BACKGROUND: The relationship between invasive medical devices and infective endocarditis (IE) has not been comprehensively assessed. We describe our experience of patients with IE, with particular attention to the role of pre-existing intravascular catheters and implanted cardiac devices in the pathogenesis.
METHODS: We performed a retrospective review of hospital records over a 10-y period (1997-2007), and included patients with 'definite' or 'possible' IE as per the modified Duke criteria. The complete electronic medical record was reviewed for the presence of intravascular devices prior to

Prosthetic valve endocarditis (PVE) is rare but devastating. A 69-year old man admitted for active endocarditis caused by Streptococcus pasteurianus. Antibiotic therapy was started, but the patient developed bowel obstruction owing to cancer with multiple liver metastases, and underwent transverse colectomy. Following colectomy, antibiotic agent was given continued for 4 weeks after and mitral valve replacement( MVR) using a bioprosthesis was performed. Oral antibiotic therapy was continued for six months after MVR to avoid infection recurrence. One year after MVR, the size of multiple liver m

PURPOSE: To determine the superior spacer design, a growing number of studies are comparing treatment results between patients having been treated with articulating and static knee spacers in the setting of two-stage revision for periprosthetic joint infection (PJI). In contrast, the primary objective of this study was to compare preoperative characteristics between patients from both spacer groups and examine whether significant differences were present prior to spacer implantation.
METHODS: This retrospective, single-centre, cohort study examined the preoperative situation of 80 consecutive

BACKGROUND: Periprosthetic joint infection (PJI) is a leading cause of revision arthroplasty. Considerable controversy still exists whether single- or two-stage exchange is the better approach for patients with chronic PJI. Historically, single-stage exchange arthroplasty was thought to have an unacceptably high risk of reinfection compared with two-stage exchange but recent studies have demonstrated that this may not be the case. To be considered for single-stage exchange, patients should meet certain criteria including a preoperatively identified nonvirulent pathogen in an immunocompetent ho

The use of artificial urinary sphincter (AUS) for the treatment of stress urinary incontinence has become more prevalent, especially in the "prostate-specific antigen (PSA)-era", when more patients are treated for localized prostate cancer. The first widely accepted device was the AMS 800, but since then, other devices have also entered the market. While efficacy has increased with improvements in technology and technique, and patient satisfaction is high, AUS implantation still has inherent risks and complications of any implant surgery, in addition to the unique challenges of urethral compli

Periprosthetic knee infection is a complication associated with prosthetic failure; incidence change from 0,4-2% of primary total knee replacement and 5,6% in revisions; incidence is increasing over the years. Two-stage revision is the technique used in chronic infection. Aim of the work is to check success rate in our data. Methods. We analyzed retrospectively data of patients who undergone two stage revision surgery between 01/01/2010 to 31/12/2015. We made a clinical and radiological control after 1, 3, 6, 12, 24 months and we evaluate the outcome in December 2016. Results. Between 2010 and

Anterior cruciate ligament reconstruction with the use of synthetic graft material has been used as an alternative to biologic grafts. The use of these grafts has largely been abandoned in reconstruction of the anterior cruciate ligament because of mechanical failure of the graft and production of wear debris leading to synovitis and recurrent effusions. This article presents a case of early, extensive periprosthetic osteolysis around a total knee arthroplasty associated with wear debris from retained fragments of a Gore-Tex (WL Gore and associates, Inc, Flagstaff, Ariz) (polytetrafluoroethyle

As the volume of heart valve replacements increases, more patients are undergoing repeated operations. There are many reasons for reoperations that include incomplete or imperfectly executed primary operation, infection on valve prostheses, valve related complications. Repeated operations have their own specific technical problems, and from them depends mortality and complications rate. At Vilnius University Heart Surgery Clinic from 1967 we performed 6200 heart valve replacement operations. At the same time we performed 428 reoperations for 394 patients. Two reoperations were performed for 28

