The origin of non-specific effects of vaccines and the role of randomised clinical trials.

Recent articles claim that randomized clinical trials (RCTs) have not documented non-specific effects of vaccines (NSE), i.e. broader immunomodulatory effects of vaccines. We reviewed the role of RCTs in the discovery of NSEs. The articles claiming no NSEs misread the literature, disregarded non-RCT data, and did not take interactions with other interventions into consideration. RCTs have played a limited role in the discovery of the NSEs of vaccines. Instead, contradictions in non-RCT data have helped to document NSEs; most important have been abrupt mortality changes, sex-differential effect

A scoping review of ICHOM sets identifies a small number of commonly recommended baseline characteristics.

OBJECTIVES: The capture of patient baseline characteristics in randomized controlled trials (RCTs) is essential for assessing external validity and for exploring treatment effect heterogeneity. Yet, the capture of such information is highly inconsistent across trials with limited harmonization efforts to date. The International Consortium for Health Outcomes Measurement (ICHOM) has issued expert recommendations for baseline characteristics to capture in clinical practice, as part of Sets of Patient-Centered Outcome Measures ("Sets"). We reviewed which characteristics were recommended across di

Components of cognitive-behavioral interventions to prevent the onset of depression among the high-risk population: a systematic review and component network meta-analysis.

OBJECTIVES: Cognitive Behavioral Therapy (CBT) is a promising strategy for preventing onset of depression. This systematic review and component network meta-analysis (cNMA) investigated efficacious and harmful components including various skills and delivery formats of CBT for preventing depression onset among at-risk populations. ELIGIBILITY CRITERIA: We selected randomized trials of preventive CBT interventions from a previous systematic review (Cuijpers et al., 2021), and conducted an additional search on PubMed and PsycINFO up to January 2022. METHODS OF SYNTHESIS: The primary outcome wa

Clinical Surveillance Technologies in Nonintensive Care Unit Hospital Settings: Systematic Review and Bayesian Network Meta-Analysis of Randomized Trials.

BACKGROUND: Failure to recognize clinical deterioration in hospitalized patients has prompted the development of rule-based electronic surveillance (RB-ES), predictive model-based electronic surveillance (PM-ES), and continuous physiologic monitoring (CPM). However, their comparative effects on patient-centered outcomes remain uncertain. OBJECTIVE: The aim of this study is to compare the clinical effects of emergent intensive RB-ES, PM-ES, CPM, and local standard care in nonintensive care unit hospital settings. METHODS: We searched PubMed, Embase via online, the Cochrane Central Register of

Percussion, diuresis, and inversion therapy can facilitate lower renal pole stone passage after extracorporeal shock wave lithotripsy and retrograde intrarenal surgery? A systematic review and meta-analysis of randomized controlled trials.

PURPOSE: Patients with lower pole stones (LPS) present a significant challenge for urologists because of the increased likelihood of residual fragments and recurrence following extracorporeal shockwave lithotripsy (ESWL) and retrograde intrarenal surgery (RIRS). Although percussion, diuresis, and inversion (PDI) therapy has demonstrated potential for enhancing the stone-free rate (SFR) in these patients, its application remains a topic of debate. Consequently, our objective was to conduct a systematic review and meta-analysis of randomized controlled trials (RCTs) to evaluate SFR outcomes when

Methotrexate for Osteoarthritis: What Does the Evidence Say?

Osteoarthritis (OA) is a leading cause of disability worldwide, characterised by chronic pain and reduced quality of life. Despite its prevalence, pharmacotherapy options remain limited. Inflammation has emerged as a promising target, with anti-inflammatory agents used in other rheumatological conditions, such as methotrexate (MTX), being explored for OA treatment. MTX is a cornerstone therapy in rheumatoid arthritis (RA), owing to its broad immunomodulatory properties and well-established clinical efficacy. This review summarises evidence from seven randomised controlled trials and two observ

Fluid silicone gels in the prevention and treatment of hypertrophic scars: a systematic review and meta-analysis of randomised controlled trials.

Hypertrophic scars are a benign yet potentially debilitating cutaneous manifestation, characterised by elevated, pigmented scarring confined within the wound boundaries. They can affect the patients both physically, through pruritus, pain, stiffness and contractures, and also psychologically through stigmatization, anxiety, depression, diminished self-esteem and daily activity disruption, reducing the quality of life. There are currently various treatments used, and although silicone gel fluid is widely recognised as a leading non-invasive treatment, there is inconsistent evidence of superiori

Menthol for the Relief of Dyspnea: A Systematic Review of Randomized Controlled Trials.

BACKGROUND: Dyspnea is a symptom that can cause significant burden on activities of daily living. Overall, dyspnea has associations with increasing age, high body mass index (BMI), female sex and cardiorespiratory diseases. Aside from pharmacological treatments for underlying cardiorespiratory conditions that cause dyspnea, menthol has been discussed as a potential non-pharmacological intervention. However, its role remains contentious with uncertainty regarding efficacy and potential harm. Thus, this systematic review aims to explore the impact of menthol on dyspnea. METHODS: A comprehensive

A systematic review of reviews on ventriculostomy related infection definitions: A fundamental problem.

Despite being a common, well-recognized and important complication to External Ventricular Drainage (EVD), a consensus definition for Ventriculostomy Related Infections (VRI) has not yet been established. We conducted a review to qualitatively assess definition heterogeneity and objectivity among Randomized Controlled Trials (RCTs); and investigated systematic reviews, meta-analyses, and reviews of the literature for definition citation accuracy and common methodological approaches and points of discussion related to VRI definitions. RCTs were grouped into arbitrarily chosen infection rate bra
