Novel effects other than antisecretory action and off-label use of proton pump inhibitors.

The proton pump inhibitors (PPIs) are a relatively new class of agents used for the treatment of acid-related disorders, including peptic ulcer diseases, reflux oesophagitis and Zollinger-Ellison syndrome, and in enhancing antibiotic therapy in the eradication of Helicobacter pylori in patients with peptic ulcer disease. The PPIs are the most potent gastric acid-suppressing agents currently in clinical use. According to the recent basic study, PPIs may act not only as potent acid suppressants, but also as anti-inflammatory or pro-regenerative agents. On the other hand, in the clinical field, g

Air swallowing can be responsible for non-response of heartburn to high-dose proton pump inhibitor.

Intraluminal electrical impedance is a novel technique, which is able for the first time to provide a qualitative assessment of refluxed material moving from the stomach to the oesophagus. In other words, the presence of air can be differentiated from that of liquid, because the former is characterised by high and the latter by low impedance compared with baseline. Moreover, the combined measurement of electrical impedance and pH-metry permits to distinguish acid from non-acid liquid reflux. One of the most important clinical applications of this method is to assess the reasons for poor respon

Oesophageal acid-peptic strictures in the histamine H2 receptor antagonist and proton pump inhibitor era.

BACKGROUND AND AIM: We present a survey on the incidence, demography and natural history (re-dilatation rates) of patients with oesophageal acid-peptic stricture seen between 1977 and 1995. PATIENTS AND METHODS: Prospective, cohort observational study. Stricture severity was graded mild, moderate or severe (needing forcible dilatation with bougies). Most were treated with histamine H2 receptor antagonists or proton pump inhibitors, and were followed up by serial check endoscopy. RESULTS: A total of 156 of 7429 (2%) reflux patients had stricture: mild, n=56; moderate, n=25; severe, n=75. The

Eradication of Helicobacter pylori infection with proton pump inhibitor-based triple therapy. A randomised study.

BACKGROUND: Helicobacter pylori (H. pylori) infection plays an important role in the pathogenesis of duodenal ulcer (DU) disease. Low DU recurrences and reinfection rates were universally described, when treatment was effective. It has been suggested that short-term triple therapy, comprising a proton pump inhibitor plus 2 antibiotics (clarithromycin, amoxycillin or a nitroimidazole), should be used as first choice in treating H. pylori infection. Nevertheless, conflicting results have been reported on using these treatment regimens in different countries. Our aim was to compare cure rates of

[Microflora of gastric juice in patients after eradication of Helicobacter pylori and treatment with a proton pump inhibitor].

UNLABELLED: Previous studies have suggested that decrease of acidity of gastric juice leads to microbial overgrowth in gastric juice. AIM OF THE STUDY: Qualitative and quantitative evaluation of upper respiratory tract and gastric juice microflora depending on the change of gastric juice pH, which occurred during treatment with pantoprazole and eradication therapy. MATERIAL AND METHODS: 40 patients suffering from duodenal ulcer or reflux esophagitis have been examined. Group 1, patients without infection of H. pylori have received pantoprazole 40 mg/d for 4 weeks. Group 2, patients with infe

Effects of very long (up to 10 years) proton pump blockade on human gastric mucosa.

BACKGROUND: Long-term use of proton pump inhibitors (PPI) has been reported to worsen oxyntic mucosa gastritis and the resulting gland atrophy has been considered a potential risk factor for neoplastic changes in the gastric mucosa. AIMS: The present study examines the effect of extended continuous PPI treatment for up to 10 years on the exocrine and endocrine stomach of patients with acid-related diseases of the upper GI tract. METHODS: Biopsies from the antral and oxyntic mucosa taken at regular time intervals were examined for gastritis, atrophy, intestinal metaplasia, Helicobacter pylori

Review article: potential gastrointestinal effects of long-term acid suppression with proton pump inhibitors.

This review examines the evidence for the development of adverse effects due to prolonged gastric acid suppression with proton pump inhibitors. Potential areas of concern regarding long-term proton pump inhibitor use have included: carcinoid formation; development of gastric adenocarcinoma (especially in patients with Helicobacter pylori infection); bacterial overgrowth; enteric infections; and malabsorption of fat, minerals, and vitamins. Prolonged proton pump inhibitor use may lead to enterochromaffin-like cell hyperplasia, but has not been demonstrated to increase the risk of carcinoid form

Prevention of the gastrointestinal adverse effects of nonsteroidal anti-inflammatory drugs: the role of proton pump inhibitors.

The associations between nonsteroidal anti-inflammatory drugs (NSAIDs) and the presence and complications of gastroduodenal erosions and ulcers are well established. Evidence that acid aggravates NSAID-induced injury provides a rationale for minimising such damage by acid suppression. Other strategies discussed include avoidance of NSAIDs or minimising their dosage, selecting NSAIDs known to cause less damage, and co-prescription of various agents. Cytoprotection with misoprostol, a prostaglandin analogue, has been shown to be effective in reducing NSAID-related peptic ulcers and their complic

Peptic ulcer perforation before and after the introduction of H2-receptor blockers and proton pump inhibitors.

BACKGROUND: The aim of this retrospective study was to compare patients treated for perforated peptic ulcer before and after the introduction of the H2-receptor antagonists and proton pump inhibitors (PPI) with regard to their medical history, clinical features, methods of diagnosis and treatment, complications, and mortality. METHODS AND RESULTS: During the study period 1974 to 1992 we found a significant reduction in the incidence of peptic ulcer perforation (P < 0.001). Patients admitted during the later period of the study were older and more seriously ill. The incidence of perforation am

Pathogenesis and treatment of acid peptic disorders: comparison of proton pump inhibitors with other antiulcer agents.

