Endoscopic endonasal resection of skull base chondrosarcomas: technique and early results.

OBJECT: The authors of this study sought to report the technique and early clinical outcomes of a purely endonasal endoscopic approach for resection of petroclival chondrosarcomas. METHODS: Between 2010 and 2014, 8 patients (4 men and 4 women) underwent endonasal endoscopic operations to resect petroclival chondrosarcomas at 2 institutions. The patients' mean age was 44.8 years (range 30-64 years). One of the patients had previously undergone radiation therapy and another a staged craniotomy. Using volumetric software, an independent neuroradiologist assessed the extent of the resections on M

Enchondroma of the Skull Base in a Case of Ollier's Syndrome.

Ollier’s syndrome, a variant of multiple enchondromatosis, is a rare disease with an estimated prevalence of 1/100,000, characterized by multiple enchondromas, asymmetrically involving small bones of the hands and feet, especially the proximal phalanges. Intracranial enchondromas, such as those arising from the skull base are extremely rare. Herein, we report a 25-year-old female, known case of Ollier’s disease, presenting with right eyelid ptosis and visual disturbance. Brain MRI revealed a skull base tumour suspicious to enchondroma followed by trans-sphenoidal resection. Histologic examinat…

Advantages and limitations of endoscopic endonasal approaches to the skull base.

BACKGROUND: The anatomy of the skull base is extremely complex with an abundance of critical neurovascular bundles and their corresponding foramina as well as the insertions and origins of multiple masticatory and craniocervical muscles. These anatomic intricacies increase the difficulty of surgery within this area. METHODS: Advantages and disadvantages of endoscopic endonasal approaches (EEAs) based on the authors' sequential learning and experience are described. RESULTS: EEAs offer the advantages of using preexistent air spaces that enable accessing various areas of the skull base, while …

Clival keloid after nasopharyngeal radium irradiation masquerading as skull base malignancy.

Nonchordomatous lesions of the clivus are rare entities. Nasopharyngeal radium irradiation (NRI) treatment consists of application of radium to the posterior nasopharynx near the orifice of the Eustachian tubes, an area adjacent to the clivus. Here we present a unique case of a patient with a history of NRI 70 years prior to presentation with a space-occupying clival lesion suspicion for a skull-based malignancy. This lesion was resected using an endoscopic endonasal approach. Histopathological analysis revealed a clival keloid, an entity not previously reported in the literature.

Multi-detector-row computed tomography imaging of variant skull base foramina.

Several inconstant skull base foramina can be observed on multi-detector-row computed tomographic studies. These include the meningo-orbital foramen, foramen of Vesalius, canaliculus innominatus, palatovaginal canal, persistent craniopharyngeal canal, transsphenoidal canal, canalis basilaris medianus, and fossa navicularis. Although many of these foramina are simply incidental findings, there may be associated anomalies and important clinical implications. The multi-detector-row computed tomographic features of variant skull base foramina are depicted and discussed in this article.

Eyebrow craniotomy for anterior skull base lesions: how I do it.

BACKGROUND: The eyebrow craniotomy is a less invasive alternative approach for accessing anterior skull base lesions, compared to traditional and more extensive exposures. We give a stepwise description of this minimally invasive technique with discussion on the indications, limitations and key aspects of perioperative management. METHODS: Positioning of the head and planning the surgical corridor are dictated by the nature, site and size of the target lesion. The eyebrow incision should spare the medial and posterolateral neural structures. Microsurgical strategy is based on opening up the b…
