Effect of a facility-based multifaceted intervention on the quality of obstetrical care: a cluster randomized controlled trial in Mali and Senegal.

BACKGROUND: Maternal mortality in referral hospitals in Mali and Senegal surpasses 1% of obstetrical admissions. Poor quality obstetrical care contributes to high maternal mortality; however, poor care is often linked to insufficient hospital resources. One promising method to improve obstetrical care is maternal death review. With a cluster randomized trial, we assessed whether an intervention, based on maternal death review, could improve obstetrical quality of care. METHODS: The trial began with a pre-intervention year (2007), followed by two years of intervention activities and a post-int…

Spermiogenesis and ultrastructure of the spermatozoon of Wardula capitellata (Digenea, Mesometridae), an intestinal parasite of the sparid teleost Sarpa salpa in Senegal.

The spermiogenesis process in Wardula capitellata begins with the formation of a differentiation zone containing two centrioles associated with striated rootlets and an intercentriolar body. Each centriole develops into a free flagellum orthogonal to a median cytoplasmic process. Later these flagella rotate and become parallel to the median cytoplasmic process, which already exhibits two electron-dense areas and spinelike bodies before its proximodistal fusion with the flagella. The final stage of the spermiogenesis is characterized by the constriction of the ring of arched membranes, giving r

Chewing lice (Phthiraptera) from wild birds in Senegal, with descriptions of three new species of the genera Brueelia and Philopteroides.

A total of 170 wild birds from Senegal, belonging to 48 species and 9 orders, were searched for lice in 2005 and 2007. Chewing lice were found on 58 birds of 18 species and 5 orders (Columbiformes, Cuculiformes, Coraciiformes, Galliformes and Passeriformes). Twenty-two species of chewing lice of 13 genera were determined. Other nine samples of chewing lice that represent a new host-parasite association were determined at generic level only, because only one sex or nymph of these lice were found. Our records represent the first louse records from passerines Camaroptera brachyura (Cisticolidae),…

A new Rickettsia species found in fleas collected from human dwellings and from domestic cats and dogs in Senegal.

The insects of the order Siphonaptera, commonly named fleas, are vectors of pathogens around the world. Our previous studies showed that 4.4% of acute febrile diseases in the Sine-Saloum region of Senegal were due to Rickettsia felis. The aim of this study was to explain the high prevalence of R. felis infections in two rural Senegalese populations by an entomological, systematic monitoring protocol. A total of 232 fleas from three species (Ctenocephalides felis, Echidnophaga gallinacea, and Synosternus pallidus) were collected by candle trapping and manually from pets in the villages of Dielm…

Ultrastructure of the spermatozoon of Bothriocotyle sp. (Cestoda: Bothriocephalidea), a parasite of Schedophilus velaini (Sauvage, 1879) (Perciformes: Centrolophidae) in Senegal.

The mature spermatozoon of Bothriocotyle sp. is filiform and tapered at both extremities. It possesses 2 axonemes of unequal length, showing the 9 + "1" pattern of Trepaxonemata. The anterior extremity exhibits a crest-like body. Thereafter, the crest-like body disappears, and the first axoneme is surrounded by a ring of cortical microtubules (about 27 units) that persist until the appearance of the second axoneme. This ring of cortical microtubules is characteristic only for species of Bothriocephalidea and represents a very useful phylogenetic character. The spermatozoon cytoplasm is slightl

Feasibility of onchocerciasis elimination with ivermectin treatment in endemic foci in Africa: first evidence from studies in Mali and Senegal.

BACKGROUND: Mass treatment with ivermectin is a proven strategy for controlling onchocerciasis as a public health problem, but it is not known if it can also interrupt transmission and eliminate the parasite in endemic foci in Africa where vectors are highly efficient. A longitudinal study was undertaken in three hyperendemic foci in Mali and Senegal with 15 to 17 years of annual or six-monthly ivermectin treatment in order to assess residual levels of infection and transmission and test whether ivermectin treatment could be safely stopped in the study areas. METHODOLOGY/PRINCIPAL FINDINGS: S…

Climate impacts on environmental risks evaluated from space: a conceptual approach to the case of Rift Valley Fever in Senegal.

