Is alexithymia associated with specific mental disorders?

BACKGROUND: Alexithymia is characterized by restrictions in the perception, differentiation and regulation of affects. It is considered to be an important vulnerability factor for the development of mental disorders. Little is known, however, of whether alexithymia is associated with specific mental disorders. SAMPLING AND METHODS: Data from 1,461 patients of an outpatient clinic for psychosomatic medicine with various mental disorders (depressive disorders, anxiety disorders, adjustment disorders, somatoform disorders, eating disorders, and psychological and behavioral factors of physical il…

The "Biology-First" Hypothesis: Functional disorders may begin and end with biology-A scoping review.

While it is generally accepted that gastrointestinal infections can cause functional disturbances in the upper and lower gastrointestinal tract-known as postinfectious irritable bowel syndrome (PI-IBS) and functional dyspepsia (PI-FD)-it has still not been widely recognized that such an infection can also initiate functional non-intestinal diseases, and that non-intestinal infections can provoke both intestinal and non-intestinal functional disturbances. We conducted a scoping review of the respective literature and-on the basis of these data-hypothesize that medically unexplained functional s

Conversion visual loss: a differential diagnosis in infant amblyopia.

PURPOSE: To report the first case of amblyopia due to a conversion disorder in a child. METHODS: A 9-year-old girl without allergies or family or personal antecedents of interest presented to our clinic. She manifested a progressive visual loss after an episode of lumbar pain. This progressive loss led the patient in a 6-month period to legal bilateral blindness: visual acuity inferior to 2/20 in both eyes and severe constriction of both visual fields. RESULTS: Several pathologic processes were discarded by means of multiple explorations and a general multidisciplinary analysis: physical pat

Trauma and dissociation in conversion disorder and chronic pelvic pain.

OBJECTIVE: The purpose of this study was to: a) assess the link between sexual and/or physical abuse and dissociation in conversion disorder and chronic pelvic pain patients; and b) assess whether this effect is independent of level of general psychopathology. METHOD: This report examines data from four separate samples. Fifty-two patients with chronic pelvic pain, 61 patients with non-epileptic seizures, and two samples of patients (102 and 54) with predominantly motor or sensory types of conversion disorder were studied. RESULTS: Using point-biserial correlations no compelling evidence for

The network structure of major depressive disorder, generalized anxiety disorder and somatic symptomatology.

BACKGROUND: Major depressive disorder (MDD) and generalized anxiety disorder (GAD) often co-occur with somatic symptomatology. Little is known about the contributions of individual symptoms to this association and more insight into their relationships could help to identify symptoms that are central in the processes behind the co-occurrence. This study explores associations between individual MDD/GAD symptoms and somatic symptoms by using the network approach. METHOD: MDD/GAD symptoms were assessed in 2704 participants (mean age 41.7 years, 66.1% female) from the Netherlands Study of Depressi

Annual research review: functional somatic symptoms and associated anxiety and depression--developmental psychopathology in pediatric practice.

BACKGROUND: Medically unexplained physical symptoms, commonly referred to as functional somatic symptoms (FSS), are common in pediatric medical settings and associated with suffering, impairment, and medical help seeking. The association of pediatric FSS with anxiety and depressive symptoms and disorders across the life span is reviewed. METHOD: Review and critique of controlled studies examining cross-sectional and longitudinal associations of FSS with anxiety and depressive symptoms and disorders in community-based and clinical samples of children and adolescents. RESULTS: FSS are consiste

Psychiatric conditions in cosmetic surgery patients.

Beauty is important. As psychiatrists, we see the interface of beauty with mental health, self-esteem, and mental illness. As physicians who enhance cosmetic appearance, you encounter a broad spectrum of patients ranging from those with a healthy pursuit of enhanced appearance to those whose behavior is extremely maladaptive. This article provides some examples of unhealthy pursuit and how to recognize patients who may be inappropriate for cosmetic procedures. Patients with body dysmorphic disorder and narcissistic and histrionic personality disorders are suffering from psychiatric illnesses t

[The psychopathology of a depressive somato-autonomic symptom complex].

