Three-dimensional time-of-flight MR angiography with a specialized gradient head coil.

A gradient head coil has been developed, incorporating two independent gradients within the conventional body coil of the magnetic resonance (MR) system, with reduced rise times (200 microseconds) and maximum amplitudes of 37 and 18 mT/m in the z and y directions, respectively. This gradient coil was systematically evaluated by testing two-dimensional (2D) and three-dimensional (3D) time-of-flight (TOF) MR angiography sequences applied to a pulsatile flow phantom simulating a carotid stenosis and the intracranial vasculature. When standard 2D and 3D TOF MR angiography techniques were used to i…

[Ciliary circulation in glaucoma complicated by diabetes mellitus].

The speed of blood perfusion and the pulsation curve in ciliary arteries was checked by the method of Doppler's pulsating focused ultrasonography in 30 patients aged 46-78 years treated for glaucoma and diabetes. The data obtained were compared with analogical data in nondiabetic patients with glaucoma. It was established that the increase of the intraocular pressure caused a larger and earlier impairment of the regulating mechanism of the ciliary microcirculation in diabetic patients with glaucoma in comparison with the group of glaucomatous patients without diabetes. Particularly high values

Importance of source images of time-of-flight magnetic resonance angiography in the diagnosis of low-flow dural arteriovenous fistulae after traumatic brain injury.

Three-dimensional time-of-flight (TOF) magnetic resonance angiography (MRA) can reliably detect dural arteriovenous fistula (dAVF); however, TOF source images should be checked in cases with low-flow dAVFs. A 69-year-old woman reported intractable pulsatile tinnitus after head trauma. It was difficult to diagnose dAVF using conventional MRA, but it was confirmed using a TOF source image. Cerebral angiography revealed a dAVF with a small shunted pouch draining into the sigmoid sinus, accompanying the arterial jet flow. Transarterial embolisation of the shunted pouch completely obliterated the d

Carotid energy pulsatility index provides complementary hemodynamic information to renal function in a community-based cohort.

Conventional hemodynamic, including Carotid-femoral pulse wave velocity (cfPWV), characteristic impedance (Zc), and brachial pulse pressure, do not directly quantify pulsatile energy transmission through the arterial system. Carotid energy pulsatility index (PI), derived from synchronized carotid pressure-flow waveforms, may capture this complementary aspect of arterial hemodynamics. We examined whether carotid energy PI was independently associated with renal function after accounting for conventional vascular hemodynamic measures. Participants from the community-based Cardiovascular Disease

Cirsoid Aneurysm: Report of Two Cases With Cosmetic Surgical Treatment and Narrative Review of the Literature.

Cirsoid aneurysm (CA) is a rare high-flow arteriovenous malformation (AVM) of the scalp that may cause cosmetic deformity, neurologic symptoms, skin ulceration, or hemorrhage. We report two men with left frontoparietal lesions supplied by the frontal branch of the superficial temporal artery. A 37-year-old man presented with a progressively enlarging pulsatile mass of 3 years' duration. Duplex ultrasonography and computed tomography angiography (CTA) demonstrated a high-flow scalp AVM with extensive venous drainage and no intracranial communication. A 27-year-old man presented with a 1-year hi

LES of non-Newtonian physiological blood flow in a model of arterial stenosis.

Large Eddy Simulation (LES) is performed to study the physiological pulsatile transition-to-turbulent non-Newtonian blood flow through a 3D model of arterial stenosis by using five different blood viscosity models: (i) Power-law, (ii) Carreau, (iii) Quemada, (iv) Cross and (v) modified-Casson. The computational domain has been chosen is a simple channel with a biological type stenosis formed eccentrically on the top wall. The physiological pulsation is generated at the inlet of the model using the first four harmonic series of the physiological pressure pulse (Loudon and Tordesillas [1]). The

In vitro fluid mechanical effects of thoracic artificial lung compliance.

This in vitro study sought to determine what compliance minimizes thoracic artificial lung impedance and pump power output. A pulsatile pump drove 3.0 cP glycerol through a circuit consisting of an MC3 Biolung preceded by a piston-cylinder (PC, n = 5) chamber with a variable compliance or a polyurethane (n = 4) chamber with a fixed, yet pressure-dependent, compliance. Each chamber was tested at flow rates of 1.8, 3.0, and 5.0 l/min and heart rates of 60, 75, and 100 bpm. Compliances, C, from 0-20 ml/mm Hg were tested in the PC chamber. Instantaneous pump outlet flow and pressure were acquired

Arteriovenous fistula after temporomandibular joint arthroscopy treated with precipitating hydrophobic injectable liquid: case report and literature review.

INTRODUCTION: Arteriovenous fistulas (AVFs) are abnormal connections between the arterial and venous systems, bypassing the capillary network and causing high-flow shunting. While AVFs can be congenital, spontaneous, or traumatic, trauma remains the most common cause. Although iatrogenic AVFs after temporomandibular joint (TMJ) procedures are rare, their recognition is crucial due to potential complications such as hemorrhage, thromboembolism, and high-output cardiac failure. This report presents a rare case of an AVF following TMJ arthroscopy and discusses its management using superselective

Bidirectional Glenn shunt in association with congenital heart repairs: the 1(1/2) ventricular repair.

