Scorpion sting in children is a hazardous and potentially fatal condition. Of 34 children admitted to hospital in Mahad, Maharashtra State, India following scorpion sting, 14 had hypertension (130/90-170/130 mmHg), five had myocardial failure, acute pulmonary oedema developed in nine, two had tachycardia (110-200/min) and four died. Analysis of data suggests that cardiovascular morbidity and mortality depend upon the time lapse between sting and administration of vasodilators. Plasma immunoreactive cationic trypsin (ICT), which is a specific and highly sensitive indicator of pancreatic injury, was measured in 14 children with signs of systemic envenomation following a sting by the scorpion Leiurus quinquestriatus. High ICT levels were found in 13 children (93%), indicating that acute pancreatitis is a common complication of envenomation by this scorpion. The pancreatitis may account for the abdominal pain and vomiting commonly seen in scorpion envenomation and may also contribute to the agitation and discomfort noted in young children.

Phospholipase A2 was purified about 78 fold from the venom of scorpion Heterometrus fulvipes. The molecular weight of the enzyme was found to be 16,000 and it was optimally active at pH 7.4 and at 50 degrees C. The Km value of the enzyme was 1.8 x 10(-3) M. Calcium, Magnesium and Zinc ions stimulated whereas Mercury ion and EDTA inhibited the enzyme activity. This enzyme exhibited fluorescence emission maximum between 310-320 nm.

The echocardiographic and radionuclide angiographic abnormalities in children after scorpion envenomation with L. quinquestriatus, were evaluated. Five children were severely hypertensive, one of them in respiratory failure and another had pulmonary edema. The results revealed poor global contractility 12-15 hr after the sting in three patients. The radionuclide angiograms also revealed poor contractility with low ejection fraction. Factors involved in the pathophysiological changes such as severe pain, burning sensation, redness, swelling and edema in case of the scorpion L. laevifrons were investigated. The presence of pain-producing autacoids histamine 2.1 +/- 0.18 micrograms/mg and 5-HT 0.23 +/- 0.1 micrograms/ml was confirmed by thin layer chromatography and bioassay. Histamine releasing substance was detected in vitro in the chopped guineapig lung. Two monoclonal antibodies specific for the potent toxin II of the scorpion Androctonus australis Hector have been produced. One of them shows both high affinity binding to the toxin (Kd = 0.8 nM) and in vivo and in vitro neutralizing properties. Three toxins, i.e. toxins II and III of Androctonus australis Hector and toxin I of Buthus occitanus tunetanus, were labeled with 125I. High specific radioactivities were obtained (490-1100 Ci/mmole) that allowed the setting up of three radioimmunoassays. The scorpion venom Heterometrus fulvipes was able to uncouple the state-4 respiration of rat liver mitochondria. The venom caused mitochondrial swelling and was found to be inhibitory to mitochondrial succinate and glutamate dehydrogenases. Bovine serum albumin was able to protect against mitochondrial swelling.

C. sculpturatus sera agglutinate human erythrocytes independently of the ABO blood group, enzyme treatment, incubation temperature or sex of the scorpions. Tested with human lymphocytes and reptile and bird erythrocytes, C. sculpturatus serum reacts like an anti-sialic acid agglutinin. With leukemic lymphocytes, titers are higher than with normal lymphocytes.
