Posterior Epidural Migration of a Sequestered Lumbar Intervertebral Disc Fragment.

AIM: Posterior epidural migration of a sequestered lumbar intervertebral disc fragment (PEMSLIDF) is an extremely rare condition published so far only as case reports or small case series (ranging between 2 to 8 cases). Diagnosing this condition is often challenging and the diagnosis is usually made intraoperatively. The affected patients usually suffer cauda equina syndrome (CES). In the present study, we aimed to discuss the clinical and radiological findings, types and features of surgical therapies, and outcomes of 9 patients with PEMSLIDF. MATERIAL AND METHODS: This study included 9 (0.3

Lumbar spinal canal MRI diameter is smaller in herniated disc cauda equina syndrome patients.

INTRODUCTION: Correlation between magnetic resonance imaging (MRI) and clinical features in cauda equina syndrome (CES) is unknown; nor is known whether there are differences in MRI spinal canal size between lumbar herniated disc patients with CES versus lumbar herniated discs patients without CES, operated for sciatica. The aims of this study are 1) evaluating the association of MRI features with clinical presentation and outcome of CES and 2) comparing lumbar spinal canal diameters of lumbar herniated disc patients with CES versus lumbar herniated disc patients without CES, operated because

Spontaneous regression of posterior epidural migrated lumbar disc fragments: case series.

BACKGROUND CONTEXT: Posterior epidural migrated lumbar disc fragments is an extremely rare disorder. Surgical treatment was performed in all reported cases. To the best of our knowledge, there are no reported cases of the use of conservative treatment for posterior epidural migrated lumbar disc fragments. PURPOSE: To report the possibility of a spontaneous regression of posterior epidural migrated lumbar disc fragments. STUDY DESIGN: Case series. METHODS: Four patients with posterior epidural migrated lumbar disc fragments were treated at Karatsu Red Cross Hospital between April 2008 and Au

Is cauda equina syndrome linked with obesity?

No previous studies have examined the physical characteristics of patients with cauda equina syndrome (CES). We compared the anthropometric features of patients who developed CES after a disc prolapse with those who did not but who had symptoms that required elective surgery. We recorded the age, gender, height, weight and body mass index (BMI) of 92 consecutive patients who underwent elective lumbar discectomy and 40 consecutive patients who underwent discectomy for CES. On univariate analysis, the mean BMI of the elective discectomy cohort (26.5 kg/m2 (16.6 to 41.7) was very similar to that

[Bladder deformations in neurogenic bladder secondary to cauda equina or conus medullaris lesion].

INTRODUCTION: The bladder deformations observed in the neurogenic bladders are frequent, particularly in the "upper motor neuron" bladder type (paraplegia, multiple sclerosis). We wanted to verify the predictive factors of such damage and particularly, if the urodynamic typology intervened in their genesis by using the model of cauda equina syndrome and conus medullaris lesions. PATIENTS AND METHODS: We have studied retrospectively the presence of cystographic bladder deformations at patients with neurogenic bladder due to cauda equina syndrome or conus medullaris lesion according to their ur

Sensory disturbances and pain complaints after brachial plexus root injury: a prospective study involving 150 adult patients.

After injury of the brachial plexus, sensory disturbance in the affected limb varies according to the extent of root involvement. The goal of this study was to match sensory assessments and pain complaints with findings on CT myelo scans and surgical observations. One hundred fifty patients with supraclavicular stretch injury of the brachial plexus were operated upon within an average of 5.4 months of trauma. Preoperatively, upper limb sensation was evaluated using Semmes-Weinstein monofilaments. Pain complaints were recorded for each patient. With lesions affecting the upper roots of the brac

Cauda equina entrapment in a pseudomeningocele after lumbar schwannoma extirpation.

Incidental or intentional durotomy causing cerebrospinal fluid (CSF) leakage, leading to the formation of a pseudomeningocele is a known complication in spinal surgery. Herniation of nerve roots into such a pseudomeningocele is very rare, but can occur up to years after initial durotomy and has been described to cause permanent neurologic deficit. However, cauda equina fiber herniation and entrapment into a pseudomeningocele has not been reported before. Here, we present a case of symptomatic transdural cauda equina herniation and incarceration into a pseudomeningocele, 3 months after extirpat

Lumboperitoneal shunt for treatment of dural ectasia in ankylosing spondylitis.

