efficacy in pediatric CIDP remain unstudied. We retrospectively analyzed two pediatric patients with CIDP who received efgartigimod treatment.
CASE REPORT: Age at onset was 1.5 and 3.4 years; disease duration before efgartigimod was 1.5 and 3.2 years; and age at initiation wa

Molecular mimicry is hypothesized to play a key role in the pathogenesis of acute polyradiculoneuritis (APRN) in dogs, as in Guillain-Barré syndrome (GBS). GBS is linked to Campylobacter jejuni and cytomegalovirus, while Parvovirus B19 shows temporal and serological associations. In dogs, the cause of APRN is often unknown, although recent vaccination or ingestion of raw poultry contaminated with Clostridium perfringens is documented. Antiganglioside antibodies are potential serum biomarkers for APRN. Eight 2.5-month-old Sprocker littermates and a 3-month-old female Cockapoo developed severe g

Combined central and peripheral demyelination (CCPD) is a rare neuroimmunological condition involving inflammatory demyelination of both the central nervous system (CNS) and peripheral nervous system (PNS). We report a chronic progressive CCPD case initially diagnosed as chronic inflammatory demyelinating polyneuropathy (CIDP) and treated with conventional CIDP-directed immunotherapies, with subsequent development of multiple sclerosis (MS)-like CNS demyelination. An extensive diagnostic evaluation excluded alternative infectious, metabolic, paraneoplastic, and antibody-mediated etiologies aff

POEMS syndrome (Crow-Fukase syndrome) is a rare lymphoproliferative disease of significant interest to neurologists, since its first and most common manifestation is a progressive demyelinating sensorimotor polyneuropathy. The name of the syndrome reflects the key symptoms: Polyneuropathy, Organomegaly, Endocrinopathy, the presence of M-protein, and Skin changes. The article presents a clinical case of POEMS syndrome in a 32-year-old patient, illustrating the challenging diagnosis. It is emphasized that the disease often remains unrecognized for several years due to the low awareness of doctor

The updated 2021 criteria of the European Academy of Neurology/Peripheral Nerve Society (EAN/PNS) for chronic inflammatory demyelinating polyneuropathy (CIDP) has excellent diagnostic sensitivity and specificity for typical and variants of CIDP. For focal CIDP, applications of these criteria may be different due to limited peripheral nerve involvement. We illustrate using a rare case of focal CIDP on the applicability of 2021 EAN/PNS criteria and its practical limitations. A young woman presented with progressive right foot drop and numbness over 5 years. Clinically, she had areflexic right lo

The efficacy of immunotherapies such as steroids, plasmapheresis, and intravenous immunoglobulin have been proven in various immune-mediated neuropathies. However, these treatments sometimes lack the efficacy in a part of patients with the immune-mediated neuropathies. In addition, anti-myelin associated glycoprotein (MAG) neuropathy is usually refractory to the treatments. Recently, novel therapies targeting a molecule which are associated with pathogenesis of immune-mediated diseases, have been developed. These molecularly targeted therapies are notable in immune-mediated neuropathies as nov

INTRODUCTION/AIMS: Chronic inflammatory demyelinating polyneuropathy (CIDP) may be rarely preceded by infection. A causative link remains unproven, in contrast to Guillain-Barré syndrome (GBS), which is commonly postinfectious with well-demonstrated pathophysiological mechanisms of molecular mimicry following Campylobacter jejuni enteritis. Uncommonly, infections are reported before the onset of CIDP. In this study we aimed to determine the frequency and characteristics of CIDP occurring after antecedent infections or vaccinations in two large European cohorts.
METHODS: We reviewed the record

A 9-year-old female mixed-breed dog presented with ascending flaccid tetraparesis, and a 5-year-old castrated male Poodle dog presented with ventroflexion of neck, dysphonia, and hindlimb weakness, which progressed to acute ascending tetraparesis. Both dogs were fed raw poultry for over 9 and 5 years, respectively. Blood examination and other test results were normal or unrelated to the present case. Fecal polymerase chain reaction analysis in the Poodle dog was positive for Clostridium perfringens and Campylobacter jejuni. Tetraparesis improved with supportive care in both dogs. Human IV immu

Paraproteinemia is associated with different peripheral neuropathies. The major causes of neuropathy correlated with paraproteinemia are the deposition of immunoglobulin in the myelin, represented by anti-myelin-associated glycoprotein (MAG) neuropathy; deposition of immunoglobulin or its fragment in the interstitium, represented by immunoglobulin light chain amyloidosis (AL amyloidosis); and paraneoplastic mechanisms that cannot be solely attributed to the deposition of immunoglobulin or its fragment, represented by polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, and skin

Immunoglobulin G (IgG) is a commonly used treatment for chronic neuromuscular disorders (NMDs), such as chronic inflammatory demyelinating polyneuropathy and multifocal motor neuropathy. IgG therapy has also shown promise in treating other NMDs including myasthenia gravis, polymyositis, and dermatomyositis. IgG is administered as either intravenous immunoglobulin (IVIg) or subcutaneous immunoglobulin (SCIg), with SCIg use becoming more popular due to the treatment burden associated with IVIg. IVIg requires regular venous access; long infusions (typically 4-6 hours); and can result in systemic

Background Traumatic brachial plexus injuries affect 1% of patients involved in major trauma. MRI is the best test for traumatic brachial plexus injuries, although its ability to differentiate root avulsions (which require urgent reconstructive surgery) from other types of nerve injury remains unknown. Purpose To evaluate the accuracy of MRI for diagnosing root avulsions in adults with traumatic brachial plexus injuries. Materials and Methods For this systematic review, MEDLINE and Embase were searched from inception to August 20, 2018. Studies of adults with traumatic nonpenetrating unilatera

The classic immunologic alteration of the cerebrospinal fluid (CSF) in Guillain-Barré syndrome (GBS), albuminocytologic dissociation, has been known since the original paper by Guillain, Barré, and Strohl. Albuminocytologic dissociation has been also described in other forms of the GBS spectrum, such as axonal motor or motor-sensory forms (AMAN, AMSAN), the anti-GQ1b spectrum of Miller Fisher syndrome, and Bickerstaff brainstem encephalitis. Cytokines, chemokines, antibodies, complement components, and molecules with a putative neuroprotective role or indicating axonal damage have also been ex
