Horizontal extraocular muscle insertion locations in children undergoing extraocular muscle surgery.

BACKGROUND: The location of extraocular muscle (EOM) insertions is clinically relevant in ophthalmologic surgery. The spiral of Tillaux has been a reference for normal EOM insertion since the nineteenth century. Research on EOM insertions is limited and has focused on adult cadaveric eyes. In this study we investigated the location of EOM insertions in children undergoing EOM surgery. METHODS: Children who underwent EOM surgery for strabismus or anomalous head position at a large tertiary care center were consecutively identified from October 2022 to December 2023. Intraoperative measurements

Clinical analysis of dry eye after refractive surgery in army recruits in 2024.

Dry eye is among the most prevalent complications following refractive surgery, significantly impacting the training and daily lives of recruits. While recent years we have witnessed some advancements in understanding the occurrence and progression of dry eye, the specific effects of refractive surgery on this condition remain unclear. This study aims to investigate the impact of refractive surgery on dry eye among 300 army recruits. A series of examinations specific to dry eye were conducted on the subjects using the OSDI questionnaire and the ocular surface comprehensive analyzer. The correl

Effect of Limbal relaxing incisions during implantable collamer lens surgery.

BACKGROUND: The limbal relaxing incisions (LRIs) technique is a safe and an inexpensive procedure, which is simple for experts to perform. It can effectively reduce astigmatism and result in a rapid visual rehabilitation. But there are few reports about reducing pre-existing corneal astigmatism by LRI in ICL surgery. Our research was aimed to study the effect of limbal relaxing inci sions during implantable collamer lens (ICL) surgery. METHODS: A prospective analysis reviewing consecutive cases of corneal astigmatism that had either independent ICL surgery (control group) or combined with LRI

Prevention of Iatrogenic Keratectasia.

Iatrogenic corneal ectasia is a rare but devastating complication after refractive surgery. While its incidence appears to be declining, it has not been eliminated. Its cause is probably related to a combination of an intrinsic predisposition to ectasia and an additional anatomical destabilising effect from the refractive surgery. Determining which eyes are intrinsically "weak" and exactly how much additional "weakening" an eye can withstand before developing progressive thinning and protrusion are both difficult tasks. The essentially universal availability of corneal topography and the incre

Five-year results of laser in-situ keratomileusis (LASIK) after penetrating keratoplasty.

OBJECTIVE: To study the long-term refractive results of LASIK after penetrating keratoplasty. PLACE: Clinica Barraquer de América. METHODS: Retrospective review of 46 eyes of 38 patients that underwent LASIK for the correction of refractive errors after penetrating keratoplasty. RESULTS: The mean interval between keratoplasty and LASIK was 7 years. The preoperative defocus equivalent refraction (DEQ) was 2.0 to 7.0 D in 56% (26/46) of eyes and 8.0 to 24.0 D in the remaining 20 eyes. Five years after LASIK, DEQ was 1.0 to 7.0 D in all eyes. Seventy-five percent of eyes had a refractive error

Visual Function After Schlemm's Canal-Based MIGS.

Filtration surgery is highly effective in lowering intraocular pressure; however, it is associated with a higher risk of severe complications. Visual dysfunction may persist in relatively uneventful cases because of induced astigmatism or worsening optical aberrations. Therefore, for early- to moderate-stage glaucoma, an increasing number of surgeons are prioritizing surgical safety and preserving postoperative visual function by opting for minimally invasive glaucoma surgery (MIGS). Among the various MIGS techniques, canal-opening surgery-targeting aqueous outflow through the Schlemm's canal

Future Trends in Presbyopia Correction.

Presbyopia affects 1.8 billion people worldwide. This reduction in distance corrected near visual acuity impacts quality of life, which prompts patients to seek treatment. Presbyopia is an early manifestation of the "crystalline lens optical dysfunction through aging," or dysfunctional lens syndrome, and appropriate management of presbyopia is dependent on coexisting factors such as increased higher order aberrations, reduced contrast sensitivity, light scatter, and lenticular opacification. This review of published literature (PubMed and MEDLINE) is presented in narrative format and discusses

What DSAEK is going on? An alternative to penetrating keratoplasty for endothelial dysfunction.

BACKGROUND: Traditionally, penetrating keratoplasty (PK) has been the only surgical treatment for patients with Fuchs' dystrophy and pseudophakic bullous keratopathy. Over the last several years, an alternative technique has become viable. Descemet's stripping automated endothelial keratoplasty (DSAEK) is a partial transplant that replaces only posterior corneal tissue, leaving the healthy anterior cornea intact. METHODS: A retrospective review of the current literature was conducted. RESULTS: Initially, DSAEK surgeons were plagued by high graft dislocation and failure rates. However, modifi

Biomechanics and wound healing in the cornea.

The biomechanical and wound healing properties of the cornea undermine the predictability and stability of refractive surgery and contribute to discrepancies between attempted and achieved visual outcomes after LASIK, surface ablation and other keratorefractive procedures. Furthermore, patients predisposed to biomechanical failure or abnormal wound healing can experience serious complications such as keratectasia or clinically significant corneal haze, and more effective means for the identification of such patients prior to surgery are needed. In this review, we describe the cornea as a compl
