[Sex-specific differences in risks and management of coronary heart disease].

Reports from several countries indicate that women are disadvantaged in the treatment of coronary heart disease. The aim of the present review is to examine--on the basis of current population-based data (National Health Survey [NHS], hospital diagnosis registry, mortality rates)--whether in the Federal Republic of Germany more recently a change in favor of women could be established. According to NHS results prevalence rates of hypertension, overweight, and smoking in men exceed those of women, whereas hypercholesterolaemia is equally prevalent in both sexes. From 1984/85 to 1990/91 the NHS r

The role of placental examination and its pathology in obstetric risk management.

The placenta is a marker of biologic events that occur during pregnancy. Placental pathologic examination gives insight to pregnancy-related insults, which affect the newborn at and after birth. Clinicians and risk managers can use this information to identify the cause of newborn complications in cases suggested to be birth-related injuries.

Identification and management of the woman at increased risk for breast cancer development.

Multiple factors which increase a women's breast cancer risk have been identified. These range from conditions such as lobular carcinoma in situ which increase risk to relatively high levels, to reproductive factors such as nulliparity which are associated with only a small increase in risk. When determining an individual's risk, all her potential breast cancer risk factors must be considered. In order for risk information to be meaningful to a woman, risk must be expressed as absolute risk over a defined time interval since there is no uniform agreement on what risk level is high enough to re

The extremely large fetus--antenatal identification, risks, and proposed management.

The aim of this study was to evaluate the efficacy of current methods in identifying fetuses with a weight greater than or equal to 5,700 g, and to study the perinatal mortality and morbidity in relation to the mode of delivery. Between 1973 and 1984, 110 newborns in Sweden had a birthweight greater than or equal to 5,700 g. These cases were identified using the National Medical Birth Registry, and from this data base, collecting antenatal, perinatal and infant data since 1973, a reference population was also selected. Tall, parous women with a high pre-pregnancy weight and an abnormal weight

Postsplenectomy sepsis syndrome. How to identify and manage patients at risk.

Established postsplenectomy sepsis syndrome, although infrequent, may carry a mortality rate of over 90%. Pneumococcus (Streptococcus pneumoniae) has been the infective organism in more than 50% of published cases. Certain groups, such as infants, patients with hematologic malignancy, and those with compromised humoral immunity, may be especially susceptible. However, even healthy individuals who have been splenectomized because of trauma may be affected. Within the general population there is a significant pool of asplenic or functionally hyposplenic patients who are not aware of their condit

Dantrolene sodium in the management of patients at risk from malignant hyperthermia.

Malignant hyperthermia is a dominantly inherited, usually subclinical, disease that occurs in individuals who have an underlying muscular disorder and connotes the gravest possible consequences. When it occurs, it is usually during the use of muscle relaxants in anesthesia and potent anesthetic agents such as halothane. Patients at risk must be identified through careful history and screening procedures; however, a patient susceptible to this condition may have had general anesthesia in the past without complications. A careful monitoring regimen must be established for the procedure and some

The Importance of Recognition and Management of Oral Lichen Planus: Navigating Epidemiology, Pathogenesis, Cancer Risk, and Advances in Management.

BACKGROUND: Oral lichen planus (OLP) is a chronic inflammatory disorder classified as an oral potentially malignant disorder (OPMD) due to its association with an increased prevalence and a risk of progressing to oral squamous cell carcinoma (OSCC) of up to 5%. METHODS: We conducted a comprehensive literature review to update the epidemiology, etiology, pathogenesis, diagnosis, and management of OLP using PubMed and Web of Science databases up to April 2025. RESULTS: OLP is a T-cell-mediated disorder, with CD4+ and CD8+ T cells playing a key role in its pathogenesis. Its multifactorial natur

Therapeutic Options for the Management of Pompe Disease: Current Challenges and Clinical Evidence in Therapeutics and Clinical Risk Management.

Pompe disease is a genetic disorder produced by mutations in the GAA gene leading to absence or reduced expression of acid alpha-glucosidase, an enzyme that metabolizes the breakdown of glycogen into glucose. There are two main phenotypes, the infantile consisting of early onset severe weakness and cardiomyopathy, and the adult which is characterized by slowly progressive skeletal and respiratory muscle weakness. Enzymatic replacement therapy (ERT) has been available for Pompe disease for more than 15 years. Although the treatment has improved many aspects of the disease, such as prolonged sur

The drivers of sea lice management policies and how best to integrate them into a risk management strategy: An ecosystem approach to sea lice management.

The control of sea lice infestations on cultivated Atlantic salmon is a major issue in many regions of the world. The numerous drivers which shape the priorities and objectives of the control strategies vary for different regions/jurisdictions. These range from the animal welfare and economic priorities of the producers, to the mitigation of any potential impacts on wild stocks. Veterinary ethics, environmental impacts of therapeutants, and impacts for organic certification of the produce are, amongst others, additional sets of factors which should be considered. Current best practice in both

Clinical usefulness of polygenic risk scores in risk prediction models for lung cancer screening and lung nodule management.

