A rare case of the axillary necrotizing fasciitis.

Necrotizing fasciitis is a rapidly progressive, life-threatening soft tissue infection that needs acute surgical intervention and other types of supportive care. The upper extremities are a less frequent site for this infection than the lower extremities. Axillary necrotizing fasciitis is quite rare and bears a high risk of iatrogenic neurovascular injuries. We report a severe case of a 39-year-old man with a right axillary necrotizing fasciitis caused by Streptococcus pyogenes. While dealing with important nerves and blood vessels at the initial emergency surgery, we marked and preserved them

A 46-Year-Old Man With Dyspnea, Hypoxemia, and Radiographic Asymmetry After Redo Bilateral Lung Transplantation.

A 46-year-old man underwent redo bilateral sequential lung transplantation for rapidly progressive bronchiolitis obliterans syndrome that developed 3.5 years after initial transplantation. In the operating room, he was sedated and intubated with a dual lumen endotracheal tube with subsequent single right-lung ventilation and left allograft implantation. His pulmonary arterial pressure became elevated with reperfusion of the newly implanted left lung, which required initiation of cardiopulmonary bypass to facilitate implantation of the right lung. After implantation and reperfusion of the right

Efficacy and safety of obinutuzumab on progressive IgA nephropathy: a case series.

IgA nephropathy (IgAN) is the most prevalent glomerulonephritis globally, significantly contributing to kidney failure. B cells are central to its pathogenesis through IgA production. While rituximab is commonly used to deplete B cells, obinutuzumab, a type II anti-CD20 antibody, may provide more effective and sustained depletion. This report regards the efficacy and safety of obinutuzumab in patients with progressive IgAN refractory to other immunosuppressive therapies. We discuss three patients with progressive IgAN aged 21, 35, and 57 years. All patients exhibited significant proteinuria an

Hearing Stability in Patients With Unilateral Hearing Loss Due to Congenital CMV.

OBJECTIVE: To determine the incidence and severity of progressive hearing loss in both ears in a population of longitudinally-tracked patients with unilateral hearing loss due to congenital cytomegalovirus (cCMV). By determining the natural history of unilateral hearing loss due to cCMV infection, we aim to facilitate therapeutic recommendations. STUDY DESIGN: Retrospective chart review. SETTING: Two tertiary care hospitals. METHODS: Pure-tone averages (PTAs) based on yearly audiograms were calculated for each patient for both ears, and changes were assessed using a linear mixed-effects mod

Incidental diagnosis of Bardet-Biedl syndrome in a case of abdominal tuberculosis: a case report.

BACKGROUND: Bardet-Biedl syndrome is a rare autosomal recessive disease occurring due to a ciliopathic genetic defect. It is caused by mutations in genes encoding proteins vital for the BBSome complex. This complex is essential for ciliary function and cellular signaling. It has multisystem involvement and presents with a variety of phenotypes. CASE PRESENTATION: A 30-year-old adult male patient, Indian by ethnicity, presented with a 2-week history of ascites and dyspnea. The ascitic fluid analysis confirmed abdominal tuberculosis. However, the patient showed other symptoms and signs of a syn

Disseminated Intravascular Coagulation With a Predominant Thrombotic Phenotype in Severe Plasmodium falciparum Malaria: A Case Report.

Severe Plasmodium falciparum malaria may be complicated by disseminated intravascular coagulation (DIC) with heterogeneous clinical manifestations. We report the case of a 61-year-old man with recent travel to Angola who presented with severe Plasmodium falciparum malaria complicated by DIC with a predominant thrombotic phenotype. On admission, he exhibited profound thrombocytopenia, markedly elevated D-dimer levels, metabolic acidosis, shock, and evidence of multiorgan dysfunction. Peripheral blood smear demonstrated parasitemia exceeding 25%. Despite prompt initiation of intravenous artesuna

Schwannoma causing chronic medial calf pain in a recreational skiing and running athlete: a case report.

BACKGROUND: Soft tissue masses in the extremities of athletes present commonly in clinical practice. Clinicians focusing on musculoskeletal care will be familiar with common causes such as myofascial trigger points, hematomas and even lipomas, but may overlook less common causes such as benign or metastatic tumours. This paper describes a case of a schwannoma, a benign nerve tumour. CASE PRESENTATION: A 28-year-old recreational athlete presented with a five-year history of progressive calf pain and a palpable nodule unresponsive to conservative care. Imaging revealed a well-circumscribed intr

Idiopathic Scrotal Elephantiasis: Case Report of a Rare Occurrence.

Idiopathic scrotal elephantiasis is a rare yet physically and psychologically debilitating condition characterized by chronic lymphedema of the scrotum. This case report presents an 18-year-old male with progressive scrotal enlargement over seven years. The patient applied to our clinic due to discomfort and occasional discharge. Clinical examination revealed scrotal edema without systemic involvement. Diagnostic imaging excluded other pathologies. Surgical intervention was planned, and 1.2 kg of excess tissue was meticulously excised while preserving the testes and spermatic cords. No complic

Immunological and molecular diagnosis of cysticercosis.

Cysticercosis, the infection with the larval stage of Taenia solium, is a cause of neurological symptoms including seizures, affecting the quality of life of patients and their families. Diagnosis focuses on brain imaging and serological tests are mostly used as confirmatory tools. Most cases, however, occur in poor endemic areas, where both kinds of diagnostic tools are poorly available. Development of point of care diagnostic tests is one of the most important priorities for cysticercosis researches today. The ideal point of care test would require detection of viable cysticercosis and hopef

Long-term care, rehabilitation, and legal and ethical considerations in the management of neuromuscular disease with respiratory dysfunction.