BACKGROUND: Climate and environment vary across many spatio-temporal scales, including the concept of climate change, which impact on ecosystems, vector-borne diseases and public health worldwide. OBJECTIVES: To develop a conceptual approach by mapping climatic and environmental conditions from space and studying their linkages with Rift Valley Fever (RVF) epidemics in Senegal. DESIGN: Ponds in which mosquitoes could thrive were identified from remote sensing using high-resolution SPOT-5 satellite images. Additional data on pond dynamics and rainfall events (obtained from the Tropical Rainfa…

Spermiogenesis and ultrastructure of the spermatozoon of the liver fluke Fasciola gigantica Cobbold, 1856 (Digenea: Fasciolidae), a parasite of cattle in Senegal.

The present paper describes the spermiogenesis and the ultrastructure of the spermatozoon of Fasciola gigantica, as revealed by transmission electron microscopy. Spermiogenesis in F. gigantica begins with the formation of a differentiation zone containing 2 centrioles with associated striated roots and an intercentriolar body between them. Each centriole develops a flagellum. Proximodistal fusion of these flagella with the median cytoplasmic extension gives rise to the spermatozoon. Spermiogenesis in F. gigantica is characterized by the formation of a dorsolateral cytoplasmic expansion, an ext

[Mortality from snake bites, wild and domestic animal bites and arthropod stings in the savannah zone of eastern Senegal].

From 1976 to 1999, we conducted a prospective study of overall and cause-specific mortality among the population of 42 villages of south-eastern Senegal. Of 4,228 deaths registered during this period, 26 were brought on by snakebites, 4 by invertebrate stings and 8 by other wild or domestic animals. The average annual mortality rate from snakebite was 14 deaths per 100,000 population. Among persons aged 1 year or more, 0.9% (26/2,880) of deaths were caused by snakebite and this cause represented 28% (26/94) of the total number of deaths by accident. We also investigated the snake fauna of the …

Prevalence of malnutrition and vitamin A deficiency in the Diourbel, Fatick, and Kaolack regions of Senegal: feasibility of the method of impression cytology with transfer.

The evaluation of the prevalence of ophthalmological diseases and vitamin A deficiency in Senegalese preschool-aged children enabled us to confirm the method of impression cytology with transfer (ICT) and to assess the impact of ophthalmological diseases on the cytological appearance of conjunctival cells. A simplification of the ICT method consisted of transferring cells present on a strip of paper to a glass slide by finger pressure. Harris-Schorr staining of the sample papers confirmed the results obtained by the transfer. Inflammatory trachoma influenced the results of the test. This simpl…

Diagnostic challenges in inherited platelet disorders in sub-Saharan Africa: first clinical case study of seven patients in Senegal.

INTRODUCTION: Constitutional thrombopathies, also known as inherited platelet disorders (IPDs), represent a complex and heterogeneous group of bleeding disorders. These conditions are still poorly documented in resource-limited settings, particularly in sub-Saharan Africa. This study was initiated with the primary objective of confirming suspected cases of IPD in Senegal. METHODOLOGY: We conducted an observational study of all suspected cases of constitutional thrombopathy at the clinical hematology department in Dakar. Each patient provided written consent, and complete clinical files were o…

Acacia senegal gum attenuates systemic toxicity in CCl4-intoxicated rats via regulation of the ROS/NF-κB signaling pathway.

Acacia senegal (AS) gum (Gum Arabic) is a natural emulsifier exudate from the branches and trunk of Acacia trees and it is recognized by the Food and Drug Administration (FDA) agency as a secure dietary fiber. The present research evaluated the systemic oxidative and necroinflammatory stress induced by CCl4 administration and the alleviating effect of AS gum aqueous extract (ASE, 7.5 g/Kg b.w.). The results demonstrated the presence of certain phenolic compounds in ASE, as well as its in vitro potent scavenging ability against ABTS (2,2'-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid), NO, a…

Immunogenicity of seasonal inactivated influenza and inactivated polio vaccines among children in Senegal: Results from a cluster-randomized trial.

Data on influenza vaccine immunogenicity in children are limited from tropical developing countries. We recently reported significant, moderate effectiveness of a trivalent inactivated influenza vaccine (IIV) in a controlled, cluster-randomized trial in children in rural Senegal during 2009, a year of H3N2 vaccine mismatch (NCT00893906). We report immunogenicity of IIV3 and inactivated polio vaccine (IPV) from that trial. We evaluated hemagglutination inhibition (HAI) and polio antibody titers in response to vaccination of three age groups (6 through 35 months, 3 through 5 years, and 6 through…
