A novel m.12908T>a mutation in the mitochondrial ND5 gene in patient with infantile-onset Pompe disease.

Pompe disease is a progressive metabolic myopathy caused by deficiency in lysosomal acid α-glucosidase and results in cellular lysosomal and cytoplasmic glycogen accumulation. A wide spectrum of clinical phenotypes exists from hypotonia and severe cardiac hypertrophy in the first few months of life to a milder form with the onset of symptoms in adulthood. The disease is typically due to severe mutations in GAA gene. In the present study, we described a newborn boy with clinical features of Pompe disease particularly with hypertrophic cardiomyopathy, hypotonia and hepatomegaly. This case was at

Dosimetric analysis of post-radiotherapy sexual dysfunction in patients receiving definitive stereotactic body radiotherapy (SBRT) for prostate cancer: a secondary analysis of MIRAGE.

PURPOSE: The etiology of the improved long-term sexual function seen following MRI-guided stereotactic body radiotherapy (SBRT) with aggressive margin reduction for prostate cancer remains unknown. METHODS AND MATERIALS: To investigate potential explanatory mechanisms for this finding, we conducted a secondary analysis of data collected for the prospective, randomized phase III MIRAGE trial. Dosimetric data for the structures hypothesized to drive sexual function decline (neurovascular bundles (NVB) and internal pudendal arteries (IPA)) were retrospectively collected and analyzed for correlat

Parent injury admission as a potential adverse childhood experience: A 25 US Level I Trauma center investigation.

INTRODUCTION: Adverse Childhood Experiences (ACEs), such as violence exposure, are linked with numerous long-term health consequences. Adult firearm and other injury survivors presenting to level I trauma centers frequently report having youth family members exposed to firearm violence and other traumatic life events. Few investigations have examined the demographic and familial characteristics, or cumulative trauma burden of exposed family members. METHODS: The investigation was a secondary analysis of data collected as part of a 25-site national US level I trauma center randomized clinical

Utilizing depression symptom-based phenotypes to explore ketamine treatment response in major depression: The Bio-K multicenter trial.

BACKGROUND: Previous work has identified four correlated factors (depression phenotypes) using the Montgomery-Åsberg Depression Rating Scale (MADRS)- Sadness, Negative Thoughts, detachment/Interest and Activity (IA), and Neurovegetative. We explored treatment responses to ketamine based on these 4 phenotypes in treatment resistant depression (TRD). METHODS: This is a secondary analysis of data from an open-label multi-center, clinical trial (BioK) examining biomarkers of treatment response, that enrolled 75 patients (ages 18-65 years old) with treatment-resistant unipolar or bipolar depressio

Altered Connectome Topology in Newborns at Risk for Cognitive Developmental Delay: A Cross-Etiologic Study.

The human brain connectome is characterized by the duality of highly modular structure and efficient integration, supporting information processing. Newborns with congenital heart disease (CHD), prematurity, or spina bifida aperta (SBA) constitute a population at risk for altered brain development and developmental delay (DD). We hypothesize that, independent of etiology, alterations of connectomic organization reflect neural circuitry impairments in cognitive DD. Our study aim is to address this knowledge gap by using a multi-etiologic neonatal dataset to reveal potential commonalities and di

Comparison of Cervical Spine Injury Clinical Prediction Rules for Children After Blunt Trauma.

IMPORTANCE: Pediatric cervical spine injury (CSI) is a rare occurrence; however, CSI can result in significant disability and death. It is essential to determine the optimal CSI clinical prediction rule to risk stratify children with potential CSI after blunt trauma who require cervical spine imaging. OBJECTIVE: To compare the test characteristics and projected imaging rates between 3 prospectively derived CSI clinical prediction rules: the Pediatric Emergency Care Applied Research Network CSI prediction rule (PECARN CSI rule), the National Emergency X-Radiography Utilization Study (NEXUS), a

In Silico Prediction of the Effects of Mutations in the Human Mevalonate Kinase Gene: Towards a Predictive Framework for Mevalonate Kinase Deficiency.

