[Strongyloidiasis: a clinical, biological and epidemiological study of 350 cases (author's transl)].

The authors report on a series of 350 cases of strongyloidiasis. The patients had come from the French West Indies or from Africa and had been living in France for less than five years. Stool examinations were performed systematically or on account of eosinophilia, which was often pronounced (400-1500 per cubic millimetre). Although 44 patients offered favourable conditions for the worms to multiply, no death was recorded. The relevant published data are discussed.

Strongyloidiasis in high-income countries: Taiwan's data illuminate a global blind spot.

Strongyloidiasis remains neglected despite its lethal potential in immunocompromised hosts. Recent Taiwanese data reveal a prevalence of 1.9-4.8 % among at-risk cohorts, challenging assumptions that strongyloidiasis is rare in high-income countries (HICs). This commentary highlights three key implications: (i) systematic screening for elderly and immunosuppressed patients, (ii) integration into One Health surveillance frameworks recognizing zoonotic cycles, and (iii) economic evaluations to define cost-effective screening strategies. Addressing this silent infection is urgent amid expanding mi…

Challenges in managing a multifactorial eosinophilic pneumonia: daptomycin vs strongyloidiasis case report.

BACKGROUND: Eosinophilia is defined as a blood eosinophil count > 500/mcL with etiology usually an allergic reaction or parasitic infection which can lead to serious organ damage. CASE PRESENTATION: A patient being treated for hardware infection develops eosinophilia while on daptomycin in the setting of a positive strongyloides antibody. The patient was on chronic steroids prior to admission for epitheliopathy which complicated care. The daptomycin was discontinued, ivermectin initiated to treat strongyloidiasis, and high dose steroids initiated simultaneously. Eosinophilia resolved and pati…

Asymptomatic Strongyloidiasis among Latin American Migrants in Spain: A Community-Based Approach.

UNLABELLED: Strongyloides stercoralis infection is frequently underdiagnosed since many infections remain asymptomatic. AIM: To estimate the prevalence and characteristics of asymptomatic S. stercoralis infection in Latin American migrants attending a community-based screening program for Chagas disease in Spain. METHODOLOGY: Three community-based Chagas disease screening campaigns were performed in Alicante (Spain) in 2016, 2017, and 2018. Serological testing for S. stercoralis infection was performed using a non-automatized IVD-ELISA detecting IgG (DRG Instruments GmbH, Marburg, Germany).

World Gastroenterology Organisation Global Guidelines: Management of Strongyloidiasis February 2018-Compact Version>.

Strongyloides stercoralis is a soil-transmitted helminth, but it has a unique life cycle that can be completed in the human host, in a process known as autoinfection. Worldwide, the burden of disease is substantial (300 to 400 million infections). Strongyloidiasis is mainly prevalent in the tropics and subtropics, but there is as yet no global public health strategy for controlling the parasite.

Duodenal strongyloidiasis - Our 15-year histopathological experience at a tertiary gastrointestinal centre.

In order to emphasise the importance of histopathology in the clinically unsuspected diagnosis of duodenal strongyloidiasis, we report six cases diagnosed on duodenal biopsies identified from the database over a period of 15 years, and clinical, endoscopic and histopathological findings were analysed retrospectively. Four were elderly males and the remainder young females. Only one patient had an underlying immunocompromised state. Three presented with cholestatic jaundice and simulated hepatobiliary malignancy. In all cases, endoscopy provided non-specific findings. Only one case showed a pre…

Severe strongyloidiasis and systemic vasculitis: comorbidity, association or both? Case-based review.

A possible association between strongyloidiasis and systemic vasculitis is rarely reported in the literature. We report the case of a patient with severe strongyloidiasis and an angiographic finding consistent with polyarteritis nodosa. Diagnosis of strongyloidiasis was made by finding of larvae and adult parasites in samples of the upper gastrointestinal tract mucosa and stool. The patient was treated with albendazole, ivermectin and corticosteroid withdrawal. This therapy led to the resolution of symptoms, with repeated stool samples negative for S. stercoralis. However, the clinical course …

Parasitological versus molecular diagnosis of strongyloidiasis in serial stool samples: how many?

Strongyloidiasis is usually an asymptomatic disease in immunocompetent patients, caused by Strongyloides stercoralis. However, in immunocompromised patients it can produce a severe clinical profile. Therefore, a correct diagnosis is necessary in these cases and in those chronic asymptomatic patients. The low sensitivity of classical parasitological techniques requires the analysis of multiple serial stool samples. Molecular diagnostic techniques represent an improvement in the detection of the parasite. The objective of this study was to evaluate the minimum number of samples necessary to achi…

Disseminated strongyloidiasis arising from a single dose of dexamethasone before stereotactic radiosurgery.

A fatal case of disseminated strongyloidiasis is described, abruptly following a single high dose of dexamethasone before stereotactic radiosurgery. The mechanism of steroid-induced dysregulation of Strongyloides infection is unclear. Treatment failure and the use of rectal thiabendazole in the presence of bowel obstruction is discussed. This case reinforces the need to screen selected patients for strongyloidiasis before any high-dose steroid therapy, particularly in the presence of persistent eosinophilia.
