Diagnosis of temporomandibular joint pain in patients seen at a pain clinic.

A retrospective survey of a pain clinic population found 43 patients (2.6% of the total population) who had pain in the temporomandibular joint region. The clinical presentation of these patients did not differ greatly from that observed with typical responsive temporomandibular joint pain; however, important discriminating factors were duration of the pain, a strong history of pain elsewhere in the body, and major psychiatric disturbance. The diagnosis was atypical facial pain in 39 (90%) of the cases; other diagnoses included glossopharyngeal neuralgia and perineural invasion by adenocystic

Gender roles, illness orientation and use of medical services.

The study investigates illness orientation as a factor which may account for sex differences in the utilization of medical care. First, sex … ely to perceive symptoms than males, there is no apparent sex difference in a tendency to adopt the sick role when ill. In addition, results indicate that gender role factors such as level and type of role responsibility and concern with health are related to female though not male symptom reports. Illness orientation variables are related to rates of medic …

How to write a psychiatric consultation.

The written psychiatric consultation is the distillation, the official permanent record, and the one universal element of the consultation process. Both the document and process present a good and growing opportunity for service and teaching. The authors offer a conceptual and practical scheme to help potential consultants make decisions about the content, style, and wording of their written communications. Each of the components of the consultation document, including headings, openings, history, examination, and formulations, is considered in terms of its effects on the liaison with the cons

Intractable facial pain and illness behaviour.

Management of intractable facial pain (IFP) patients is time consuming and potentially frustrating. Earlier identification of IFP patients may be helped by considering such cases in terms of their illness behaviour rather than a large rang of other diagnostic labels. A group of 24 IFP patients, with diagnoses including temporomandibular joint dysfunction pain, atypical facial pain and facial neuralgia, completed a questionnaire designed to measure illness behaviour. Compared to a control group of patients with minor odontogenic pain, IFP patients were more somatically preoccupied, could not ac

From trypanosomes to the nervous system, from molecules to behavior: a survey, on the occasion of the 90th anniversary of Castellani's discovery of the parasites in sleeping sickness.

The observation of Trypanosomes in patients affected by sleeping sickness has been reported in 1903 by Aldo Castellani. On the occasion of the 90th anniversary of this discovery, we here present the findings recently obtained in and experimental model of African trypanosomiasis in the rat. The molecular and cellular mechanisms of the interplay between the parasite and the host have been largely clarified: a bidirectional signalling occurs between trypanosomes and CD8+ T cells of the host animal. This new pathogenetic mechanism of infection involves a lymphocyte triggering factor released by th

Impact of pulmonary African trypanosomes on the immunology and function of the lung.

Approximately 20% of sleeping sickness patients exhibit respiratory complications, however, with a largely unknown role of the parasite. Here we show that tsetse fly-transmitted Trypanosoma brucei parasites rapidly and permanently colonize the lungs and occupy the extravascular spaces surrounding the blood vessels of the alveoli and bronchi. They are present as nests of multiplying parasites exhibiting close interactions with collagen and active secretion of extracellular vesicles. The local immune response shows a substantial increase of monocytes, macrophages, dendritic cells and γδ and acti

Cytokine polymorphisms have a synergistic effect on severity of the acute sickness response to infection.

BACKGROUND: Functional polymorphisms in immune response genes are increasingly recognized as important contributors to the marked individual differences in susceptibility to and outcomes of infectious disease. The acute sickness response is a stereotypical set of illness manifestations mediated by the proinflammatory cytokines induced by many different pathogens. The genetic determinants of severity of the acute sickness response have not previously been explored. METHODS: We examined the impact of functional polymorphisms in cytokine genes with critical roles in the early immune response (tu

Atrial natriuretic peptide may inhibit motion sickness through reducing aldosterone-induced increase in endolymph volume of the inner ear.

Previous studies have suggested that aldosterone is potentially involved in the development of motion sickness and that atrial natriuretic peptide (ANP) may inhibit it. Therefore, the present study was conducted to investigate the inhibitory effects of ANP on aldosterone activity in motion sickness and on aldosterone-induced changes in endolymph volume in the inner ear. We found that rotatory stimulation induced an elevation in plasma aldosterone levels, intraperitoneal aldosterone injection caused motion sickness-like responses in both guinea pigs and mice, and ANP inhibited these responses.

Anterior insula morphology and vulnerability to psychopathology-related symptoms in response to acute inflammation.

INTRODUCTION: The role of inflammation in common psychiatric diseases is now well acknowledged. However, the factors and mechanisms underlying inter-individual variability in the vulnerability to develop psychopathology-related symptoms in response to inflammation are not well characterized. Herein, we aimed at investigating morphological brain regions central for interoception and emotion regulation, and if these are associated with acute inflammation-induced sickness and anxiety responses. METHODS: Systemic inflammation was induced using an intravenous injection of lipopolysaccharide (LPS)

Sensitized rotatory motion perception and increased susceptibility to motion sickness in vestibular migraine: A cross-sectional study.