Mevalonate kinase (MVK) catalyses the phosphorylation of mevalonate. Deficiency of MVK is associated with two rare periodic fever syndromes, mevalonic aciduria (MA), a severe form and hyper-immunoglobulin-D syndrome (HIDS), a milder form. An in silico approach was used to analyse the physicochemical and structural effects of 47 disease-associated variants of MVK. A further 20 variants, which are present in human genome databases, were also analysed. Variants associated with MA are clustered into a "hotspot" consisting of residues 8-35 and 234-338 and tended to result in a prediction of severel

Polyglutamine induced misfolding of huntingtin exon1 is modulated by the flanking sequences.

Polyglutamine (polyQ) expansion in exon1 (XN1) of the huntingtin protein is linked to Huntington's disease. When the number of glutamines exceeds a threshold of approximately 36-40 repeats, XN1 can readily form amyloid aggregates similar to those associated with disease. Many experiments suggest that misfolding of monomeric XN1 plays an important role in the length-dependent aggregation. Elucidating the misfolding of a XN1 monomer can help determine the molecular mechanism of XN1 aggregation and potentially help develop strategies to inhibit XN1 aggregation. The flanking sequences surrounding

Spontaneous breathing trials as predictors of extubation outcomes in neurocritical care: insights from the ENIO study.

PURPOSE: In critically ill patients, extubation readiness is typically assessed using a spontaneous breathing trial (SBT). Among patients with acute brain injury (ABI), the optimal SBT method remains uncertain. METHODS: We conducted a post-hoc analysis of the ENIO study (NCT03400904), including mechanically ventilated ABI patients with available SBT data, undergoing extubation attempt. SBTs were classified as T-piece, pressure support ventilation (PSV), or continuous positive airway pressure (CPAP). The primary outcome was extubation failure within 5 days. Associations between SBT modality an

Nasal biomarker inflammatory profile in response to intranasal corticosteroids in pediatric obstructive sleep apnea syndrome.

STUDY OBJECTIVES: Nasopharyngeal inflammation contributes to pediatric obstructive sleep apnea syndrome (OSAS). Intranasal corticosteroids (INCS) are used to treat pediatric OSAS; a randomized controlled trial showed an improvement in OSAS symptoms but without polysomnography or neurobehavioral outcome differences. There is a lack of data demonstrating an objective decrease in the nasal inflammatory biomarker profile associated with INCS. Hence, we evaluated the association of nasal inflammatory biomarker profile and response to INCS. METHODS: Secondary analysis of a randomized controlled tri

Prediction of Brugia malayi antigenic peptides: candidates for synthetic vaccine design against lymphatic filariasis.

Brugia malayi is a threadlike nematode cause's swelling of lymphatic organs, condition well known as lymphatic filariasis; till date no invention made to effectively address lymphatic filariasis. In this analysis we a have predicted suitable antigenic peptides from Brugia malayi antigen protein for peptide vaccine design against lymphatic filariasis based on cross protection phenomenon as, an ample immune response can be generated with a single protein subunit. We found MHC class II binding peptides of Brugia malayi antigen protein are important determinant against the diseased condition. The

Novel mutations in 3-phosphoglycerate dehydrogenase (PHGDH) are distributed throughout the protein and result in altered enzyme kinetics.

Three-phosphoglycerate dehydrogenase (3-PGDH) deficiency is a rare recessive inborn error in the biosynthesis of the amino acid L-serine characterized clinically by congenital microcephaly, psychomotor retardation, and intractable seizures. The biochemical abnormalities associated with this disorder are low concentrations of L-serine, D-serine, and glycine in cerebrospinal fluid (CSF). Only two missense mutations (p.V425M and p.V490M) have been identified in PHGDH, the gene encoding 3-PGDH, but it is currently unclear how these mutations in the carboxy-terminal regulatory domain of the protein

Plasma aldosterone is low in patients hospitalized with COVID-19 and not associated with changes in serum potassium levels: post hoc observational analyses of clinical trial data.

BACKGROUND: Hypokalemia is common in patients hospitalized with coronavirus disease 2019 (COVID-19) and is associated with mortality, disease progression and severity. Increased aldosterone levels were previously suggested to be the main cause of potassium loss in this population. We aimed to assess the effects of the latest morning plasma aldosterone levels on changes in serum potassium during severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. METHODS: We used dynamic generalized estimating equations (GEEs) on longitudinal data (3 weeks) from an adequately sized (159 pat
