[Antibiotic therapy in skin diseases].

Antibiotics are used for systemic and topical skin diseases treatment, taking advantage of their antiseptic, bacteriostatic and also their anti-inflammatory and immunomodulating properties. In case of localized external skin layers infections, a topical treatment is adequate. In case of widespread and deeper infections a systemic treatment is needed. The latter dermatological diseases are mostly treated by beta-lactam antibiotics (penicillins and cefalosporins) macrolides and tetracycline. In topical treatment mostly erythromycin, clindamycin, mupirocyne and fusidic acid are used. Most common

Cerebrospinal fluid shunt infections.

Cerebrospinal fluid (CSF) shunt infection is one of the most frequent and disabling complications. We reviewed the records of 306 patients who underwent CSF shunt surgery from 1983 through 1992. Six hundred and twelve procedures were performed in these 306 patients. Infection occurred following 46 of the procedures for an infection rate of 7.5% per procedure. The 46 infections involved 39 patients. There were 8 recurrent infections. The infection rate per child was 12.7%. Staphylococcal species were isolated in 50% of all infections. Patients younger than 1 year old and children with multiple

Pyodermitis.

Pyodermitis are primary skin infections mainly caused by pyogenic bacteria of the Staphylococcus and Streptococcus genera. They are relatively common diseases that affect adults and children. There have been frequent reports of bacterial resistance to the recommended antibiotics over the last few years; however, new substances are in use or under development, and this represents an evolution in the treatment of pyodermitis. This review aims at describing clinical, diagnostic and therapeutical features of major pyodermitis: impetigo, ecthyma, erysipelas, staphylococcal scalded skin syndrome and

[Exfoliative erythroderma and infective dermatitis in an infant infected with human T-lymphotropic virus type I (HTLV I)].

We report a HTLV-I positive infant, whose infection was confirmed by polymerase chain reaction. The infant presented with an acute, severe, generalized eczema, exfoliation and severe erythroderma that yielded to an acute proteic malnutrition and frequent staphylococcal infections, unresponsive to treatment, since the second month of life. Immunodeficiencies from other origin and other causes of erythroderma were ruled out. The histopathology studies and clinical course yielded to the diagnosis of infective dermatitis associated to HTLV-I. A review of the literature is performed.

[Hyper-IgE syndrome].

The Hyper-IgE-syndrome (Job-, Buckley-) is characterized by recurrent staphylococcal infections of the skin, the ears and the lungs, by an eczematoid dermatitis from early infancy on, and by extreme elevation of serum IgE. The inconstantly found decreased chemotaxis of the polymorphonuclear leukocytes seems to be a secondary sign of a so far unknown deficiency possibly of the T-cell mediated immunity. Thus, therapy is restricted to antibiotic and surgical treatment. Many patients have a typical coarse face and some involvement of the bones (osteoporosis, craniosynostosis).

[Drepanocytic osseous infections (osteitis and osteomyelitis) (author's transl)].

Osseous infections have a special importance among the various polymorphic osteoarticular troubles of drepanocytemia. From 92 cases, the authors emphasize 4 points: --Osteitis and osteomyelitis are frequent, specially in the homozygotic form. --Salmonellae are the prevailing pathogenic bacterias. --Osteomyelitis caused by staphylococcus have, in drepanocytic patients, a more serious and more expanding evolution than in other patients. --Anatomic and radiologic features as well as evolution are different in the staphylococcal forms and in the salmonellal ones in which, most often, surgery may b

If left unchecked, infections involving antibiotic-refractory bacteria are expected to cause millions of deaths per year in the coming decad … ir sensitivity to FQs, including delafloxacin (Dela)-a drug that was recently approved for treating staphylococcal infections. We found that while the added nutrients increased nucleic acid synthesis, this increase was not required to sensitize S. aureus to FQs. We further demonstrate that addition of these nutrients increases membrane potential and the ability to generate …

Methicillin-resistant Staphylococcus aureus: a controversial food-borne pathogen.

UNLABELLED: Methicillin-resistant Staphylococcus aureus (MRSA) is a major cause of severe healthcare-associated (HA) infections. Although during the last decade the incidence of HA invasive infections has dropped, the incidence of community-associated MRSA (CA-MRSA) infections has risen among the general population. Moreover, CA-MRSA, livestock-associated MRSA (LA-MRSA) and HA-MRSA (HA-MRSA) can be found in foods intended for human consumption. Several studies from different geographical areas have reported the presence of enterotoxin genes in several MRSA food isolates. Molecular typing studi

Purpura Fulminans and Spotted Fever: A Case Series from South India.