Neuromuscular disease, while for the most part progressive, allows for ample time to discuss options for long-term care. Frank discussions among clinicians, the patient, and family will allow for an ethical decision process. With the clinician's awareness of the inherent difficulties in judging life satisfaction, an early discussion of first noninvasive measures, then tracheostomy with positive pressure ventilation, will allow for a rational, moral, and continuing treatment plan. The early introduction of rehabilitation techniques should allow for maximal time in the home setting where life sa

Atrial fibrillation patterns are associated with arrhythmia progression and clinical outcomes.

OBJECTIVES: Determinants of atrial fibrillation (AF) patterns and of progression of earlier forms to permanent AF, and their relationship with outcome are still poorly understood. METHODS: We examined AF patterns (paroxysmal, persistent and permanent), rate and predictors of AF progression, and outcomes in the PREFER (PREvention oF thromboembolic events-European Registry) in AF. The primary analysis was performed in the PREFER in AF prolongation dataset (n=3223 patients with AF with a complete 1-year follow-up, mean age 72±9 years, 40% women). Sensitivity analyses were performed using the PRE

Weakness Due to Anemia? Go Fish! Melena as a Red Herring in the Diagnosis of Statin-Induced Myopathy.

Statins are a ubiquitous medication class in the primary care setting where they provide effective primary and secondary prevention of coronary artery disease by lowering cholesterol. While statins are mostly safe, muscle-related adverse events are well described. Very rarely patients can actually develop elevated creatine kinase (CK) consistent with myonecrosis. We present a case of progressive anti-hydroxymethylglutaryl coenzyme A reductase (anti-HMGCR) inflammatory myopathy, which was misdiagnosed for many months. Our patient was a 67-year-old gentleman sent to the ER by the Internal Medici

The prevalence of intrinsic host risk factors associated with progressive disease in patients with chronic hepatitis C viral infections.

The prevalences of three risk factors that have been identified as important predictors of more progressive forms of chronic hepatitis C viral (HCV) infections (male gender, transfusion recipients and age greater than 50 years at the onset of infection) were documented by a retrospective chart review of 337 HCV-infected patients attending an urban, hospital-based, viral hepatitis clinic. One hundred and ninety-five patients (58%) were male. One hundred and eighteen (35%) had received blood or blood product transfusions in the past, 33% of whom also gave a history of intravenous drug use. Appro

The Future of Interventional Keratoconus Management: Perspectives on Integrating Epithelium-on Oxygen-Enriched Cross-Linking into Comprehensive Care Pathways.

This perspective presents a new conceptual framework for the comprehensive care of patients with keratoconus. Keratoconus is an ectatic degeneration of the cornea associated with progressive vision loss and diminished quality of life. Until recently, treatment consisted of optical correction in early stages and corneal transplantation in later stages, neither of which impacted the progressive nature of the disease. The development of corneal cross-linking-a minimally-invasive procedure that stabilizes the disease process and prevents progressive ectasia and vision loss-has significantly altere

Cervical Agenesis and Compressive Hydronephrosis in a Patient With Limited English Proficiency.

Cervical agenesis is an extremely rare congenital anomaly of the female reproductive tract characterized by normal fallopian tubes, an absent cervix, a functioning uterine body, and, occasionally, partial or total vaginal aplasia. It often presents with primary amenorrhea and cyclic pelvic pain. Delayed diagnosis can lead to severe morbidity from retrograde menstruation, including hematometra, hematosalpinx, and endometriosis. Patients with limited English proficiency (LEP), particularly Haitian women, face compounded systemic hurdles when navigating the healthcare system, significantly increa

Expert recommendations and clinical considerations in the use of onasemnogene abeparvovec gene therapy for spinal muscular atrophy.

Spinal muscular atrophy (SMA) is an autosomal recessive, neurodegenerative disease caused by biallelic mutations in the survival motor neuron 1 (SMN1) gene. SMA is characterized by motor neuron degeneration, resulting in progressive muscle atrophy and weakness. Before the emergence of disease-modifying therapies, children with the most severe form of SMA would never achieve the ability to sit independently. Only 8% survived beyond 20 months of age without permanent ventilator support. One such therapy, onasemnogene abeparvovec, an adeno-associated virus-based gene replacement therapy, delivers

Advance care planning in progressive neurological diseases: lessons from ALS.

BACKGROUND: There is increasing awareness of the need for an integrated palliative care approach in chronic progressive neurological diseases. Advance care planning (ACP) is an integral part of this approach. As a systematically organized and ongoing communication process about patients' values, goals and preferences regarding medical care during serious and chronic illness, ACP aims to involve patients in decision-making before they become cognitively and communicatively incapable. However, it remains underutilized in daily neurological practice except for speciality clinics such as ALS cente

Successful Management of Milroy's Disease: A Rare Condition Complicated by Tuberculous Pericardial Effusion.

Milroy's disease is a rare hereditary primary lymphedema, typically presenting at birth or early infancy with chronic lower limb swelling due to mutations in the FLT4 gene encoding VEGFR-3. While congenital lymphedema is the hallmark, its association with tubercular pericardial effusion leading to tamponade is extremely rare. We report an 18-year-old female with longstanding bilateral lower limb lymphedema who presented with progressive dyspnea and fever. Evaluation revealed tubercular pericardial effusion, and genetic testing confirmed an FLT4 mutation. The patient improved with antitubercula
