Neurobehavioral and autonomic alterations in adults with obstructive sleep apnea.

OBJECTIVE: Obstructive sleep apnea (OSA) is associated with sympathetic hyperactivity, excessive nocturnal sweating, sleepiness, and neurobehavioral cognitive alterations. However, it is not well known if cognitive consequences of OSA are independent from autonomic alterations. Thus, we assessed the association between polysomnographic, autonomic, and cognitive tests performance in OSA patients. METHODS: Fifty eight OSA patients (53 male) were administered with questionnaires assessing demographic, Epworth, Beck Depression Inventory, Syndrom Kurz test (SKT), Trail Making part B (TMT-B), and F

Salivary biomarkers indicate obstructive sleep apnea patients with cardiovascular diseases.

Although obstructive sleep apnea (OSA) patients are at high risk of developing cardiovascular disease (CVD), only a small proportion is currently diagnosed. To explore and identify the differentially expressed proteins/peptides of OSA patients with CVDs, a mass spectrometry-based salivary analysis was performed. In our study, eleven peaks were observed differentially expressed in saliva from the non-CVD and CVD groups. Five masses mass peaks (1594.1, 1673.7, 1196.6, 1290.5, and 1447.0 Da) showed an upregulated trend in the CVD group, whereas six mass peaks (3038.6, 2164.3, 2301.4, 3195.0, 2628

Obstructive sleep apnea and type 2 diabetes in older adults.

Both obstructive sleep apnea (OSA) and type 2 diabetes mellitus are commonly seen in older adults. Over the last decade, there has been increasing recognition that OSA is highly prevalent in persons with type 2 diabetes and related metabolic conditions such as insulin resistance and glucose intolerance. Intermittent hypoxemia and recurrent arousals in OSA trigger a repertoire of pathophysiological events, which can in turn alter glucose homeostasis and possibly increase the risk for type 2 diabetes. Conversely, there is evidence that type 2 diabetes may alter the progression and expression of

Localized cortical thinning in patients with obstructive sleep apnea syndrome.

STUDY OBJECTIVES: To investigate differences in cortical thickness in patients with obstructive sleep apnea (OSA) syndrome and healthy controls. DESIGN: Cortical thickness was measured using a three-dimensional surface-based method that enabled more accurate measurement in deep sulci and localized regional mapping. SETTING: University hospital. PATIENTS: Thirty-eight male patients with severe OSA (mean apnea-hypopnea index > 30/h) and 36 age-matched male healthy controls were enrolled. INTERVENTIONS: Cortical thickness was obtained at 81,924 vertices across the entire brain by reconstructi

Causes of sudden death in patients with obstructive sleep apnea.

There are few autopsy studies of patients dying suddenly with obstructive sleep apnea (OSA). Twenty-five forensic autopsies of unexpected sudden death in individuals with OSA were reviewed. The causes of death were as follows: cardiomyopathy (n = 11); sudden unexpected death without morphologic findings (SUDNA, n = 6); and other cardiovascular diseases not related to OSA (n = 8). The cardiomyopathy group comprised five hearts with concentric left ventricular hypertrophy without dilatation and six with left ventricular diameter >4 cm (dilated cardiomyopathy). Four of six hearts in the SUDNA gro

Adenotonsillectomy for obstructive sleep apnea in children with syndromic craniosynostosis.

BACKGROUND: The role of adenotonsillectomy in the treatment of obstructive sleep apnea in patients with craniosynostosis is not well established. METHODS: A retrospective review was conducted of all children with syndromic craniosynostosis (Apert, Crouzon, and Pfeiffer syndromes) seen at Boston Children's Hospital from January 2001 through April 2011. The primary outcome measure was the apnea/hypopnea index before and after adenotonsillectomy. RESULTS: There were 47 patients (66 percent Apert syndrome, 19 percent Pfeiffer syndrome, and 15 percent Crouzon syndrome) less than 21 years of age (

Temporomandibular disorder pain and dental treatment of obstructive sleep apnea.

Treatment of sleep apnea with mandibular advancement devices (MADs) may be associated with the development of symptoms of temporomandibular disorder (TMD). This article discusses the different types of TMD and orofacial pain problems that may occur during treatment of obstructive sleep apnea (OSA) with a MAD. It is critical that the general dentist who is providing dental devices for OSA perform a thorough physical and neurologic assessment of the temporomandibular joint and associated structures before providing such a device so that preexisting problems are identified and discussed with the

Elevated fractalkine in patients with obstructive sleep apnea hypopnea syndrome.