INTRODUCTION: Cutibacterium acnes is a commensal skin bacterium that is involved in bone prosthesis infections (BPIs) and presents low-grade clinical symptoms. C. acnes has been thought to escape the immune system at bone sites.
MATERIAL AND METHODS: Our study was carried out on a laboratory strain and two BPI-related clinical strains, one of which surprisingly induced clinical symptoms of inflammation in the patient. We investigated the ability of these C. acnes strains to trigger in vitro human primary neutrophils (PMN) response through inflammatory mediators measurements (antibody arrays,

BACKGROUND: Aortic valve prosthetic infections can lead to the spread of infection, causing annular abscesses and annular destruction, which may require annular reconstruction and aortic root replacement. Reports on the infection and reoperation of sutureless valves are rare. Here, we present a case of infection involving a Perceval sutureless valve, which led to an annular abscess.
CASE PRESENTATION: We report the case of a 73-year-old woman who had previously undergone sutureless valve implantation for severe stenosis of a bicuspid aortic valve. Approximately 23 months after surgery, the pa

BACKGROUND: Carotid endarterectomy (CEA) is the most commonly used invasive procedure for treatment of carotid stenosis. Different methods are used to close the arteriotomy including primary closure and patch repair with a graft. Prosthetic patch infection is a rare but serious complication of patch closure, and we will present a unique case of carotid patch infection (CPI) 12 years after implantation.
CASE: Patient is 76-year-old male ex-smoker with history of bilateral CEA with Dacron patch closure 12 years prior to presentation. He had a left neck draining sinus one year prior to presentat

OBJECTIVE: To explore the diagnostic efficacy of electrochemical detection of synovial fluid leukocyte esterase (LE) based on glucosyl ester for periprosthetic joint infection (PJI) in rabbits.
METHODS: The enzyme kinetic parameters of the LE-catalyzed reaction based on glucosyl esters were determined electrochemically, and the charge-LE concentration relationship was plotted. Forty-eight healthy New Zealand rabbits were randomly divided into a sham-operation group (blank group), a knee joint metal prosthesis implantation group (control group), and a knee joint metal prosthesis implantation w

INTRODUCTION: Periprosthetic joint infections (PJI) represent a major complication of total joint arthroplasty. While most infections are caused by staphylococci species, a notable proportion involves gram-negative bacteria. Due to the smaller numbers, outcome reports in literature are scarce and heterogenous success rates have been reported. This study aimed to (1) evaluate the overall treatment success of gram-negative PJI and (2) identify the most suitable surgical treatment strategy in eradicating gram-negative PJI.
MATERIALS AND METHODS: Seventy-two cases of gram-negative PJI treated bet

BACKGROUND: Prosthetic valve endocarditis (PVE) after transcatheter aortic valve replacement (TAVR) is an uncommon but life-threatening complication associated with high embolic risk.
CASE PRESENTATION: A 82-year-old woman with prior TAVR presented with fever and malaise and was diagnosed with PVE due to Streptococcus Mutans. Transesophageal echocardiography revealed a large, highly mobile vegetation attached to the transcatheter valve, floating in the systolic jet of blood flow. Computed tomography demonstrated progressive splenic infarction and embolic involvement of the kidneys and mesente

Overwhelming post-splenectomy infection (OPSI) is a severe and potentially life-threatening condition that can occur in patients undergoing splenectomy. We report a case of a patient who had a splenectomy approximately 30 years ago during prosthetic valve insertion for infective endocarditis (IE). The patient later developed prosthetic valve endocarditis caused by Streptococcus pneumoniae associated with OPSI. Although IE due to Streptococcus pneumoniae is rare, the presence of a prosthetic valve may increase the risk of IE in splenectomized patients. This case highlights the need for increase

BACKGROUND: Staphylococcus capitis (S. capitis) is a subtype of coagulase-negative staphylococci and a commensal of the skin of the human scalp and forehead. S. capitis has been occasionally reported in infective endocarditis and rarely in prosthetic valve endocarditis (PVE). The purpose of this report is to present the clinical course and the surgical management of a series of four patients with S. capitis PVE.
METHODS: The medical records of 190 adult patients with a definite diagnosis of infective endocarditis by the Duke modified criteria and who underwent surgery at our center between Ja
