Family roles in homes with alcohol-dependent parents: an evidence-based review.

The behavioral health care field has seen attempts to understand the functioning of families in which a parent is dependent on alcohol as a set of roles into which the other family members fall. The most popular of these classifications taught in the United States includes five roles (enabler, hero, lost child, mascot, and scapegoat) that are used to conceptualize families and individuals in treatment and support group settings, as well as in popular self-help literature. Attempts to operationalize and measure these roles have, however, been fraught with difficulties. The resulting research ba

[Institutionalized treatment of agoraphobia with panic anxiety].

We describe an integrated behavioral-psychodynamic treatment programme for inpatients suffering from agoraphobia and panic anxiety. This programme emphasizes intensive drug-free exposure training followed by work with anxiety-laden dynamic conflicts. We present promising results of evaluation studies, and discuss the important role of cognitive reconstruction of anxiety and of passive and active avoidant attitudes in exposure training. Lastly, we make certain recommendations concerning the approach to anxiety problems in general practice.

Perspectives of healthcare professionals on the pharmacist's role in delivering vaccinations for patients with cancer: a qualitative study using role theory.

BACKGROUND: Vaccination rates for influenza and pneumococcal disease globally remain below targets for patients with cancer. Pharmacists already provide vaccinations in the community, highlighting an opportunity to bridge the gap by expanding hospital pharmacists scope of practice to deliver vaccinations in outpatient oncology settings. AIM: To explore the perceptions of multidisciplinary healthcare professionals (HCPs) for the potential expansion of pharmacists' role to include a vaccination clinic for patients undergoing cancer treatment. METHOD: Semi-structured interviews were conducted w

Route of delivery of infants with congenital anomalies.

Obstetric delivery may be accomplished by only two methods, vaginally or abdominally. In the management of the pregnancy complicated by a fetal malformation, the choice of delivery method may be made on obstetrical grounds or in belief that one method offers the fetus benefit over the other. That choice must be based on knowledge of the nature of the individual malformation in question, the presence or absence of associated fetal malformations, the presence or absence of fetal karyotype abnormalities, fetal maturity, and fetal presentation. Clear evidence of benefit from cesarean delivery is n

War trauma and psychomotor developmental disorders in children aged 6-12 in Lubumbashi: a comparative study.

Background: Persistent armed conflict in eastern Democratic Republic of the Congo has resulted in large-scale population displacement, exposing many children to traumatic experiences. Although the psychological consequences of such exposure are well documented, its impact on psychomotor development remains insufficiently investigated, particularly in African settings affected by chronic violence. Objective: This study aimed to compare the psychomotor development of displaced children living in Lubumbashi according to their level of exposure to war-related trauma (direct, indirect, or none) and

Temporal disorientation during war: associations with work-family conflict, emotional distress, and burnout.

INTRODUCTION: Disruptions in the temporal experience-such as subjective sense of time being long, short or distant compared to "objective" time-have been found in the context of extreme emotional events. This research, conducted a month after the start of the war in Israel, aimed to identify characteristics of temporal experience among a population facing ongoing crisis conditions, and to examine their relationship to emotional distress and burnout. Based on Hobfoll's Conservation of Resources Theory it was hypothesized that temporal disorientation is essentially the loss of a vital psychologi

Conflict and fragmented public health emergency management system in Tigray region of Northern Ethiopia: A double burden to accommodate resilient and advanced public health emergency management. A commentary review for policy-makers and a call to action.

An estimated 2.5 million people have been internally and externally displaced in the Tigray region of northern Ethiopia in conflict and post-conflict settings. This induced a loss of access to basic and essential healthcare services. The situation was overwhelming, causing service inaccessibility, inadequate health facilities, unstable security to access the services, shortage of supplies and drugs, and medical equipment's in the region. The regional public health emergency management is one service delivery set up for the critically ill. It is characterized by weak emergency management capaci

Impaired inhibition of prepotent motor actions in patients with Tourette syndrome.

BACKGROUND: Evidence that tic behaviour in individuals with Tourette syndrome reflects difficulties inhibiting prepotent motor actions is mixed. Response conflict tasks produce sensitive measures of response interference from prepotent motor impulses and the proficiency of inhibiting these impulses as an act of cognitive control. We tested the hypothesis that individuals with Tourette syndrome show a deficit in inhibiting prepotent motor actions. METHODS: Healthy controls and older adolescents/adults with persistent Tourette syndrome without a history of obsessive-compulsive disorder or atten

Spinal injuries after improvised explosive device incidents: implications for Tactical Combat Casualty Care.

