A 15-year-old girl complained of swelling and shooting pains in the right upper extremity, which had bothered her for seven months. Physical examination revealed swelling, cyanosis, weakness and hyperesthesia over the entire right upper extremity. Serological and biochemical data were within normal limits. She was diagnosed as having reflex neurovascular dystrophy (RND). Psychological problems with school and her family might have contributed to the pathogenesis of the disease. With autogenic training (AT), remission was obtained within eighteen months.

We present the unique case of a patient with lumbosacral radiculoplexus neuropathy (LRPN) and a small intramedullary lesion, probably an infarction, in the spinal cord. A previously healthy 58-year-old female developed acute severe pain involving the entire right lower limb, followed by weakness of the right lower limb a few hours later. The patient could not walk and was referred to our clinic. The patient reported no sphincter symptoms. Examination of the cranial nerves and upper limbs revealed no abnormalities. The strength in her right lower limb was reduced to Medical Research Council gra

Acute infectious purpura fulminans is reported in a 16-month-old male with a history of posttraumatic asplenia and complete left brachial plexus palsy. This patient developed peripheral necrosis of both lower extremities and the right upper extremity, whereas the left upper extremity was completely spared from ischemia and tissue damage. Amputation of four digits on the right hand and debridement of both lower extremities were required. This patient demonstrated the protective effect of a traumatic sympathectomy, which suggests the requirement of an intact sympathetic reflex in the development

BACKGROUND: A nerve root anomaly, typified by a conjoined nerve root, is a rare finding. Conjoined nerve root anomalies are easily missed even in preoperative advanced imaging modalities, which can be potentially troublesome during and after surgery. In this report, we present a case of conjoined right L5-S1 nerve root in a patient with lumbar disc herniation, accompanied by spina bifida occulta, which was undiagnosed on preoperative imaging studies.
CASE REPORT: A 55-year-old Asian (Japanese) woman presented with low back pain and right leg radiating pain due to lumbar disc herniation at the

BACKGROUND: Trigeminocardiac reflex (TCR) is a vagal reflex produced by trigeminal nerve stimulation that causes bradycardia, hypotension, and, occasionally, cardiac arrest. TCR usually occurs during surgical manipulation of the posterior fossa. In this report, the authors describe an extremely rare case of TCR that developed when a burr hole was drilled on the temporal bone during surgery for an intraventricular meningioma.
OBSERVATIONS: The patient was referred to the authors' department after brain imaging revealed a mass lesion in the right trigone of the lateral ventricle. The surgery, w

Spasticity is common following stroke; however, high subject variability and unreliable measurement techniques limit research and treatment advances. Our objective was to investigate the use of shear wave elastography (SWE) to characterize the spastic reflex in the biceps brachii during passive elbow extension in an individual with spasticity. The patient was a 42-year-old right-hand-dominant male with history of right middle cerebral artery-distribution ischemic infarction causing spastic left hemiparesis. We compared Fugl-Meyer scores (numerical evaluation of motor function, sensation, motio

OBJECTIVE: To study the effect of electrical stimulation on accelerating facial nerve functional recovery from a crush injury in the rat model.
STUDY DESIGN: Experimental.
METHOD: The main trunk of the right facial nerve was crushed just distal to the stylomastoid foramen, causing right-sided facial paralysis in 17 Sprague-Dawley rats. An electrode apparatus was implanted in all rats. Nine rats underwent electrical stimulation and eight were sham stimulated until complete facial nerve recovery. Facial nerve function was assessed daily by grading eyeblink reflex, vibrissae orientation, and vi

A 21 year old female patient developed Südeck's atrophy of the right foot secondary to a chronic Achilles tendinitis. The condition was complicated by the occurrence of painful muscle spasms in the right leg and incontinence of urine. The spasms had characteristics of both a tonic ambulatory foot response and a spinal flexor reflex. The movements disappeared during sleep. Regional anaesthesia of the right leg made the spasms disappear both in and outside the region of anaesthesia. Backaveraging of the EEG showed the involuntary spasms to be preceded by a cortical potential similar to a readine

A 63-year-old man presented with an 11-month history of progressive myoclonus in the right abdominal wall. Administration of clonazepam reduced the frequency and amplitude. When the therapy was discontinued, the frequency and amplitude of the myoclonus increased, and synchronous and weak myoclonus also was observed in the left abdomen. The trunk was twisted just after the appearance of the abdominal myoclonus associated with myoclonic jerks spreading from the rostral to caudal paraspinal muscles. Later in the clinical course, the myoclonus became stimulus sensitive and was induced by tendon ta

Persistent hiccups manifesting as the sole symptom of aspiration pneumonia is a rare occurrence. Approximately 10 cases have been reported in the last 15 years. Hiccups are defined as persistent if it occurs beyond 48 hours and intractable if it occurs continuously for one month. We highlight a case of an elderly man diagnosed with a subacute ischemic infarct of the right occipital lobe with a preserved gag reflex and swallow reflex. The patient's persistent hiccups began eight hours after an emetic episode. Typical signs of pneumonia were absent. Chest x-ray revealed bilateral lower lobe pulm