BACKGROUND: Obstructive sleep apnea hypopnea syndrome (OSAHS) has been increasingly linked to cardiovascular disease. Inflammatory processes associated with OSAHS may contribute to atherosclerosis in these patients. Fractalkine is a unique chemokine which has both adhesive and chemoattractant functions. We tested the hypothesis that OSAHS patients have increased fractalkine. METHODS AND RESULTS: We studied 20 patients (18 males and 2 females) with newly diagnosed OSAHS, who were free of other diseases, had never been treated for OSAHS, and were taking no medications. We compared fractalkine m

Bispectral analysis of tracheal breath sounds for Obstructive Sleep Apnea.

Obstructive Sleep Apnea (OSA) is a respiratory disorder with serious consequences that is characterized by repetitive cessation of breathing for more than 10s often associated with a drop of more than 4% in the blood's Oxygen saturation level. The gold standard for OSA diagnosis is full-night Polysomnography (PSG), which is a time-consuming, inconvenient, and costly assessment. On the other hand, our team has showed that the analysis of tracheal respiratory sounds recorded during wakefulness holds promises to be used as a simple and effective tool for screening moderate and severe OSA. In this

Polysomnographic characteristics of daytime sleepiness in obstructive sleep apnea syndrome.

PURPOSE: The study is intended to explore the nocturnal sleep determinants for excessive daytime sleepiness (EDS) in patients with obstructive sleep apnea syndrome (OSAS). METHODS: Consecutive patients (n = 182) who had an apnea/hypopnea index (AHI) greater than 5 times per hour were used in this study. EDS (n = 32) was considered present whenever the Epworth sleepiness scale (ESS) score was >10 and the multiple sleep latency test (MSLT) score was <5 min. Absence of EDS (no EDS, n = 48) was determined in patients with an ESS score of <10 and a MSLT score of >10 min. RESULTS: Compared to no E

Assessment of obstructive sleep apnea and its severity during wakefulness.

In this article, a novel technique for assessment of obstructive sleep apnea (OSA) during wakefulness is proposed; the technique is based on tracheal breath sound analysis of normal breathing in upright sitting and supine body positions. We recorded tracheal breath sounds of 17 non-apneic individuals and 35 people with various degrees of severity of OSA in supine and upright sitting positions during both nose and mouth breathing at medium flow rate. We calculated the power spectrum, Kurtosis, and Katz fractal dimensions of the recorded signals and used the one-way analysis of variance to selec

Increased load on the respiratory muscles in obstructive sleep apnea.

We wished to quantify, in patients with obstructive sleep apnoea (OSA), the activity of the respiratory muscles in relation to upper airway occlusion and patency in sleep. We hypothesized that particular levels of neuromuscular activation are directly associated with upper airway patency. 21 patients with previously diagnosed OSA and 21 healthy control subjects underwent respiratory muscle testing and polysomnography. Neural respiratory drive, as measured by the electromyogram of the diaphragm (EMG(di)) was elevated in the obese OSA patients, awake and supine (13.1(5.6)%max), compared to norma

Association between coronary spastic angina pectoris and obstructive sleep apnea.

BACKGROUND: Previous studies have shown a higher prevalence of obstructive sleep apnea (OSA) in patients with coronary artery disease than in control subjects. However, the association between coronary spastic angina pectoris (CSA) and OSA has not been investigated. Accordingly, we examined this association. METHODS AND RESULTS: We performed overnight polysomnography in 42 consecutive patients with CSA confirmed by an acetylcholine or ergonovine provocation test and in 20 age- and gender-matched control subjects. OSA was classified into the following three groups: mild OSA (5≤apnea-hypopnea i

Obstructive sleep apnea and metabolic dysfunction in polycystic ovary syndrome.

Obstructive sleep apnea (OSA) is an underrecognized, yet significant factor in the pathogenesis of metabolic derangements in polycystic ovary syndrome (PCOS). Recent findings suggest that there may be two "subtypes" of PCOS, i.e. PCOS with or without OSA, and these two subtypes may be associated with distinct metabolic and endocrine alterations. PCOS women with OSA may be at much higher risk for diabetes and cardiovascular disease than PCOS women without OSA and may benefit from therapeutic interventions targeted to decrease the severity of OSA. The present chapter will review what is currentl
