[Combined lateral gaze palsy with unilateral internuclear ophthalmoplegia in high mesencephalic-pontine hemorrhage. Report of a case].

The authors report a case of right midbrain and high pons haemorrhage with complete external ophthalmoplegia, except bilateral convergence, voluntary and automatic-reflex abduction of the left eye and adduction of this eye with vestibulo-ocular reflex ("one-and-a-half syndrome"). There is voluntary-automatic dissociation of the eyelids motricity and the vertical gaze. Still, there is sensitive-motor hemiplegia at left and asterixis.

Motor recovery mechanism in a quadriplegic patient with locked-in syndrome.

Locked-in syndrome (LIS) is a rare neurologic condition caused by bilateral pontine lesions. Quadriplegia is one of the most serious clinical manifestations in patients with LIS. However, little is known about the motor recovery mechanism of quadriplegia in patients with LIS. In the current study, we present with a quadriplegic patient with bilateral pontine infarcts, whose motor function appeared to be reorganized into the peri-infarct areas of the infarcted pons, as demonstrated by diffusion tensor tractography (DTT). A 60-year-old was diagnosed as LIS due to bilateral pontine infarcts 6 yea…

Isolated pontine infarcts: etiopathogenic mechanisms.

Although there are several clinico-topographical studies of pontine infarcts, few include vascular studies. To clarify the etiopathogenic mechanisms of pontine infarcts we analyzed the vascular findings and their association with MRI lesions. The clinical features and vascular findings on transcranial Doppler (TCD) or MR angiography (MRA) of 67 patients with acute infarcts involving the pons were studied. Functional outcome was assessed by modified Rankin Scale (mRS) scores on admission and 2 months later. Two groups of isolated pontine infarcts were found on the basis of lesion location on MR…

Protective role of Vaccinium myrtillus's (Bilberry) antioxidant and anti-inflammatory effects in experimental lower limb ischemia-reperfusion injury.

BACKGROUND: Skeletal muscle ischemia-reperfusion (IR) injury is a common clinical problem encountered in vascular and orthopedic surgery, trauma, and transplantation, and is characterized by pronounced oxidative stress and structural tissue damage. Vaccinium myrtillus (bilberry) is rich in anthocyanins and has demonstrated antioxidant and vasoprotective properties in various organ systems; however, its effects on skeletal muscle IR injury have not been previously investigated. METHODS: Twenty-four male Wistar rats were randomly assigned to four groups (n = 6 per group): Sham (surgical exposur

Adipokines as Prognostic Biomarkers in Multiple Myeloma: A Case-Control Study.

Background and Objectives: Multiple myeloma (MM) remains an incurable plasma cell malignancy with heterogeneous clinical outcomes. Although current prognostic systems integrate biochemical and cytogenetic parameters, they do not fully capture disease complexity. Adipocytes within the bone marrow microenvironment secrete adipokines that regulate inflammation, metabolism, and immune interactions and may influence disease progression. This study aimed to assess circulating adipokines and related microenvironmental mediators as potential biomarkers of disease activity and treatment response in MM.…

A Case Report of Wall-Eyed Bilateral Internuclear Ophthalmoplegia with Bilateral Facial Palsy.

Case reports of bilateral facial palsy with horizontal gaze restriction are rare. A 62-year-old woman experienced sudden onset of bilateral adduction deficits, bilateral abducting nystagmus accompanied with facial diplegia. We confirmed acute ischemic stroke in the midline dorsal pons, where medial longitudinal fasciculus (MLF) and facial nerve fascicles are located. This can be explained by vascular variation of pontine perforating arteries.

Ophthalmoplegic migraine with reversible enhancement of intraparenchymal abducens nerve on MRI.

We describe a patient with ophthalmoplegic migraine and right abducens nerve palsy, in whom serial magnetic resonance imaging showed a transient, gadolinium-enhancing lesion in the right lower pons, during both headache and the headache-free period. The enhancing linear lesion was felt to represent intraparenchymal fibers of the affected abducens nerve. The possible pathophysiology of this unique finding is discussed.

Pontine anosognosia for hemiplegia.

Four patients had anosognosia for hemiplegia (AHP) as a manifestation of pontine infarction in the mediolateral region. Patients with AHP syndrome had no mental and neuropsychologic disturbances, and all had involvement of the medial or lateral part of the pons (medial or lateral pontine reticular nuclei). Brainstem lesions, which activate frontoparietosubcortical areas, may be a critical factor in the development of AHP syndrome.

Pontine tegmentum hematoma: a case report with the "one-and-a-half" syndrome without pyramidal tract deficit.

The author reports the case of a 54-year-old male patient with a pontine hematoma and with the one-and-a-half syndrome, cerebellar ataxia and no signs of pyramidal tract involvement. The absence of involvement of the pyramidal tract in the case reported herein is likely due to variation in the vascular anatomy of the pons. The pathophysiologic mechanisms of the one-and-a-half syndrome and of the clinical findings recorded are discussed.

Infarction in the territory of the anterior inferior cerebellar artery: report of five cases.

The clinical and MRI features were correlated in five cases of infarction in the territory of the anterior inferior cerebellar artery. The lateral portion of the pons area was affected in four cases, the middle cerebellar peduncle in two, and the cerebellar hemisphere in three. The lesion was restricted to the cerebellar hemisphere in one patient. In no case did the clinical features conform to the classical description.

Neurotrophin signaling is required for the correct formation of the precerebellar nuclei in mouse embryos.

BACKGROUND: In the developing brain, neuronal migration is one of the critical steps in the establishment of an accurate neural network. When neurons reach their final destination, they form layered structures or nuclei. Precerebellar neurons (PCNs) form several nuclei in the pons and medulla and project mossy and climbing fibers to the cerebellum. PCNs originate from the lower rhombic lip in the dorsal hindbrain and migrate tangentially toward their destinations. When PCNs reach their presumptive nucleus-forming region, they change their migration direction from tangential to radial to form t…

Eight-and-a-Half Syndrome: An Uncommon but Potentially Life-Threatening Disease Secondary to Acute Brainstem Infarction.

A localized pontine lesion affecting the abducens nucleus or paramedian pontine reticular formation (PPRF), the medial longitudinal fasciculus (MLF), and the facial nerve fascicle results in eight-and-a-half syndrome, a rare neurological condition. It manifests as ipsilateral peripheral facial nerve palsy, internuclear ophthalmoplegia (INO), and horizontal gaze palsy. This condition typically indicates a minor but well-placed infarct in the dorsal pons. We diagnosed eight-and-a-half syndrome in a patient who presented with dizziness and double vision, with a background history of hypertension …

Of generators, networks and migraine attacks.

PURPOSE OF REVIEW: One of the most discussed topics in migraine pathophysiology is where migraine attacks originate. Although recent evidence suggests central attack generating loci, there is an ongoing debate about the involved centres of the brain and brainstem. RECENT FINDINGS: Recent neuroimaging studies focussing on the preictal stage of migraine attacks suggest a predominant role of the hypothalamus and its functional connectivity shortly before the beginning of migraine headaches. In interictal migraineurs, changes in resting state functional connectivity of the dorsal pons and the hyp…
