The changes in laryngeal resistance (LR) during respiratory arrest produced by withdrawal of hypoxia stimulation and administration of various respiratory depressants were studied in 14 spontaneously breathing, anesthetized cats (11 cats with alpha-chloralose and three cats with halothane). Withdrawal of hypoxia stimulation caused a transient respiratory arrest with no central inspiratory activity, during which a considerable increase in LR was observed to a level higher than the fixed resistance after muscle paralysis [LR(fix)]. Intravenous injection of thiopentone, ketamine, and lidocaine al

Despite major development of complementary investigations, diagnosis of muscular disorders is mainly clinical. Myopathies can present with unusual or atypical clinical features: myotonia, periodic paralysis, respiratory failure, swallowing difficulties, ptosis, ophtalmoplegia, camptocormia, distal or asymmetrical limb muscle weakness. It can be cause of inaccurate diagnosis. The clinical variability of muscular disorders must be recognized by physicians to drive diagnosis process and to offer an opportunity for early interventional management strategies. This paper reviews the main atypical cl

We reported a rare case of early-onset acute disseminated encephalomyelitis (ADEM). After a nonspecific upper respiratory infection, a 14-month-old boy developed oculomotor nerve paralysis, nystagmus, intention tremor and ataxic gait. MRI showed extensive symmetric high signal lesions in th bilateral cerebellar and cerebral white matters. We made the diagnosis of ADEM on the basis of high CSF myelin basic protein, clinical course, symptoms and MRI findings. Following the administration of steroid hormone, his clinical symptoms rapidly improved.

We report a patient with severe obstructive sleep apnea (OSA) associated with a unique combination of syringobulbia-myelia, Chiari malformation type I (CM), absent hypoxic ventilatory drive, vocal cord paralysis, post-menopausal status, obesity, and acute respiratory failure necessitating mechanical ventilation. The remote onset of OSA five years after surgery underscores the need for long-term follow-up of patients with syringobulbia-myelia and CM and the importance of addressing multiple interacting neurologic, metabolic, and mechanical predispositions to sleep-disordered breathing.

Diaphragmatic paralysis (DP) in neonates is a rare yet potentially life-threatening cause of respiratory distress, often resulting from obstetric trauma or cardiac surgery. This report presents two distinct cases of bilateral DP: one following a dystocic delivery with associated brachial plexus involvement, and the other linked to a genetic mutation (SYNGAP1) in a neonate with no birth trauma. Diagnosis was established through imaging, fluoroscopy, electromyography, and genetic testing. In both cases, conservative management was initially pursued; however, due to persistent respiratory failure

The sudden and storming onset of coronavirus 2 infection (severe acute respiratory syndrome coronavirus 2 [SARS-CoV-2]) was associated by severe acute respiratory syndrome. Recently, corona virus disease 19 (COVID-19) has appeared as a pandemic throughout the world. The mutational nature of the virus, along with the different means of entering and spreading throughout the body has involved different organs. Thus, patients are faced with a wide range of symptoms and signs. Neurological symptoms, such as anosmia, agnosia, stroke, paralysis, cranial nerve deficits, encephalopathy, meningitis, del

The authors report a child with spinal muscular atrophy with respiratory distress type 1 (SMARD1). She presented atypically with hypothyroidism and heart failure due to septal defects that required early heart surgery and microcephaly in association with cerebral atrophy and thin corpus collosum. The subsequent asymmetrical onset of diaphragmatic paralysis, persistent hypotonia, and generalized muscle weakness led to the suspicion of spinal muscular atrophy with respiratory distress type 1. Sanger sequencing confirmed a compound heterozygous mutation in the Immunoglobulin Mu Binding Protein 2

A case of a 55-year-old woman with iatrogenic vocal cord trauma and sleep-related symptoms is reported. In particular, this case highlights sleep-disordered breathing as a delayed complication after iatrogenic vocal cord trauma. The patient developed acute stridor from a contralateral vocal cord hematoma following vocal fold injection for right vocal cord paralysis. Acute respiratory symptoms resolved with oxygen, steroids, and nebulized therapy, but nocturnal symptoms persisted and polysomnography revealed sleep-related hypoventilation and mild obstructive sleep apnea. Positive pressure thera

Unilateral diaphragmatic paralysis is an uncommon cause of dyspnea and orthopnea and may be overlooked when symptoms are attributed to more common cardiopulmonary conditions. This is particularly relevant in patients with underlying chronic obstructive pulmonary disease, where progressive breathlessness may initially be attributed to worsening obstructive lung disease. Parsonage-Turner syndrome, also known as neuralgic amyotrophy, is a rare neuropathic disorder that typically presents with acute shoulder or upper limb pain followed by weakness. Phrenic nerve involvement is uncommon but can res

