Bone replacement in head and neck surgery: a biocompatible alternative.

BACKGROUND: Defects in the skull have presented difficult reconstructive problems. Recently glass-ionomer cement and preformed. implants have been used to repair bony defects in the skull base and in cranio-facial surgery. Three patients are reported to illustrate possible applications for this new material. METHODS: Ionocap cement and Ionoroc-skull standardized implants have been used. The cement is available as a two component blister pack. When mixed and blended a gel is produced which can be worked for approximately 5 min before hardening. It can then be contoured and drilled to the requi

Dural Closure in Confined Spaces of the Skull Base with Nonpenetrating Titanium Clips.

BACKGROUND: Dural repair in areas with limited operative maneuverability has long been a challenge in skull base surgery. Without adequate dural closure, postoperative complications, including cerebrospinal fluid (CSF) leak and infection, can occur. OBJECTIVE: To show a novel method by which nonpenetrating, nonmagnetic titanium microclips can be used to repair dural defects in areas with limited operative access along the skull base. METHODS: We reviewed 53 consecutive surgical patients in whom a dural repair technique utilizing titanium microclips was performed from 2013 to 2016 at our inst

Parasellar Meningiomas.

Parasellar spaces remain particularly singular, comprising the most important neurovascular structures such as the internal carotid artery and optic, oculomotor, and trigeminal nerves. Meningiomas are one of the most frequent tumors arising from parasellar spaces. In this location, meningiomas remain mostly benign tumors with WHO grade I and a meningothelial subtype. Progestin intake should be investigated and leads mostly to conservative strategies. In the case of benign nonsymptomatic tumors, observation should be proposed. Tumor growth will lead to the proposition of surgery or radiosurgery

[Brain metastasis from rectal adenocarcinoma: about a case and review of the literature].

The most common primary sites for bone metastases in men are lung, prostate, kidney, thyroid or bladder. Colorectal origin is rare. Few studies have described this type of metastases; the axial skeleton or the pelvis are the most common metastasis locations. Craniofacial location is exceptional. We here report the case of a 38 years old man treated for metastatic rectal cancer metastasized to temporal bone. He initially had undergone surgical procedure for low anterior resection, tumor was classified as pT3N0M0; 24 months after the patient had left exophthalmos revealing a temporal tumoral pro

Endonasal endoscopic transsphenoidal excision of tuberculum sellae meningiomas: a systematic review.

INTRODUCTION: The endonasal endoscopic approach (EEA) for the resection of tuberculum sellae meningiomas (TSMs) has, more recently, been advocated as an alternative approach to deal with this challenging tumor. The aim of this study was to conduct a systematic review of publications of TSMs excised through the transsphenoidal route in the past 10 years and review data on the extent of excision, visual outcomes and complication rates. EVIDENCE ACQUISITION: We performed a thorough systematic review of the medical literature following the PRISMA guidelines. A medical librarian retrieved a list o

Nasopharyngeal carcinoma with skull base invasion and hydrocephalus: a case report.

We describe a case of nasopharyngeal carcinoma (NPC) with diffuse skull base bony destruction, resulting in obstructive hydrocephalus. Previously, these lesions were reported to be NPC with brain metastasis, without diffuse bony destruction of the skull base or hydrocephalus. We found no other cases of NPC with brain metastasis and diffuse bony destruction of the skull base resulting in hydrocephalus in the literature.

Diagnostic challenges in Burkitt's lymphoma with skull base and paravertebral involvement, and hearing loss: a case report.

BACKGROUND: Burkitt's lymphoma is a highly aggressive B cell non-Hodgkin lymphoma that typically presents with abdominal tumors or jaw involvement, depending on the variant. Involvement of the skull base or paravertebral regions and associated symptoms such as hearing loss are exceedingly rare in Burkitt's lymphoma. This case highlights the diagnostic challenges when Burkitt's lymphoma presents with such uncommon manifestations in an adult patient. CASE PRESENTATION: We report a case of a 36-year-old South Asian male individual who presented with generalized body pain, fever, headache, and ep

The different concepts of surgical managements between anterior and lateral skull base reconstructions based on surgical purposes.

OBJECTIVE: Anterior and middle cranial fossa defects require different strategies, depending on their anatomical location. The aim of this study was to elucidate the surgical managements for the defects based on surgical purposes and to clarify the conceptional differences relating to anterior and lateral skull base reconstructions. METHODS: This study included patients who had undergone reconstructive surgery for anterior or middle cranial fossa defects in our institution between July 2008 and June 2019. It consisted of 66 patients who had been subject to anterior skull base reconstructions,

Skull base repair following endonasal pituitary and skull base tumour resection: a systematic review.