Acid peptic disorders, including gastric ulcers, duodenal ulcers, and gastroesophageal reflux disease, are commonly occurring conditions with high direct and indirect costs. The pathogenesis of these disorders involves an imbalance between acid secretion and gastric mucosal defenses. Pharmacologic treatment of acid peptic disorders has focused on correcting this imbalance by either improving mucosal defenses with drugs such as sucralfate, bismuth, and prostaglandin analogs, neutralizing acid with antacids, or decreasing acid secretion with histamine2 (H2)-receptor antagonists, or, more recentl

Influence of H2-receptor- and proton pump inhibitors on some functions of the oxydative and conjugative drug metabolism.

There are numerous investigations describing the influence of histamine H2-receptor antagonists and proton pump inhibitors on cytochrome P450-mediated hepatic oxydative and conjugative drug metabolizing enzymes. The aim of this study was to investigate the influence of the H2-receptor blockers cimetidine, ranitidine, famotidine, nizatidine and of the proton pump inhibitors omeprazole and lansoprazole on the acetylation capacity and on different microsomal monooxygenases of the rat liver. The experiments were performed in two randomized studies with male Wistar rats after a 7-day pretreatment o

Dose-range finding study with the proton pump inhibitor pantoprazole in acute duodenal ulcer patients.

Pantoprazole is a newly developed benzimidazole derivative with strong inhibitory actions on gastric acid secretion by blocking H(+)-K(+)-ATPase. This randomized double-blind multicenter trial investigated the efficacy of 20 mg, 40 mg and 80 mg pantoprazole o.m. on ulcer healing and symptomatic relief in 219 out-patients with endoscopically assessed acute duodenal ulcer. After 2 weeks complete ulcer healing was achieved in 58%, 89% and 82% of the patients with 20 mg, 40 mg and 80 mg pantoprazole o.m., respectively. After 4 weeks, corresponding figures were 93%, 99% and 100%; the difference of

From Prout to the proton pump--a history of the science of gastric acid secretion and the surgery of peptic ulcer.

Prior to the 17th century, there was considerable confusion regarding the process of digestion. Although some physicians were certain that it was initiated by acid in the stomach, both the source and the nature of the acid were unclear. In the early 19th century, Prout confirmed the active secretion of hydrochloric acid by the stomach and related it to the symptoms of dyspepsia. Jacob Helm and, subsequently, Beaumont studied digestion in humans with gastric fistulas and each commented extensively on the physiologic manifestation of digestion. The role of the vagus nerves in the control of gast

Inconsistency in the Association Between Proton Pump Inhibitor Use and Dementia Risk: An Updated Meta-Analysis.

BACKGROUND: Proton pump inhibitors (PPIs) are commonly used to manage acid-related gastrointestinal conditions. Nevertheless, growing attention has been paid to their long-term safety, especially their possible link to dementia and Alzheimer's disease (AD). Prior research has yielded inconsistent findings, underscoring the need for a comprehensive and current evaluation. METHODS: A systematic search was conducted across PubMed, Embase (Ovid), and the Cochrane Library to identify relevant publications up to May 28, 2025, without language restrictions. Two investigators independently extracted

Proton pump inhibitors and the risk of hospital acquired clostridioides difficile infection in critically ill patients.

BACKGROUND: Proton pump inhibitors (PPIs) are commonly prescribed medications for critically ill patients. However, the existence of a clear association between PPI prescriptions and Clostridioides difficile infection (CDI) remains controversial. This study aims to investigate the relationship between PPI use and CDI, thereby providing additional evidence for the clinical application of PPIs. METHODS: This study extracted data from the eICU and MIMIC IV databases, ultimately enrolling 26,090 patients. Propensity score matching was employed to balance covariates across groups. Clinical outcome

Proton-pump inhibitor exposure and prevalence of colorectal adenoma: a retrospective analysis from the Sakkopi study.

OBJECTIVE: Proton-pump inhibitors (PPIs) are among the most widely prescribed long-term medications worldwide, yet concerns remain about their potential role in colorectal carcinogenesis. Experimental data suggest that PPI-induced hypergastrinaemia may promote colonic epithelial proliferation, but epidemiological findings are inconsistent, and colonoscopy-based evidence is limited. METHODS: We studied 4476 asymptomatic adults who underwent structured screening colonoscopy in Austria between 2007 and 2020 as part of the Sakkopi study. Regular PPI use, including dose stratification, was evaluat

Proton pump inhibitors use and the risk of osteoporosis and fractures: A two-sample Mendelian randomization study.

The causal relationship between proton pump inhibitor (PPI) use and bone health outcomes remains uncertain. This study employs a Mendelian randomization (MR) approach to investigate the potential causal association between PPI use and the risk of osteoporosis and fractures. We selected 4 representative PPIs, including omeprazole, esomeprazole, lansoprazole, and rabeprazole, for our study. Bone health outcomes were evaluated through femoral neck bone mineral density (BMD), lumbar spine BMD, and the prevalence of osteoporosis and fracture across various anatomical sites, including the upper arm

Proton Pump Inhibitors Prescribing Behaviors and Rationalization Strategies Among Healthcare Providers in Southeast Asia.

Despite widespread use, real-world evidence regarding proton pump inhibitor (PPI) prescribing and rationalization practices in Southeast Asia (SEA) remains limited; therefore, we aimed to characterize current prescribing behaviors and rationalization strategies among healthcare professionals in the region. The SEA PPI Rationalisation Working Group developed and disseminated an online questionnaire via professional networks across SEA to collect data on respondent demographics, PPI prescribing practices, and rationalization approaches; instrument reliability testing produced a Cronbach's α of 0