BACKGROUND: Tactical Combat Casualty Care aims to treat preventable causes of death on the battlefield but deemphasizes the importance of spinal immobilization in the prehospital tactical setting. However, improvised explosive devices (IEDs) now cause the majority of injuries to Canadian Forces (CF) members serving in Afghanistan. We hypothesize that IEDs are more frequently associated with spinal injuries than non-IED injuries and that spinal precautions are not being routinely employed on the battlefield. METHODS: We examined retrospectively a database of all CF soldiers who were wounded an

Research in relation to equity: extending the agenda.

An appreciation of the role of social and emotional well-being in determining health outcomes is important in advancing the equity agenda.(1) These aspects of health are adversely affected by inequity. They also are important as potential causal factors. Low levels of emotional and social well-being among the rich may be important in perpetuating health and social inequity. In the past, most research on inequity has focused on the negative end of the continuum of emotional and social health (eg, child abuse, conduct disorder, mental illness, drug and alcohol abuse) and concentrated on the prob

Conflicts of interest in medical science: peer usage, peer review and 'CoI consultancy'.

In recent years, the perception has grown that conflicts of interest are having a detrimental effect on medical science as it influences health policy and clinical practice, leading medical journals to enforce self-declaration of potential biases in the attempt to counteract or compensate for the problem. Conflict of interest (CoI) declarations have traditionally been considered inappropriate in pure science since its evaluation systems themselves constitute a mechanism for eliminating the effect of individual biases. Pure science is primarily evaluated by 'peer usage', in which scientific inf

The Experience of Family Caregivers Accompanying Older Adults with Dementia During Hospitalization: A Qualitative Descriptive Study.

This qualitative study explored the hospitalization journey of people with dementia in Taiwan from their family caregivers' perspectives. Semi-structured interviews with nine caregivers revealed three key themes: care model tensions, psychological/moral burdens, and environmental conflicts. Hospitalization exposes systemic gaps between acute-care efficiency and dementia-friendly needs. Medical social workers play a vital role as humanizing advocates through early psychosocial support, caregiver advocacy, and integrated community transitions.

Ebola Virus Disease.

Nurses are the largest group of health care providers and, therefore, are often at the forefront of epidemics: responding, treating, educating, and coordinating care as needed. But what happens when nurses are afraid of contracting an illness and decide to leave the workplace? The fear due to Ebola was in part caused by conflicting information around the proper use of personal protective equipment and need for quarantine. The nursing response to as well as the role occupational health nurses can play in diffusing the fear of contracting contemporary infectious diseases are discussed.

Colorectal carcinoma associated with schistosomiasis: a possible causal relationship.

The association between schistosomiasis and colorectal malignancy has long been suggested in the literature, but it is not uniformly accepted. In the Far East, considerable evidence supports an etiological link between Schistosoma japonicum and colorectal cancer. However, the available data regarding the role of Schistosoma mansoni in colorectal carcinogenesis are conflicting and most often do not show causality. We report on a patient with sigmoid colonic cancer coexisting with schistosomiasis, and we provide a comprehensive review of the literature regarding the epidemiology and pathobiology

Rapamycin for focal segmental glomerulosclerosis: a report of 3 cases.

Corticosteroids and/or cyclosporine constitute the present therapeutic approach for patients with focal segmental glomerulosclerosis (FSGS). The high incidence of side effects for the former and risk of nephrotoxicity combined with the high relapse rate after discontinuation for the latter render their use problematic. Results concerning the role of rapamycin in the treatment of patients with FSGS are conflicting. We describe results for 3 patients treated with a combination of low-dose steroids and rapamycin for FSGS, focusing on the importance of maintaining low drug (rapamycin) levels by us

Importance of breast awareness in identification of breast cancer.

In a move to reduce the number of deaths from breast cancer there was a shift in emphasis in 1991 away from breast self-examination to breast awareness. This article takes a close look at the background to this shift, the conflicting messages with regards to breast awareness, and the need to re-educate and empower nurses and other health professionals with the knowledge and confidence to teach the public about these issues. Breast Cancer Care plays a vital role in networking with nurses, other health professionals and the public to clarify the breast awareness message.

Is there a role for vitamin D in osteoporosis?

Vitamin D has certain clearly defined effects on bone: vitamin D deficiency results in defective bone mineralization, whereas 1,25-dihydroxy-vitamin D3 (1,25-(OH)2D3) stimulates bone resorption. Studies of the use of 1,25-(OH)2D3 to prevent or treat osteoporosis have given conflicting results concerning bone remodeling. However, 1,25-(OH)2D3 or other vitamin D metabolites seem to play a role in the correction of calcium malabsorption, which is a common feature in osteoporosis.