BACKGROUND: Lung cancer mortality decreases as a result of low-dose computed tomography (LDCT) screening, but suffers from low uptake and high false-positive rates. The impact of integrating genetic risk using a polygenic risk score (PRS) to optimize lung cancer screening remains underexplored. METHODS: We developed a genome-wide PRS and evaluated its performance in pre- and post-screening contexts. Screening eligibility was assessed using two UK Biobank (UKB) subsets: UKBPLCO (n = 8957; PLCOm2012norace risk ≥2%) and UKBScreeningCriteria (n = 74,024 meeting screening eligibility criteria). To

Risk of Residual Cancer following Immediate Surgery or Nonoperative Management after Malignant Colorectal Polypectomy with High-Risk Features.

PURPOSE: National guidelines recommend colorectal resection when pathology from a malignant colorectal polyp reveals high-risk features. However, some patients opt to avoid surgery due to concerns about associated morbidity and potential impacts on quality of life, despite the risk of residual disease. METHODS: In this single-center cohort study of patients treated between 2015 and 2022, we retrospectively compared rates of residual disease between patients who underwent immediate surgery and patients managed nonoperatively following removal of a malignant polyp with high-risk features. Estimating Risk Factor Time Paths Among People with Type 2 Diabetes and QALY Gains from Risk Factor Management.

OBJECTIVES: Most type 2 diabetes simulation models utilise equations mapping out lifetime trajectories of risk factors [e.g. glycated haemoglobin (HbA1c)]. Existing equations, using historic data or assuming constant risk factors, frequently underestimate or overestimate complication rates. Updated risk factor time path equations are needed for simulation models to more accurately predict complication rates. AIMS: (1) Update United Kingdom Prospective Diabetes Study Outcomes Model (UKPDS-OM2) risk factor time path equations; (2) compare quality-adjusted life-years (QALYs) using original and u

Financial constraints on genetic counseling and further risk-management decisions among U.S. women at elevated breast cancer risk.

Clinical guidelines recommend that women at high risk of breast cancer should consider various risk-management options, which remain widely underutilized. We conducted semi-structured, qualitative interviews with 50 high-risk women to understand how financial constraints affect use of genetic counseling, genetic testing, and further risk-management decisions. Inductive analyses revealed three categories of health-related financial constraint: (a) lack of insurance, (b) underinsurance, and (c) other financial constraints (e.g., medical debt, raising children, managing comorbidities). Various br

Medical Management of Modifiable Risks: Improving Survival in High-Risk Prostate Cancer Patients Receiving Brachytherapy.

Background: High-risk (HR) prostate cancer has a propensity for local and distant progression with ultimate death, mandating aggressive locoregional and systemic treatment approaches to maximize oncologic outcomes. Although brachytherapy (BT) with supplemental therapies has demonstrated favorable biochemical and quality of life outcomes, improvements in overall survival have been hampered by an excessive incidence of non- prostate cancer deaths. In this HR study, we report on biochemical failure (BF), prostate cancer-specific mortality (PCSM), overall mortality (OM) and patterns of death with

Cardiovascular risks in men with prostate cancer: a pragmatic, clinician-oriented review of risk stratification and management strategies.

BACKGROUND/OBJECTIVES: Recent advances in prostate cancer management have improved overall survival and quality of life for those afflicted. However, these patients continue to suffer from a high burden of cardiovascular disease through a variety of mechanisms, including both the cancer itself as well as prostate cancer-specific treatments. SUBJECTS/METHODS: Cardiovascular disease is now the leading cause of death in this population, and current data suggests that many patients remain sub-optimally controlled from a risk factor standpoint. There is a lack of consensus and standardized guideli

COVID-19 Associated Cardiovascular Disease-Risks, Prevention and Management: Heart at Risk Due to COVID-19.

The SARS-CoV-2 (Severe Acute Respiratory Syndrome Coronavirus-2) virus and the resulting COVID-19 pandemic have had devastating and lasting impact on the global population. Although the main target of the disease is the respiratory tract, clinical outcomes, and research have also shown significant effects of infection on other organ systems. Of interest in this review is the effect of the virus on the cardiovascular system. Complications, including hyperinflammatory syndrome, myocarditis, and cardiac failure, have been documented in the context of COVID-19 infection. These complications ultima

Are gastroenterologists less tolerant of treatment risks than patients? Benefit-risk preferences in Crohn's disease management.

BACKGROUND: Crohn's disease is a serious and debilitating gastrointestinal disorder with a high, unmet need for new treatments. Biologic agents have the potential to alter the natural course of Crohn's disease but present known risks of potential serious adverse events (SAEs). Previous studies have shown that patients are willing to accept elevated SAE risks in exchange for clinical efficacy. Gastroenterologists and patients may have similar goals of maximizing treatment benefit while minimizing risk; however, gastroenterologists may assess the relative importance of risk differently than pati

[Risk management and clinical strategies for early enamel demineralization in orthodontic patients].

Early enamel demineralization, one of the common side effects of orthodontic treatment, occurs when dental plaque accumulation disrupts the balances of demineralization-remineralization and the oral microbial ecology. Clinically, the existence of orthodontic appliances can make oral hygiene more challenging. At the same time, patient adherence to oral hygiene appointments may be inconsistent. The combination of these factors often leads to inadequate plaque control and increases the risk of enamel demineralization. Presently, there is a lack of standardized guidelines for the clinical interven
