Seromucoids in chronic bowel disorders of tropics.

Serum seromucoid estimation was carried out in 60 normal controls and 100 patients. (1) The serum seromucoid values in normal subjects ranged between 37-5-127 mg.% (69-35 +/- 17-84). (2) Serum seromucoid levels were found to be increased in patients of abdominal tuberculosis (210-95 +/- 86-49). These values corresponded directly to the severity of the disease. (3) Seromucoid levels came down to normal after three months of antitubercular treatment. (4) There was an inverse relationship between serum albumin and seromucoids. (5) Serum seromucoids were also found to be increased in patients of a

Post-infective malabsorption in the temperate zone.

A series of 37 adults normally resident in Britain have been investigated for persistent bowel symptoms following acute enteritis. 26 had intestinal malabsorption, of whom 12 had been travelling in the Mediterranean area, whereas 10 developed their illness at home. Mild jejunal mucosal abnormalities were found in cases with and without malabsorption, and the intraepithelial lymphocyte count correlated more closely with the degree of malabsorption than did the histological grading. Over 80% of severe cases were folate deficient. Enterobacteria were cultured from the jejunal fluid in 30% of case

Diarrhea in the returned traveler.

Diarrhea in the returned traveler is a common problem that can be caused by a number of different pathogens. A history of the patient's travel and exposures, the duration of illness, the response to prior treatment, and the clinical syndrome can help to establish a good etiologic differential diagnosis on which further therapy can be based. Many of these patients can be treated empirically with antibiotics, either a fluoroquinolone or azithromycin, without further microbiologic evaluation. Those patients with severe or persistent disease or comorbid illnesses, or those who have failed empiric

Persistent diarrhea in the returned traveler.

In conclusion, the causes of chronic diarrhea in the returned traveler are protean. Careful evaluation requires an understanding of where the traveler has been, when they were there, the type of diarrheal illness, medications taken, and knowledge of the patients' other medical problems. Protozoa, particularly G. lamblia, C. parvum, and C. cayatenensis, are among the more commonly identified agents. If the patient is immunocompromised, microsporidia and Isospora become more likely, and a prior history of antimicrobial use raises the possibility of C. difficile colitis. Occasionally helminths, w

Fifty years after: some experiences of the medical care of prisoners of war.

Mention is made of Italian prisoners-of-war treated in Catterick military hospital but in October 1943 Italy joined the Allied side. An account is given of the medical condition of 78 Japanese POWs and of 228 members of the Japanese Indian Forces (JIFs) in a transit hospital in eastern India (now Bangladesh). Tropical sprue appeared to occur in an epidemic form each June and July in Burma and eastern India, but did not seem to have affected the Japanese or JIFs. A brief account is given of the condition of 180 European or Australian service men who had survived harsh conditions in Japanese POW

Gut hormones in tropical malabsorption.

Concentrations of various gut hormones were measured after a test breakfast in eight patients with severe tropical malabsorption and 12 controls. The patients with tropical malabsorption had greatly raised basal plasma motilin and enteroglucagon concentrations, but their postprandial release of both gastric inhibitory polypeptide and insulin was significantly reduced. The pattern of gut hormone release differed from that found in coeliac disease. The measurement of gut hormones, each of which has a specific site and function, thus throws new light on the pathophysiology of tropical malabsorpti

Profile of chronic small-bowel diarrhea in adults in Western India: a hospital-based study.

BACKGROUND: Small-bowel diarrhea is reported to account for 10% of all cases of chronic diarrhea. Data on the etiology and clinical presentation of chronic small-bowel diarrhea in adult Indians is scarce. METHODS: 50 patients (mean age 32.8 years; 26 men) with chronic small bowel diarrhea were evaluated clinically, and investigated to determine etiology. The diagnosis of small-bowel diarrhea was based on history, stool volume and associated symptoms. RESULTS: Abdominal pain (n=22, 44%) and weight loss (n=37, 74%) were the most common symptoms, apart from diarrhea. Anemia (70%) and hypoalbumi

A COMPARISON OF LABORATORY TESTS IN THE MALABSORPTION SYNDROME.

Absorption of radioiodinated triolein was tested in 93 individuals (15 controls, 28 cases of functional diarrhea, and 50 patients with suspected or manifest malabsorption syndrome). The results so obtained were compared in 59 cases with estimations of fecal radioactivity and chemically determined fecalfat excretion. A highly significant correlation was found. The urinary xylose excretion test was performed in 96 individuals. The diagnostic value of the radioactive fat absorption test was compared with that of the xylose excretion test in 78 cases. Both tests were diagnostic in cases of non-tropi