BACKGROUND AND PURPOSE: Vestibular migraine (VM) patients are ictally and interictally hypersensitive for self-motion and visual perception. Increased cortical excitability of the vestibular system represented by lowered motion perception thresholds might play an important role in the pathophysiology of VM. We aimed to compare motion perception thresholds and the vegetative response to rotatory motion, as well as the vestibulo-ocular reflex (VOR) during rotation in VM patients compared to healthy controls (HC). METHODS: In this cross-sectional study, 28 female VM patients in the interictal st

We Remember… Elders' Memories and Perceptions of Sleeping Sickness Control Interventions in West Nile, Uganda.

The traditional role of African elders and their connection with the community make them important stakeholders in community-based disease control programmes. We explored elders' memories related to interventions against sleeping sickness to assess whether or not past interventions created any trauma which might hamper future control operations. Using a qualitative research framework, we conducted and analysed twenty-four in-depth interviews with Lugbara elders from north-western Uganda. Participants were selected from the villages inside and outside known historical sleeping sickness foci. El

Motion Sickness and Resting Energy Expenditure in Chinese Male Adults.

BACKGROUND: Motion sickness can influence energy homeostasis by enhancing thermolysis. This study tested the hypothesis that resting energy expenditure (REE), as the major component of thermogenesis, might also play a role during motion sickness. METHODS: The effect of seasickness on REE at sea was examined in 71 healthy Chinese male volunteers. Change in REE, heart rate variability (HRV), blood ghrelin levels, and leptin levels were observed across baseline, voyage, and recovery stages. Seasickness severity was assessed using the Graybiel motion sickness questionnaire (GMSQ), and the nausea

Patient requests for euthanasia and assisted suicide in terminal illness. The role of the psychiatrist.

Psychosocial assessment and treatment are critical elements of care for terminally ill patients who desire hastened death. Most patients, in saying that they want to die, are asking for assistance in living--for help in dealing with depression, anxiety about the future, grief, lack of control, dependence, physical suffering, and spiritual despair. In this article, the authors review current understandings of the psychiatric aspects of requests by terminally ill patients for assisted suicide and euthanasia; describe an approach to the common problems of physical, psychological, social, and spir

Effect of cleaning and disinfection of toys on infectious diseases and micro-organisms in daycare nurseries.

BACKGROUND: The rising number of children in daycare nurseries increases opportunities for the transmission of infectious diseases. Pathogens may be transmitted directly from child to child via sneezing, coughing and touching, or indirectly via the environment. Toys are among the fomites with the highest pathogen load, but their role in disease transmission is unknown. AIM: To determine if washing and disinfection of toys can reduce sickness absence and microbial pathogen load in the nursery environment. METHODS: Twelve nurseries (caring for 587 children) were randomized to intervention and

Maternal susceptibility to nausea and vomiting of pregnancy: is the vestibular system involved?

Nausea and vomiting of pregnancy shares many characteristics with motion sickness, a vestibular dependent phenomenon. A number of physiologic changes that occur in normal pregnancy are also known to accompany nausea and vomiting in patients with motion sickness and certain vestibular disorders. This chapter summarizes some shared features of both phenomena. The unmasking of subclinical vestibular disorders may account for some cases of hyperemesis gravidarum. Hormonal effects on neurotransmitter function may also play a role in nausea and vomiting of pregnancy and in some vestibular disorders;

Mechanisms of selective attention and space motion sickness.

The neural mismatch theory of space motion sickness asserts that the central and peripheral autonomic sequelae of discordant sensory input arise from central integrative processes falling to reconcile patterns of incoming sensory information with existing memory. Stated differently, perceived novelty reaches a stress level as integrative mechanisms fail to return a sense of control to the individual in the new environment. Based on evidence summarized here, the severity of the neural mismatch may be dependent upon the relative amount of attention selectively afforded to each sensory input comp

[An experimental study of the contact transmission of the Marburg virus].

Experiments in guinea pigs and M. rhesus monkeys showed the possibility of contact infection of animals with Marburg virus. Secondary infection occurred most intensively when the monkeys were kept together but was also shown to be possible when the animals were separated but placed in the direction of the air flow from the sick monkeys as well as by "nose-to-nose" contact excluding the alimentary mode of transmission and the role of the agent excreted in the urine.

Drug resistance in African trypanosomiasis: the melarsoprol and pentamidine story.

Melarsoprol and pentamidine represent the two main classes of drugs, the arsenicals and diamidines, historically used to treat the diseases caused by African trypanosomes: sleeping sickness in humans and Nagana in livestock. Cross-resistance to these drugs was first observed over 60 years ago and remains the only example of cross-resistance among sleeping sickness therapies. A Trypanosoma brucei adenosine transporter is well known for its role in the uptake of both drugs. More recently, aquaglyceroporin 2 (AQP2) loss of function was linked to melarsoprol-pentamidine cross-resistance. AQP2, a c

New discoveries in the transmission biology of sleeping sickness parasites: applying the basics.

The sleeping sickness parasite, Trypanosoma brucei, must differentiate in response to the changing environments that it encounters during its complex life cycle. One developmental form, the bloodstream stumpy stage, plays an important role in infection dynamics and transmission of the parasite. Recent advances have shed light on the molecular mechanisms by which these stumpy forms differentiate as they are transmitted from the mammalian host to the insect vector of sleeping sickness, tsetse flies. These molecular advances now provide improved experimental tools for the study of stumpy formatio
