BACKGROUND: The scaphoid is the most commonly fractured carpal bone in children. True scaphoid fractures have proven to be difficult to diagnose, as they may not be apparent on initial imaging. Children with clinical suspicion of a scaphoid fracture may be treated with continued immobilization, even in the absence of radiographic evidence of a fracture. The purpose of this study is to identify predictors of true scaphoid fractures in children to help guide management.
METHODS: This study is a retrospective cohort study of children presenting to a tertiary pediatric hospital with hand or wrist

UNLABELLED: Carpal fractures were identified by the Geelong Osteoporosis Study Fracture Grid for 2006-2007. Incidence rates were higher in m … fractures in males and females, respectively. Of these, 131 males and 29 females had fractured the scaphoid bone. Females had a higher proportion of left-sided fractures (>70 %) than males (∼40 %). Most fractures were the result of an accidental fall (>87 %). Patterns of incidence for males showed one major peak around 20-29 years. For females, peaks occurred …

The purpose of this study was to analyze the relative bio- mechanical stability of three types of internal ixation with cancellous bone graft in a cadaveric, scaphoid nonunion model. A scaphoid nonunion model was created by remov- ing a volar wedge of bone from the waist of the scaphoid in 18 fresh frozen human cadavers. Cancellous sawbone graft was inserted into the osteotomy site and three groups of six cadavers each were then internally ixed with a pair of parallel 0.045-inch K-wires, Mini-Acutrak screws, or Standard Acutrak screws, respectively for each group. The potted specimens were tes

BACKGROUND: Volar locking plate fixation may offer several advantages over headless screw fixation for scaphoid nonunion, or segmental or comminuted fractures: (1) increased surface area for bony healing; (2) preserved vascularity; and (3) maintenance of a gap for graft insertion. The purpose of this study is to compare headless screw and locking plate fixation of segmental scaphoid fractures and to determine whether either fixation provides a greater mechanical advantage in osteoporotic versus nonosteoporotic bone.
METHODS: Sixteen matched-pair cadaver scaphoids were dissected from a range o

BACKGROUND: The goal of this study was to use nano-computed tomography to describe the intraosseous vascularity and structural characteristics of commonly used distal radius vascularized bone grafts for treatment of scaphoid nonunion.
METHODS: We obtained 8 fresh frozen human cadaver forearm specimens for infusion of barium contrast. Specimens were scanned and segmented to quantify the vascular volume and trabecular density within 3 common graft regions, including 1, 2 intercompartmental supraretinacular artery (1,2 ICSRA), fourth extensor compartment artery (4 ECA), and volar carpal artery (

Current literature does not definitely demonstrate the superiority of any particular scaphoid reconstruction method. The primary goal of this retrospective single center study was to evaluate the influence of reconstruction techniques and other factors on the union rate after reconstruction of scaphoid nonunions. In the study, 370 patients with nonunions classified as stable (Mayo 1) or unstable (Mayo 2) depending on carpal alignment were included. Minimal radiological follow-up was 3 months. Bone healing after scaphoid reconstruction was evaluated using computer tomography scans in 294 and ra

The wrist joint is a complex articulation that involves the distal radius, ulna, eight carpal bones (scaphoid, lunate, triquetrum, pisiform, trapezium, trapezoid, capitate, and hamate), and the bases of the metacarpals. Trans-scaphoid perilunate fracture-dislocations are complex wrist injuries, most commonly resulting from high-energy mechanisms involving hyperextension and ulnar deviation. Prompt diagnosis and surgical management are essential in restoring wrist function and stability. This report focuses on the case of a 28-year-old male who sustained a trans-scaphoid perilunate dislocation

The management of acute scaphoid fractures should be oriented on the concept of fracture stability, ease of reduction, associated ligamentous injury, and risk of impaired blood supply, rather than the direction of the fracture line or location of the fracture within the scaphoid. The lack of predictability of successful fracture union and prognostic discrepancies of historic and contemporary classifications may be related to subtle differences of the internal vascular architecture of each scaphoid. Because the possibility of impaired vascularity is greater with fractures located in the proxima

INTRODUCTION: This study aims to assess the rates of return to work and sports participation following arthroscopic bone grafting for severe scaphoid nonunion.
PATIENTS AND METHODS: We conducted a retrospective analysis of patients treated for scaphoid nonunion from 2017 to 2024, with a minimum follow-up of one year. All patients underwent arthroscopic repair utilizing an olecranon bone graft and antegrade screw fixation. Employment status and sports participation prior to the diagnosis of nonunion were documented and evaluated at final follow-up.
RESULTS: A total of 21 patients met the incl

