Lymphoedema and skin naevi are common in children with Turner syndrome (TS). Lymphoedema in the early stages of fetal life is thought to cause several of the phenotypic characteristics in patients with TS such as nuchal folds and pterygium colli. We present two patients with TS who have unusual lesions on the scalp. The first patient had an oval circumscribed lesion. Two biopsies were obtained from the lesion. This is the first report of malignant melanoma arising from sebaceous nevus (SN) of the scalp, although it has been known that various benign and malignant neoplasms may develop in association with SN. After the excisional biopsy for SN with 5 mm free margin, pathological examination revealed the coexistence of nodular melanoma invading to the reticular dermis (IV level of Clark) for a maximal depth of 4 mm. After resection with another 20 mm free margin, there has been no evidence of local recurrence or distant metastasis for nine years.

OBJECTIVE: To develop a scalp dermatitis-specific quality-of-life instrument.
METHODS: Based on the results of directed focus sessions with 8 patients with scalp psoriasis or seborrheic dermatitis, we conceptualized 3 major constructs that explain the way scalp dermatoses affect patient quality of life: symptoms, functioning, and emotions. We report the first case of a scalp pseudomycetoma due to Trichophyton schoenleinii in a 15-year-old girl from Senegal. The lesion was not associated with tinea capitis. Long-term antifungal therapy with itraconazole and terbinafine was unsuccessful but a favourable outcome was obtained after surgical removal of the lesion. Between 1973 and 1999, only 12 previous cases of dermatophyte pseudomycetoma of the scalp were reported in the literature. Clinical features and therapeutic outcomes of these cases are reviewed.

Sarcoidosis of the scalp is a rare manifestation of cutaneous sarcoidosis. We report 4 cases, all in African-American women with systemic sarcoidosis. A review of the English-language literature reveals only 24 reported cases, most in African-American women with systemic involvement and other cutaneous lesions. Scalp sarcoidosis may exhibit variable morphologies and therefore must be kept in the differential diagnosis of plaques or nodules of the scalp as well as both cicatricial and noncicatricial alopecia.

Five patients with avulsed scalps were treated with replantation between 1992 and 1998. All patients were women age 20 to 36 years. The percentage of the avulsed scalp ranged from 50% to 100% of the whole scalp. The vessels chosen for anastomosis were the superficial temporal artery, occipital artery, and superficial temporal vein. A 60-year-old patient presented with a mycetoma of the scalp with multiple chronic sinuses due to nocardiosis of the scalp. Recurrent bleeding from the sinuses necessitated surgical excision which led to a gratifying result. The case illustrates the importance of fungal culture in the diagnosis of cutaneous lesions and emphasises the role of surgery in mycetoma that are resistant to conventional therapy.

We report an epidermoid cyst of the scalp with light microscopic and immunohistochemical evidence of human papillomavirus (HPV) infection. Light microscopic features suggesting HPV infection included papillomatosis and hypergranulosis of the cyst lining with the presence of koilocytes. The authors report a case of a 61-year-old woman presenting with a giant mass on the occipital scalp. The patient had no neurological deficits and was of normal intelligence. From 1990 to 1991, 6 avulsed scalps (total avulsing in 5, partial avulsing in 1) were replanted successfully. Follow-up for over 6 months showed that the hair of the replanted scalps grew normally in all the 6 patients. We think that the key to successful replantation is to anastomose the vessels effectively. The scalp can survive just with its superficial temporal vessels on both sides and with an artery-vein ratio of 1:1. The management of postoperative complications is discussed.

In a search for an invisible skin donor site and a comfortable dressing for the donor site, a study was designed in which the scalp was used as the donor site and Duoderm was applied as the dressing. The study contained 18 children with minor burns. A Duoderm dressing on scalp donor sites showed a normal healing time. In using the scalp as a donor site the patient can be mobilized very quickly after the operation and the scar is hidden and invisible 1 month postoperation.