PURPOSE: Postoperative cerebrospinal fluid rhinorrhoea (CSFR) remains a frequent complication of endonasal approaches to pituitary and skull base tumours. Watertight skull base reconstruction is important in preventing CSFR. We sought to systematically review the current literature of available skull base repair techniques. METHODS: Pubmed and Embase databases were searched for studies (2000-2020) that (a) reported on the endonasal resection of pituitary and skull base tumours, (b) focussed on skull base repair techniques and/or postoperative CSFR risk factors, and (c) included CSFR data. Reconstruction of skull base bone defects using an in situ bone flap after endoscopic endonasal transplanum-transtuberculum approaches.

PURPOSE: The creation of bone flaps which can be later repositioned and fixed in situ for repairing the bone defects during the endoscopic endonasal approaches (EEAs)-similar to transcranial craniotomies-is still a challenge. We present an in situ bone flap (ISBF) closure for the repair of bone defects after endoscopic endonasal transplanum-transtuberculum approaches (EETAs). METHODS: A retrospective analysis of consecutive patients who underwent the EETAs between January 2016 and February 2019 was performed. According to whether or not to use ISBF for skull base reconstruction, these patient

Outcome analysis of the transglabellar/subcranial approach for lesions of the anterior cranial fossa: a comparison with the classic craniotomy approach.

The classic approach to anterior skull base lesions uses bifrontal craniotomies together with lateral rhinotomies. This approach requires frontal lobe retraction and is associated with postoperative anosmia and the development of frontal lobe encephalomalacia. The transglabellar/subcranial approach permits removal of anterior skull base lesions without frontal lobe retraction and avoids facial scars. No studies to date, however, have directly compared the two approaches in terms of patient morbidity. The present retrospective study compares the two approaches when used for the removal of anter

[A craniofacial approach to disseminated tumors of the anterior portions of the cranial base].

The authors have developed a modified craniofacial approach. Its essence is to extend a facial cut along the mid-forehead after Weber-Fergusson into the frontoparietotemporal region, by forming a common soft tissue craniofacial graft. The above access was used in 17 patients with benign tumors of the base of the skull, involved into the ethmoidal sinus, anterior cranial fossa, medial portions of one or both eye sockets, sphenoidal sinus, medial portions of one or both maxillary sinuses, nasal cavity, and nasopharynx. The use of this access increases the radical rate of removal of extended cran

Plasmacytoma of the clivus presenting with an unusual combination of symptoms: case report.

A case of plasmacytoma of the clivus is presented. The patient was asymptomatic until a coincidental traumatic lesion produced manifestations of vagus nerve dysfunction. Conduction was blocked in some fibers of the vagus nerve while, at the same time, episodes of activity were triggered in fibers of the same nerve trunk, resulting in an unusual combination of symptoms. The symptomatology of the case is discussed.

[A case of atypical bone metastasis from breast carcinoma].

Clinical and radiographical data are presented for a case of suspected pathological fracture of the right ulna in a 66-yr-old women mastectomised two years previously. Multiple skeletal lesions were noted, often morphologically different from those usually observed in metastasis of breast cancer, with a fibrous and cystic appearance reminiscent of von Recklinhausen's disease. However, indisputable serohumoral data, the histological picture on biopsy, and repair of bone as a result of immuno-antiblastic management showed that true metastases were involved. The radiographical picture and the res

Reconstruction of Skull Base Defects.

"Endoscopic endonasal skull base surgery has dramatically changed and expanded over recent years due to significant advancements in instrumentation, techniques, and anatomic understanding. With these advances, the need for more robust skull base reconstructive techniques was vital. In this article, reconstructive options ranging from acellular grafts to vascular flaps are described, including the strengths, weaknesses, and common uses."

Which Routes for Petroclival Tumors? A Comparison Between the Anterior Expanded Endoscopic Endonasal Approach and Lateral or Posterior Routes.

OBJECTIVE: Petroclival tumors remain a surgical challenge. Classically, the retrosigmoid approach (RSA) has long been used to reach such tumors, whereas the anterior petrosectomy (AP) has been proposed to avoid crossing cranial nerves. More recently, the endoscopic endonasal approach has been "expanded" (i.e., EEEA) to the petroclival region. We aimed to compare these 3 approaches to help in the surgical management of petroclival tumors. METHODS: Petroclival approaches were performed on 5 specimens after they were prepared with formaldehyde colored via latex injection. RESULTS: The EEEA prov
