Initial biopsy outcome prediction in Korean patients-comparison of a noble web-based Korean prostate cancer risk calculator versus prostate-specific antigen testing.

We developed and validated a novel Korean prostate cancer risk calculator (KPCRC) for predicting the probability of a positive initial prostate biopsy in a Korean population. Data were collected from 602 Koreans who underwent initial prostate biopsies due to an increased level of prostate-specific antigen (PSA), a palpable nodule upon digital rectal examination (DRE), or a hypoechoic lesion upon transrectal ultrasound (TRUS). The clinical and laboratory variables were analyzed by simple and multiple logistic regression analysis. The area under the receiver operating characteristic curve (AUC) …

Prostate-specific antigen, digital rectal examination and transrectal ultrasonography: a meta-analysis for this diagnostic triad of prostate cancer in symptomatic korean men.

We conducted a meta-analysis using results from the Korean literature to determine whether prostate-specific antigen (PSA) or digital rectal examination (DRE) or transrectal ultrasonography (TRUS) provides a better diagnostic outcome for possible prostate cancer patients. An extensive literature search of MedRIC database et al. (1980 to 2003) was performed using the medical subject headings "PSA", "DRE", "TRUS" and "prostate cancer". Of the 108 articles that we retrieved, 13 studies (2,029 subjects) were selected for this meta-analysis. The criteria for quality evaluation were as follows: the …

Prediction of post-radical prostatectomy pathological outcome for stage T1c prostate cancer with percent free prostate specific antigen: a prospective multicenter clinical trial.

PURPOSE: Prostate specific antigen (PSA) exists in bound (complexed) and unbound (free) forms in serum. The percentage of free PSA enhances the specificity of PSA testing for prostate cancer detection. We evaluated the use of percent free PSA preoperatively to predict pathological stage. MATERIALS AND METHODS: A total of 379 men with prostate cancer and 394 with benign prostatic disease 50 to 75 years old were enrolled in this prospective study at 7 medical centers. All subjects had a palpably benign prostate gland, serum PSA 4.0 to 10.0 ng./ml. and a histologically confirmed diagnosis. The H

Prostate-specific Membrane Antigen-radioguided Surgery in an EMBARK-like Cohort of Patients with Oligorecurrent Hormone-sensitive Prostate Cancer: Delay in Systemic Treatment.

We analyzed data for a cohort of 111 patients with EMBARK-like biochemical recurrence (BCR) of prostate cancer (prostate-specific antigen [PSA] doubling time ≤9 mo, PSA ≥1 ng/ml) after radical prostatectomy and localized oligorecurrence on prostate-specific membrane antigen (PSMA)-based imaging. All patients underwent PSMA-radioguided surgery (RGS). At PSMA-RGS, the median PSA was 1.95 ng/ml (interquartile range [IQR] 1.36-3.20) ng/ml and the median PSA doubling time was 4.0 mo (IQR 2.5-5.5). Clavien-Dindo grade >IIIa complications occurred in nine of 111 patients (8.1%). A complete biochemica…

Association of Deep and Durable Prostate-specific Antigen Responses with Outcomes in Metastatic Hormone-sensitive Prostate Cancer: Insights from ARASENS and ARANOTE Trials.

BACKGROUND AND OBJECTIVE: Prostate-specific antigen (PSA) is a critical biomarker in metastatic hormone-sensitive prostate cancer (mHSPC), offering insights into disease progression and treatment efficacy. An early PSA response to intensified therapy has significant prognostic implications; however, traditional methods often categorize PSA levels and focus on fixed time points, neglecting its dynamic nature. Joint models integrate longitudinal PSA trajectories and survival outcomes to provide a more comprehensive analysis. This study aims to evaluate the association between longitudinal PSA tr…

The Change of 68Ga-Prostate-Specific Membrane Antigen Uptake in Myeloma After Chemotherapy in a Patient With Multiple Myeloma and Concurrent Prostate Cancer.

A bone lesion in the right calcaneus was found in a 76-year-old man with a history of prostate cancer. Ga-PSMA-11 PET/CT showed multiple PSMA-expressing lytic bone lesions in appendicular skeleton. Biopsy of the lesion in the right calcaneus confirmed plasmacytoma. After several cycles of chemotherapy, the patient was in serologically stringent complete remission. The follow-up PET/CT showed a significant reduction of Ga-PSMA-11 uptake in the previously PSMA-avid bone lesions without morphological changes on CT. The reduction of PSMA expression in myelomas after chemotherapy is suggested to re

The frequency of thyroid incidental findings and risk of malignancy detected by 68Ga-labeled prostate-specific membrane antigen PET/CT in prostate cancer.

OBJECTIVE: Gallium-68-prostate-specific membrane antigen positron emission tomography/computed tomography (68Ga-PSMA PET/CT) has become a well-established imaging method for the evaluation of patients with prostate cancer. However, several cases have revealed PSMA uptake in a large variety of conditions other than prostate cancer. Prostate-specific membrane antigen uptake in thyroid cancer has also been reported. The aim of the present study was to systematically investigate the prevalence and clinical significance of thyroid incidental findings in patients undergoing 68Ga-PSMA PET/CT. MATERI

General practitioner perception of prostate-specific antigen testing has improved, but more awareness of prostate cancer risk in younger patients is still awaited.

BACKGROUND: In 2006, a county-wide survey of general practitioners (GPs) in the United Kingdom (UK) identified a reluctance to refer younger men with abnormal prostate specific antigen (PSA) levels. Younger men have the most to gain from early-detection of prostate cancer (PCa), which remains a national government priority in the UK and around the world. We sought to assess changes in perception of abnormal PSA-values amongst UK GPs over the past 10 years. MATERIALS AND METHODS: A total of 500 self-administered paper questionnaires were distributed to individually named GPs. One hundred and f