A 31-year-old woman had exertional right-sided hemifacial flushing and sweating. Examination demonstrated slightly dilated pupils with absent constriction to light and a tonic near response and redilatation, features consistent with Adie syndrome. Neurological examination was otherwise normal, including preservation of deep tendon reflexes. Magnetic resonance imaging of brain and spine were normal. The combination of unilateral loss of sudomotor and vasomotor activity without loss of ocular sympathetic innervation fulfills the diagnosis of Harlequin syndrome. The combination of Harlequin and A

INTRODUCTION: Lack of habituation is the most reproducible interictal neurophysiological phenomenon in episodic migraine. The aim of this study is to evaluate the habituation phenomenon by examining the nociceptive blink reflex in patients with episodic migraine.
SUBJECTS AND METHODS: We conducted a cross-sectional observational case-control study. Patients with episodic migraine were selected consecutively and studied during the interictal phase. The nociceptive blink reflex was examined and a study was conducted comparing symmetry, analysis of the Euclidean distances and assessment of laten

BACKGROUND: The oculocardiac reflex presents when traction is applied to ocular muscles or on compression of the eyeball in the orbit. It is a vasovagal reaction that may be accompanied by bradycardia, nausea and vomiting.
CASE DESCRIPTION: A 6-year-old boy presented with vomiting and acute pain in his right eye, but no history of trauma. On physical examination we found no abnormalities other than bradycardia. Ocular examination showed an elevation restriction and slight depression restriction as well as ptosis of the affected eye. A cerebral MRI scan showed a blow-out fracture of the orbita

OBJECTIVE: To determine if active listening modulates the strength of the medial olivocochlear (MOC) reflex in children.
DESIGN: Click-evoked otoacoustic emissions (CEOAEs) were recorded from the right ear in quiet and in four test conditions: one with contralateral broadband noise (BBN) only, and three with active listening tasks wherein attention was directed to speech embedded in contralateral BBN.
STUDY SAMPLE: Fifteen typically-developing children (ranging in age from 8 to14 years) with normal hearing.
RESULTS: CEOAE levels were reduced in every condition with contralateral acoustic st

We reported a case of Fisher's syndrome with upbeat nystagmus. The syndrome manifested itself with the appearance of diplopia and ataxic gait preceded by cold-like symptoms. Although slight blephaloptosis, marked bilateral upward gaze palsy, and restricted abduction of the right eye were observed, light reflexes and Bell's phenomenon were normal. A slight decrease in the muscular power of the distal extremities, bilateral fingertip dysesthesia, and diminished sensation of vibration were observed. All muscle stretch reflexes were absent, and no pathologic reflexes were noted. Truncal ataxia and

A 12-year-old male case of reflex epilepsy with visual field defects attack induced by a family computer game was reported. This patient showed binasal visual field defects attack when he was playing a family computer game. Neurological findings and ophthalmological examinations were normal. EEG showed spike and slow wave complex distributed on right occipital region. These paroxysmal waves were activated by checkerboard pattern reversal stimuli. Stimulus intensity was closely related with these activations, but not stimulus ratio was. Examinations of brain CT and brain MRI were normal. This p

Twenty patients admitted to hospital with congestive heart failure were evaluated for severe concomitant lumbosacral and leg pain. In each instance the discomfort was worse at night and progressively decreased with a resolution of the pulmonary edema. Neurologic and electromyographic examinations in all but four patients were normal with reflexes, strength and straight leg raising testing normal. An absent Achilles reflex was recognized in two, a diminished knee jerk reflex in one and in a third, weakness in the extensor hallucis longus. Lumbar spinal stenosis was identified in all of the pati

A 60-year-old man suddenly suffered from left hemiplegia with sensory loss on the left side of his face, trunk, and extremities. Brain CT disclosed hemorrhage in the right thalamus and internal capsule. Blink reflex with electrical stimulation on the left supraorbital nerve showed normal early response (R1) on the left side but absent late response (R2) on both sides, while stimulation of the right supraorbital nerve showed normal R1 on the right side and normal R2 on both sides. This observation suggests that the impulses of R2 from stimulation of the supraorbital nerve on the affected side m

A case of isolated vagus nerve palsy with herpes zoster was reported. A 31-year-old woman was admitted to our hospital with six days' history of difficulty of swallowing of fluid and hoarseness with a painful vesicle on the right ear. Neurological examination revealed poor elevation of soft palate on the right side, but the pharyngeal reflex was preserved. Herpetic vesicles were present on the right concha, within posterior wall of the external auditory meatus. No facial palsy, loss of hearing and mucosal lesions in the mouth or pharynx were present. This is the first case of isolated vagus ne

Central and peripheral neural regulation of swallowing and aerodigestive reflexes is unclear in human neonates. Functional near infrared spectroscopy (NIRS) is a noninvasive method to measure changes in oxyhemoglobin (HbO) and deoxyhemoglobin (HbD). Pharyngoesophageal manometry permits evaluation of aerodigestive reflexes. Modalities were combined to investigate feasibility and to test neonatal frontoparietal cortical changes during pharyngoesophageal (visceral) stimulation and/or swallowing. Ten neonates (45.6 ± 3.0 wk postmenstrual age, 4.1 ± 0.5 kg) underwent novel pharyngoesophageal manome