The investigation is based on the hypothesis about psychopathological heterogeneity of the somatovegetative (biological) symptom complex of depression. To testify this statement the results of the analysis of the somatovegetative component of depressive syndrome in randomized selection (20 adult patients) were presented. Somatovegetative symptoms (disorders of sleep and appetite mainly) defined the pattern of debut and preceded manifestations of other signs of affective disorder in this group. Psychopathological qualification of disorders belonging to such symptom complex was presented as the

Psychogenic cough treated with biofeedback and psychotherapy. A review and case report.

Psychogenic cough is a barking or honking cough, which is persistent and disruptive to normal activity. The cough may be a debilitating condition that interferes with work and social relationships. Although the frequency of this condition is low, it is not rare. The majority of cases reported involve pediatric or adolescent patients. Surprisingly, there are scant data describing this condition in the adult population and no reports of biofeedback being used to treat this syndrome. We present a case report of an adult patient with psychogenic cough and review the available pediatric and adult l

Adverse reactions to foods: relevance to psychiatric disorders.

There is some evidence in the literature that adverse reactions to foods can contribute to behavioral dysfunction. In view of what is known about food and behavior, this suggestion is entirely plausible. However, clear demonstrations of adverse behavioral reactions to foods have not been reported. The major methodologic problems are those of defining suitable baseline conditions and dealing with large ranges in dose, time course, and severity of reactions among putatively food-sensitive individuals. These problems, although large, are surmountable. Another problem for studies in this area is n

All-cause and cause-specific mortality in people with mental disorders and intellectual disabilities, before and during the COVID-19 pandemic: cohort study.

BACKGROUND: People with mental disorders and intellectual disabilities experience excess mortality compared with the general population. The impact of COVID-19 on exacerbating this, and in widening ethnic inequalities, is unclear. METHODS: Prospective data (N=167,122) from a large mental healthcare provider in London, UK, with deaths from 2019 to 2020, used to assess age- and gender-standardised mortality ratios (SMRs) across nine psychiatric conditions (schizophrenia-spectrum disorders, affective disorders, somatoform/ neurotic disorders, personality disorders, learning disabilities, eating

[Psychosomatic dermatology - state of the art].

Based on the current literature, the paper gives an overview of the state of knowledge in psychosomatic dermatology. The authors propose the following classification: (1.) Dermatologic problems as a consequence of psychic disorders (e.g. factitious disorder, trichotillomania, dysmorphophobia, delusions of parasitosis etc.); (2.) Multifactorial dermatologic disorders, which can be substantially influenced by psychological factors (e.g. atopic dermatitis, psoriasis, chronic urticaria etc.); (3.) Psychic disorders developing as a consequence of disfiguring or life-threatening dermatoses (e.g. adj

Alprazolam-induced hypomania.

OBJECTIVE: There are reports of alprazolam-induced hypomania/mania. Here is a case report of a patient who developed hypomania during treatment with alprazolam, but not with diazepam, another benzodiazepine derivative. CLINICAL PICTURE: The illness was of 2 months' duration and the patient received a diagnosis of anxious depression. Following treatment with alprazolam, the patient developed hypomania characterised by euphoria, overactivity, overtalkactivity, racing thoughts, oversocialisation, enhanced self-confidence and disturbed sleep. TREATMENT: Hypomania subsided when alprazolam was wit

A synthesis of meta-analyses of mindfulness-based interventions in pain.

Mindfulness interventions have become popular in recent decades, with many trials, systematic reviews, and meta-analyses of the impact of mindfulness-based interventions (MBIs) on pain. Although many meta-analyses provide support for MBIs, the results are more mixed than they at first appear. The aim of this umbrella review was to determine the strength of evidence for MBIs by synthesizing available meta-analyses in pain. We conducted a systematic search in 5 databases and extracted data from published meta-analyses as the unit of analysis. For each outcome, we reported the range of effect siz

Associations of Psychologic Factors with Multiple Chronic Overlapping Pain Conditions.

AIMS: To characterize psychologic functioning across five chronic overlapping pain conditions (COPCs)-temporomandibular disorders, fibromyalgia, low back pain, headache, and irritable bowel syndrome-and their overlaps. METHODS: Participants were 655 adults in the OPPERA study. Psychologic variables were standardized in separate logistic regression models to compare their relative strength of association with each COPC. Random forest regression was used to explore the association of all psychologic measures with COPCs simultaneously. Linear regression analyses examined whether the count of COP