Neurological complications of ankylosing spondylitis (AS) are reported in 2.1% of patients. Cauda equina syndrome (CES) is rare and occurs at the ankylosing stage. MRI and CT of the lumbar spine show a cauda equina deformation with dural ectasia and bony erosion. We report three patients with AS presenting with progressive CES. These patients underwent lumboperitoneal shunting (LPS) surgery. The motor deficit improved in all cases. We suggest that CES develops from arterial pulsation of the CSF on a dural sac with reduced elasticity and that LPS reduces these intradural pressure shock waves. A

Intradural lumbar disc herniation--is it predictable preoperatively? A report of two cases.

BACKGROUND CONTEXT: Intradural disc herniations are thought to be rare events, and there have been relatively few literature reports of intradural disc herniations available with regard to magnetic resonance imaging findings. PURPOSE: The authors describe two patients with intradural lumbar disc herniations, one with and one without preoperative diagnosis, who had different postoperative outcomes. STUDY DESIGN: Case study METHODS: The first patient underwent an extended L3 subtotal laminectomy followed by bilateral medial facetectomy and foraminotomy at L3-L4. A durotomy uncovered large dis

Extraskeletal Ewing's sarcoma presenting with neurological symptoms. Report of two cases.

Extraosseous Ewing's sarcoma (EES) is an uncommon condition that may develop in soft tissues at any location. Neurological symptoms leading to diagnosis have been reported exceptionally. We report two cases of EES in patients aged 16 and 74 years, presenting with cauda equina syndrome and radicular ulnar involvement respectively. Radiological images showed a paraspinal mass in both cases and biopsy disclosed the histological features of Ewing's sarcoma. EES should be kept in mind in the differential diagnosis of paraspinal soft tissues tumours at any age.

[Physiotherapy in spinal stenosis].

Spinal stenosis mainly is a disease of the elderly patient. Mostly the lumbar spine is affected. The assessment is based on the typical constellation of symptoms (neurogenic claudication, subjective weakness) and physical findings (abnormal reflex status, loss of strength, sensory deficits). It further is supported by the radiographic proof of stenosis of the spinal canal, the lateral recess and the intervertebral foramina. The targets of physical therapy are the relief of pain and an improvement concerning the activities of daily living, which are especially impaired by reduced walking distan

[Cauda equina neurography. Results from normal persons and patients with lumbosacral root compression syndrome].

The results with the cauda-equina-neurography in more than 150 patients demonstrate a value for differentiation of lumbosacral disorders. The action potentials of normal adults are mostly biphasic, have the same configuration for both sides and their amplitude is between 0,3 and 20 uV. The conduction velocity decreases with age for both nerves. In cases of lumbosacral disorders with isolated nerve root affection by discogenic compressions the potentials are delayed, reduced, polyphasic or absent. Mostly the clinical, electromyographic, myelographic and surgical results correspond with neurogra

Bilateral sixth nerve palsy. Analysis of 125 cases.

Bilateral abducens nerve pareses were nearly as common as unilateral cases in an inpatient setting (125:143). Cerebrospinal fluid abnormalities were more frequent among the bilateral cases, but generally the same causes produced unilateral and bilateral sixth nerve palsy. The relative ease of diagnosis was in contrast with the large number of undiagnosed or "vascular" cases in previous studies of outpatients. The degree of lateral rectus limitation proved to be of limited help in suggesting the cause or predicting recovery of oculomotor function. The etiology was of some prognostic value, with

F-wave conduction velocity in the central segment of the peroneal and tibial nerves.

A simple method was devised to measure the F-wave conduction velocity (FWCV) in the central segment (knee to spinal cord) of the peroneal and tibial nerves in contrast to the conventional motor nerve conduction velocity (MNCV) in the distal segment (knee to ankle). In 33 normal subjects, the FWCV in the central segment (56.3 +/- 4.9 meters/sec for peroneal nerve; 54.4 +/- 3.6 meters/sec for tibial nerve) was slightly faster than the MNCV in the distal segment (49.5 +/- 3.8 meters/sec; 46.8 +/- 3.4 meters/sec) as measured in the conventional manner. In 4 of 14 patients with the Guillain-Barrĕ s
