On the suggestion of a Chinese physician the author took up Tai Chi, a traditional Chinese exercise, in an attempt to relieve symptoms from his moderately severe ankylosing spondylitis. Conventional medical therapy, used over a 15 year period, had proven of only limited benefit. Tai Chi consists of a series of intricate exercise sequences, and after 2 1/2 years of daily practice the author now feels stronger and healthier than before. This case report describes a patient who suffered from backache at the age of 12 years. The initial diagnosis was Scheuermann's disease but repeated examinations showed a progressive discitis, without signs of arthritis of the sacro-iliac joints. There was also a recurring effusion of the left knee. Three years later the clinical and radiological evaluation showed irreversible immobilisation of the dorsolumbar spine, whereas the sacro-iliac joints were mildly affected. Discitis seems to be a rare early symptom of ankylosing spondylitis.

The causes of death of 79 patients with ankylosing spondylitis (AS) diagnosed between 1952 and 1959 were investigated. The basic cause of death was a cardiovascular disease in 35.4% of the patients, AS in 29.1%, violent death in 10.1%, malignancy in 8.9%, gastrointestinal diseases in 6.3%, pulmonary tuberculosis in 2.5%, urogenital diseases in 2.5%, respiratory diseases in 3.8% and diabetes mellitus in 1.3%. HL-A antigens were determined in Haida and Bella Coola native Indians, two communities known to have a high prevalence of ankylosing spondylitis. Tests were conducted on those with x-ray evidence of sacro-iliitis and on a sample of the community at large. Sacro-iliitis was found to prevail in approximately 10 per cent of adult Haida males and in over two per cent of Bella Coola adult males. Of 20 Haidas with sacro-iliitis, 17 were HL-A 27 positive.