BACKGROUND: The bidirectional Glenn shunt has been used to incorporate a smaller tripartite ventricle into the circulation and create pulsatile pulmonary artery flow. We reviewed our operative experience and assessed hemodynamics of the bidirectional Glenn shunt in 1(1/2) ventricular repair or in conjunction with other repairs of congenital heart defects. METHODS: Between 1992 and 1998, 15 patients (mean age, 8.1+/-7.9 years) had bidirectional Glenn shunt in association with repair of congenital heart defects. Eighty-seven percent had at least one previous operation. All patients had simultan

Atypical Feeding and Drainage of Dural AV Fistula.

Dural arteriovenous fistulas (DAVF) are frequent causes of pulsatile tinnitus. Color Doppler sonography may play a useful, complementary role to CTA/MRA and digital subtraction angiography (DSA) in the assessment of these anomalies' characteristics, such as an ipsilateral increased flow volume and a low resistive index. In this article, we report a case of DAVF first detected with Color Doppler sonography that displayed an uncharacteristic venous drainage pattern.

Arteriovenous malformation of the uterus associated with secondary postpartum hemorrhage.

We present the case of a young woman with persistent secondary postpartum hemorrhage. Transvaginal imaging demonstrated an irregular pulsatile lesion in the anterior myometrium. Color Doppler analysis revealed the presence of abnormal vessels consistent with an arteriovenous malformation. Typically this vascular abnormality had a turbulent pattern of arterial and venous flow with high peak velocities and low resistance. The patient was treated with selective arterial embolization leading to a full recovery.

New hemodynamic variables as predictors of arteriovenous fistula maturation.

INTRODUCTION: Failure to mature of native arteriovenous fistula (AVF) assessed by image is not well documented. METHODS: The aim of this study is to evaluate AVF maturation by Doppler ultrasound within 6 weeks after surgery and check for new hemodynamic variables as prognostic factors. RESULTS: A total of 155 native AVFs were created in 3 years. Median age of patients was 71 years (60;79), with male gender predominance of 59.4% (n = 92). Diabetes was present in 60.6% (n = 94). AVF was created in distal forearm in 49.7% (n = 77). Un-assisted and overall maturation was of 67.1% (n = 104) and 8

Channel impeller design for centrifugal blood pump in hybrid pediatric total artificial heart: Modeling, magnet integration, and hydraulic experiments.

BACKGROUND: The purpose of this research is to address ongoing device shortfalls for pediatric patients by developing a novel pediatric hybrid total artificial heart (TAH). The valveless magnetically-levitated MCS device (Dragon Heart) has only two moving parts, integrates an axial and centrifugal blood pump into a single device, and will occupy a compact footprint within the chest for the pediatric patient population. METHODS: Prior work on the Dragon Heart focused on the development of pump designs to achieve hemodynamic requirements. The impeller of these pumps was shaft-driven and thus co

Transcranial Doppler evaluation of cerebral blood flow in patients with paroxysmal atrial fibrillation.

Cerebral blood flow was measured by transcranial Doppler sonography (TCD) in 15 patients during and after an attack of paroxysmal atrial fibrillation. During the attack the mean flow velocity (MFV) was reduced significantly in the middle cerebral artery (p < 0.05) and nonsignificantly in the other arteries. The pulsatility index (PI) was reduced in all the arteries but not significantly. During the fibrillation phase the MFV and PI tended to decrease as the heart rate increased. None of the patients showed clinical signs of cerebral ischemia during the attacks. The absence of such signs despit

Impaired visual evoked flow velocity response in cerebral amyloid angiopathy.

OBJECTIVE: Animal models of cerebral amyloid angiopathy (CAA) exhibit abnormal vascular reactivity. We determined whether vascular reactivity, measured by transcranial Doppler ultrasound (TCD), is reduced in humans with CAA. METHODS: Cases were recruited from an established prospective study of CAA. Healthy controls were recruited from a study of normal aging. Evoked mean flow velocity increase in the posterior cerebral artery (PCA) was measured while subjects viewed a flashing alternating checkerboard stimulus. In a separate but partially overlapping cohort we measured the mean flow velocity

Color Doppler imaging of orbital arteries for detection of carotid occlusive disease.

BACKGROUND AND PURPOSE: Distal to a hemodynamically significant stenosis, the Doppler effect becomes dampened. Thus, measuring the flow profile in the ophthalmic artery and the central retinal artery with color Doppler imaging may provide hemodynamic information about the carotid circulation. METHODS: To validate the flow profile measurement with color Doppler imaging in the ophthalmic and central retinal arteries and to determine the sensitivity and specificity of this examination in the detection of hemodynamically significant carotid stenosis, we compared color Doppler imaging examinations

Successful Liver Transplantation From a Donor With a Continuous-Flow Left Ventricular Assist Device for 9 Months.

Organ donor criteria continue to be extended in an attempt to meet growing demands. Patients with continuous-flow left ventricular assist devices are one group of potential donors being considered. One concern with this group is the effect of continuous flow for a prolonged duration, as opposed to normal pulsatile flow, on end-organ function. We report the 1st case of a liver transplantation from a donor who had a continuous-flow left ventricular assist device for 9 months. The recipient was a 69-year-old woman with a history of cryptogenic cirrhosis and hepatocellular carcinoma. The transplan…