BACKGROUND AND AIM: Interscalene Brachial Plexus Block (ISBPB) is commonly used for shoulder surgery anesthesia to reduce opioid use and general anesthesia complications. However, it may cause diaphragmatic paresis due to phrenic nerve involvement. This study compares the incidence of hemidiaphragmatic paralysis and the frequency of side effects-including hemodynamic changes and postoperative complications-between the Extrafascial (ExF) and Intrafascial (InF) approaches for ISBPB using reduced anesthetic volume. The aim is to assess whether the ExF approach may be preferable in patients with r

The article describes pregnancies and labors of five women with myotonic muscular dystrophy and their four severely involved infants, and reviews the pertinent literature. Three of the four neonatal cases died by 3 weeks of age of respiratory failure or aspiration; the fourth infant is now 4 months old and has respiratory and swallowing difficulties. The symptoms of myotonic dystrophy worsen during pregnancy. A high rate of fetal loss occurs due to spontaneous abortion, prematurity, and neonatal involvement with the disease. Prolonged labor has been described as a consistent complication, but

Guillain-Barre syndrome (GBS) is an immune-mediated disease of the peripheral nerves and cause of acute flaccid paralysis in children around the world. The most common type of GBS in North America targets myelin and leads to demyelinating neuropathy. Often there is a history of infection in the weeks preceding motor symptoms. GBS has been associated with different infections, including COVID. Children usually recover motor function, but autonomic instability and respiratory compromise can occur necessitating close observation and potentially intensive care unit admission.

An 8-year-old neutered male Siberian forest cat was presented for respiratory distress. Endoscopy, exploratory surgery of the neck, and sternotomy confirmed initial placement of the esophagostomy tube (e-tube) into the mediastinum. The patient recovered from surgery, developed laryngeal paralysis, and then arrested and died within 48 h after surgery. Key clinical message: To the authors' knowledge, there are no other case reports documenting inadvertent placement of an e-tube into the mediastinum of a cat. This case report highlights the complications that can be associated with incorrect plac

Vocal cord dysfunction (VCD) has been frequently discussed as confounding the diagnosis of asthma, and it is recommended to examine vocal cord function in cases of difficult asthma. However, there is also strong evidence that asthma may be associated with VCD, sometimes masquerading as exercise-induced asthma. Gerhardt's syndrome frequently induces severe acute respiratory distress requiring ventilatory support, and this condition might be mistaken for a severe acute asthma attack. Here, a case of bilateral paralysis of the vocal cords, which was associated with asthma, is described.

Since 1973 we have used intralaryngeal injection of Teflon for vocal improvement in cases of unilateral vocal cord paralysis, postoperative scarring of the vocal cord and hypofunctional dysphonia. So far we have treated 84 patients. Teflon is the best substance to use, it remains at the place of instillation, keeps its shape and is not resorbed. As immediate reactions we have observed sporadic oedematous swellings of the injected vocal cord. No long-term reactions are know. The functional results are vocal improvement without restriction of the respiratory passage.

INTRODUCTION: Neurotoxic envenomation often presents with non-specific neurological symptoms and minimal local signs, which can delay appropriate diagnosis and treatment. This is the first reported case of a neurotoxic snakebite presenting with an atypical symptom of unilateral neck pain.
CASE REPORT: A 12-year-old girl was referred to our emergency centre with neck weakness progressing to quadriplegia, attributed to a fall while playing. A diagnosis of acute flaccid paralysis secondary to cervical trauma was made and treated at the first hospital; however, she developed respiratory distress

BACKGROUND: Botulism caused by type F botulinum toxin accounts for less than 0.1% of all human botulism cases and is rarely reported in the literature.
CASE REPORT: A 45-year-old woman presented to an emergency department complaining of blurred vision, difficulty focusing, and dysphagia. The treating physician initially considered the possibility of paralytic shellfish poisoning due to a report of shellfish ingestion, which was later determined to be frozen shrimp and a can of tuna, but no gastroenteritis or paresthesias were present. During the emergency department observation, the patient d

Our understanding of the effects of anaesthesia is incomplete. Anaesthesia works primarily by causing widespread depression of the central nervous system. This article will consider the anaesthesia provided for a patient undergoing a hysteroscopy, endometrial ablation and biopsy for abnormal uterine bleeding. The relationship between physiology and the effects of anaesthesia is discussed. Several effects of anaesthesia are observed during the case. Relating to the respiratory system, preoxygenation increased end tidal oxygen by 39%, delaying desaturation during apnoea. Propofol had a profound