Corticocancellous grafting was considered the therapy of choice for treating scaphoid bone pseudarthrosis. However, there has been more criticism recently, since this method neither prevents nor stops arthrosis of the wrist in the long term. Ligamental damage with carpal instability often remains untreated. The malposition and shortening of the scaphoid bone also remains uncorrected. Fifty-five patients in who Matti-Russe reconstruction was carried out to treat schaphoid non-union were followed up. The inclusion criteria were that there had been no preoperative arthrosis and no previous operat…

We carried out a comprehensive analysis of publications to investigate long term union rates of acute proximal scaphoid fractures. Of 1147 acute scaphoid fractures managed nonoperatively that were available for analysis, 67 (5.8%) were proximal. Amalgamating publications revealed that 34% of acute proximal scaphoid fractures progress to nonunion when managed nonoperatively. A meta-analysis showed that the relative risk of nonunion for these fractures is 7.5 compared with more distal fractures, also managed nonoperatively. More trials are needed to allow direct comparison of acute proximal scap

The management of patients with clinical evidence of a fracture of the carpal scaphoid bone but without radiological evidence of a fracture is based on dogma emphasizing the need to immobilize the wrist in all cases. Because of the apparently high proportion of patients who spend up to six weeks in a plaster cast and in whom no fracture is ever demonstrated radiologically, a study was undertaken to determine the fate of those wrist injuries diagnosed as clinical fractures of the scaphoid. All patients who presented with clinical or radiological evidence of fractures of the scaphoid over a one …

Carpal fractures are common orthopaedic injuries, with the scaphoid being the most frequently affected bone. Hamate fractures are uncommon and easily missed due to their subtle clinical presentation and difficulty in detection on standard radiographs, often being misdiagnosed as soft tissue trauma. Simultaneous involvement of the scaphoid and hamate is uncommon, and progression to dual non-union is even more so. We report a case of concurrent scaphoid and hook of hamate non-union to highlight the importance of early recognition and appropriate imaging to prevent delayed diagnosis and long-term

The purpose of this study was to review the clinical results of carpal ligaments injuries with scaphoid nonunion. We hypothesized that scaphoid nonunion with carpal ligament injury is associated with clinical result. We retrospectively reviewed 60 cases of -Herbert screw fixation with bone graft for scaphoid nonunions. Scapholunate (SL) and lunotriquetral (LT) ligaments lesions were confirmed by arthroscopy. Approximately half of the nonunion scaphoid cases had carpal ligaments injuries. At final follow-up evaluation, wrist function as evaluated by the Mayo wrist score was excellent in 34 pati

Fractures of the hand are frequently encountered with injuries to the phalanges and metacarpals comprising the vast majority. Fractures of the carpal bones excluding the scaphoid, however, are fairly uncommon. Despite the rarity of fractures of the remaining seven carpal bones, they can cause a disproportionate amount of morbidity from missed diagnosis due to their subtlety as well as their frequent association with significant ligamentous disruption or even other carpal bone fractures. Delayed diagnosis can result in inadequate fracture care, which places the wrist at risk of disabling sequel

The results of an anatomical and radiological study of one unusual case of bilateral osteochondritis dissecans of the carpal scaphoid are reported. On both sides, histological examination of the excised surgical specimen showed previous remodelling of the bone and marrow which had been followed by further aseptic necrosis. On the painful left side the sequestrum itself was fractured and appeared to be involved in a localised cystic remodelling reaction. The observations are discussed as an example of the histological changes in aseptic bone necrosis as well as an unusual type of pathology affe

We have seen in 4 cases of 46 operated scaphoid pseudarthrosis aseptic bone necrosis of a fragment. According to the tendon interposition plastic of Froimson for the substitute arthrose of the saddlepoint of the thumb. We exstirpated the bionecrotic fragment of the scaphoid, splitted the tendon of the flexor carpi radialis muscle, removed a part of the tendon from the belly of the muscle, rolled it in and fixed it in the excavated cavity of the bone. After an observation time of 6 months to 1.5 years all 4 patients have not developed complications.

Nonunion is one of the main complications associated with scaphoid fractures. Over the past decades, the use of arthroscopy in the treatment of scaphoid nonunion has proven to be a valuable technique, even in cases presenting deformity and carpal instability. This approach facilitates rapid consolidation while minimizing damage to the joint capsule and ligaments, thereby preserving the already compromised vascular supply of the scaphoid. Due to their simpler anatomy, dorsal portals are commonly employed in wrist arthroscopy, reducing the risk of injury to neurovascular structures during joint

Background: Cystic lesions of the carpal bones are rare entities that are infrequently reported in the literature. Scaphoid intraosseous cystic lesions represent a rare subset of carpal bone cysts. This review aims to summarize the available evidence on the evaluation and treatment of scaphoid cystic lesions to help guide clinical management. Methods: Systematic electronic searches were performed using PubMed, Ovid, and Embase databases. Studies included were graded for their risk of bias.