Purpura fulminans (PF) is associated with acute infections such as meningococcal, staphylococcal, streptococcal, and rickettsial infections. However, there are only a few reports of association of PF with rickettsial fever from India. In this case series of seven adults with PF, four were definitive cases of spotted fever as the ompA real-time polymerase chain reaction was positive. The other three adults were probable cases of spotted fever, as they were positive by immunoglobulin M enzyme-linked immunosorbent assay, and their fever subsided within 72 h of rickettsia-specific therapy. Three o

Antibiotic resistance in Staphylococcus aureus and its relevance in therapy.

Staphylococcus aureus is a major cause of infections. Only approximately 20% of the strains remain sensitive to penicillin. Beta-lactamase stable penicillins such as flucloxacillin form the mainstay of treatment of staphylococcal infection. Meticillin-resistant S. aureus (MRSA) are resistant to all beta-lactam antibiotics. Glycopeptide antibiotics are effective against most MRSA strains but, in the last few years, isolates of MRSA that have reduced susceptibility to glycopeptides (glycopeptide-intermediate S. aureus) have been isolated. Some strains exhibit frank resistance to glycopeptides (v

Severe bacterial infections of the skin: uncommon presentations.

Although most bacterial infections of the skin prove to be minor in nature, a few such dermatologic entities are significant, to the point of even being fatal. Their course can be extremely rapid and can lead to dreadful complications. The mortality rate is usually up to 30% to 50% and depends upon the type of infection, underlying disease, and immune status. Patients suffering them usually need to be hospitalized, sometimes in intensive care or burn units. They should be treated systemically with appropriate antimicrobial therapy plus aggressive supportive care. The two life-threatening skin

Factors associated with methicillin resistance in diabetic foot infections.

Methicillin-resistant staphylococcal infections often present a challenge to physicians treating patients with pedal wounds. Most methicillin-resistant Staphylococcus aureus infections have been thought of as nosocomial in origin. Several studies have identified specific modes of transmission via hospital reservoirs such as the anterior nares of the patient, inanimate objects within close proximity of the patient, and direct contamination from health care providers. Exposure of patients to these reservoirs through hospitalization has been shown to increase the patient's risk of obtaining a met…

Antibiotics and the expression of staphylococcal virulence.

The last 25 years have witnessed a continuing interest in staphylococci as causes of human infection even though over 100 years have elapsed since their discovery. This has been due in part to the recognition of new disease entities such as scalded skin syndrome, toxic shock syndrome and various infections due to coagulase-negative staphylococci. Development of antimicrobial agents has not solved the problem of these infections partly because of their mediation by novel toxins and partly due to the emergence of multiple drug resistance. However study of the interaction between certain antibiot

Staphylococcus aureus: the enduring pathogen in surgery.

Staphylococcus aureus is part of the normal bacterial flora of the upper respiratory tract, especially the nose, but it can colonise other sites, such as the skin. However, S. aureus is also the commonest cause of surgical site infection (SSI) and is a major cause of bloodstream infection (BSI). The development of staphylococcal infection arises from a combination of bacterial factors, e.g. production of toxins and host factors, including underlying patient disease, e.g. diabetes mellitus. The surveillance of SSI and BSI are increasingly components of national quality programmes to reduce heal

Protective role of complement C5a in an experimental model of Staphylococcus aureus bacteremia.

The complement system is a key component of the innate immune response. Here, we have examined the role of complement anaphylatoxin C5a in an experimental model of Staphylococcus aureus bacteremia. Our data provide compelling evidence for a protective role of C5a during staphylococcal bloodstream infection.

Fatal infections in protein-calorie malnourished children with thymolymphatic atrophy.

The clinicopathological features of 25 children who died with protein-calorie malnutrition were studied. All but four subjects were found at necropsy to have nutritional thymectomy and all but 3 died of infectious diseases. The infectious agents were chiefly intracellular micro-organisms including miliary tuberculosis, Herpes simplex, varicella, measles, Pneumocystis carinii, and Plasmodium falciparum. Staphylococcal infections, salmonellosis, shigellosis, strongyloidiasis, and hookworm were other significant infectious agents. Nutritionally acquired defective immunity, especially cell-mediate…

Evolving Patterns of Etiological Profiles and Susceptibility Trends of Healthcare-Associated Infections Since Inception: A Meta-Analysis.

Over six decades, global surveillance has revealed major shifts in healthcare-associated infections and antimicrobial resistance. Synthesizing data from multicontinental studies spanning 1992-2021, early reports showed balanced Gram-positive and Gram-negative pathogens with rising methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant Enterococcus (VRE). Subsequent findings indicated a shift toward multidrug-resistant Enterobacterales, persistent ICU non-fermenters, and emerging Candida auris, alongside declining MRSA and Clostridioides difficile rates but increasing carba
