Sleep apneas, convulsive syncopes and autonomic impairment in type I Arnold-Chiari malformation.

A 51-year-old man with type I Arnold-Chiari malformation presented with severe autonomic impairment. Polysomnography disclosed central and obstructive sleep apneas. During REM sleep apneas were accompanied by hypotension and led to convulsive nonepileptic attacks. Marked blood pressure oscillations and central apneas persisted after tracheostomy.

Pilot screening for prodromal Parkinson's disease in post-9/11 Veterans with probable rapid eye movement sleep behavior disorder.

This pilot examined prodromal markers in post-9/11 Veterans with TBI and probable REM sleep behavior disorder (N = 12). RBD was confirmed in 5 of 10 (50 %) participants with polysomnography. Veterans with subthreshold parkinsonism had lower olfactory scores. Future studies should quantify REM sleep without atonia and include broader markers.

Restless legs syndrome and nocturnal myoclonus: initial clinical manifestation of familial amyloid polyneuropathy.

Restless legs syndrome was the first isolated clinical manifestation in four siblings of a family with familial amyloid polyneuropathy. Clinical and electrophysiological evidence of peripheral neuropathy appeared after a variable time interval. Polysomnography showed abnormal sleep patterns and nocturnal myoclonus in all patients. The restless legs syndrome responded favourably to clonazepam.

Actigraphy in human African trypanosomiasis as a tool for objective clinical evaluation and monitoring: a pilot study.

BACKGROUND: Human African trypanosomiasis (HAT) or sleeping sickness leads to a complex neuropsychiatric syndrome with characteristic sleep alterations. Current division into a first, hemolymphatic stage and second, meningoencephalitic stage is primarily based on the detection of white blood cells and/or trypanosomes in the cerebrospinal fluid. The validity of this criterion is, however, debated, and novel laboratory biomarkers are under study. Objective clinical HAT evaluation and monitoring is therefore needed. Polysomnography has effectively documented sleep-wake disturbances during HAT, bu…

Adenotonsillectomy in obstructive sleep apnea syndrome. Proposal of a surgical decision-taking algorithm.

OBJECTIVE: The incidence of snoring in the pediatric population is 10% and the incidence of OSAS is equal to 2-3%. Sometimes, primary snoring and OSAS overlap and only polysomnography can differentiate the two disorders but its use is complex, expensive and highly controversial. The purpose of this paper is to demonstrate the value of the clinical assessment associated with the patient's history in selecting children with OSAS and to validate the therapeutic algorithm, thereby employing polysomnography only for selected cases. METHODS: A population of 118 patients was considered, selected by …

[Sleep apnea syndrome in children].

Obstructive sleep apnea is characterized by prolonged, generally partial, upper airways obstruction associated with hypoxemia and/or hypercapnia. Main etiological factors include hypertrophy of the tonsils and adenoids. Polysomnography is the gold standard for diagnosis. Diagnosis of obstructive sleep apnea must be established early in order to prevent complications. Adenotonsillectomy is an effective therapy.

[Characteristics of narcolepsy in childhood].

Narcolepsy is a rare disorder with brief daytime sleep periods, transient loss of muscle tone in emotional situations (cataplexy), dream mentation during short periods of sleep (hypnagog hallucinations), and transient inability to move on waking (sleep paralysis), affecting only 2–3000 people in Hungary. Despite its characteristic symptoms, it is difficult to recognise the disease and there is often a delay of decades before a correct diagnosis and hence effective therapy. Cases with a childhood onset require special attention, as the classic complaints mentioned above are accompanied by a num

Restless Sleep Disorder.

Restless sleep disorder is (RSD) a condition characterized by frequent large movements during sleep associated with daytime impairment. RSD has been studied in children aged 6 to 18 years. Polysomnography is necessary for the diagnosis of RSD. The current diagnostic criteria include more than 5 large movements per hour of sleep documented by PSG. The pathophysiology is not known yet, but iron deficiency and sleep instability and increased sympathetic activation are suspected to play a role. Iron supplementation is the only treatment option studied so far.

[Launois-Bensaude disease and sleep apnea syndromes. Apropos of a recent case].

About a recent case of Launois-Bensaude's disease (Madelung's disease) the authors discuss the possibility of complications such as bronchopathy and sleep apnoea syndrome. General anaesthetics have to be performed with care, the anaesthetic risk exists even so during the premedication. Such patients have to be explored before the general anaesthetics by sleep polysomnography.

The role of the WatchPAT device in the diagnosis and management of obstructive sleep apnea.

Obstructive sleep apnea (OSA) is a common condition affecting an estimated 936 million individuals worldwide, leading to a considerable demand for diagnostic services. Polysomnography, the current gold standard for diagnosis, is resource intensive and inconvenient for patients and healthcare providers. The WatchPAT is an unobtrusive device for home OSA diagnosis. It utilizes peripheral arterial tomography in conjunction with heart rate, oximetry, actingraphy, and respiratory movements for the diagnosis of OSA. It has good correlation with polysomnography for OSA diagnosis and also reports slee…

The Clinical Usefulness of Sleep Studies in Children.

Sleep disordered breathing is common in children and has the potential to have a significant impact on cognition, activity and social interaction. The overnight in-laboratory polysomnography (PSG) continues to be the gold standard instrument for the investigation of sleep-disordered breathing in children. It has the ability to rule in or rule out the need for intervention for common conditions such as obstructive sleep apnoea, assess the role of sleep quality in children and adolescents with hypersomnolence, provide physiologic data in children with hypoventilation as may be seen in neuromuscu…

Heart rate variability during REM and non-REM sleep in preterm neonates with and without abnormal cardiorespiratory events.

AIM: Analyse heart rate variability (HRV) of preterm neonates undergoing a polysomnography in relation to the occurrence of abnormal cardiorespiratory events on one hand and the type of sleep states on the other hand. METHODS: To quantify nonlinear HRV, the numerical noise titration technique is used, adapted to neonatal heart rate data. HRV is calculated for 30 preterm neonates with mean post-conceptional age of 36.4weeks, divided into three groups according to the occurrence of abnormal events during the polysomnographies and the eventual home monitoring. RESULTS: Periods of non-REM sleep …
