Communication and swallowing outcomes following functional salvage total laryngectomy.

OBJECTIVES: Severe laryngeal dysfunction following (chemo)radiotherapy for head and neck cancer may be managed with functional salvage total laryngectomy (FSTL). We investigated communication and swallowing outcomes following FSTL at our tertiary centre. METHODS: All patients treated with FSTL from 2009-2023 were included. Functional Oral Intake Scale score and primary mode of communication were recorded at pre-surgical baseline, point of discharge from inpatient admission, six and 12 months post-surgery. RESULTS: Ten patients were identified. Pre-surgery all patients were nil by mouth due t

Clinical Conundrum: The Post-tonsillectomy Patient Who Refused to Swallow.

Tonsillectomy is a common surgical procedure performed by otolaryngologists, primarily indicated for the management of recurrent tonsillitis and obstructive sleep apnea. While postoperative pain and hemorrhage are well-recognized complications, persistent dysphagia following tonsillectomy is rare. In such cases, a comprehensive evaluation is warranted to identify potential mechanical or functional causes. Psychogenic dysphagia is a diagnosis of exclusion and may significantly affect a patient's quality of life. Timely intervention, including detailed swallowing assessment and multidisciplinary

[Swallowing disorder after stroke - diagnosis and therapy in primary care].

Dysphagia is a common consequence of stroke, affecting a significant proportion of patients especially during the acute phase. They carry a substantial risk of complications such as aspiration pneumonia, malnutrition, and dehydration, and can considerably impair both quality of life and rehabilitation outcomes. In general practice, where many stroke survivors receive follow-up care, early identification of dysphagia is essential to prevent serious consequences like pneumonia or nutritional deficits. This article provides an overview of the pathophysiology and clinical symptoms of post-stroke d

Swallowing Pathophysiology in Primary Sarcopenia: A Multimodal Assessment in Older Adults.

Primary sarcopenia, characterized by age-related decline in muscle mass and strength, is increasingly recognized as a contributor to oropharyngeal dysphagia in older adults. Understanding its mechanical and functional components is essential for early detection and intervention. This study aimed to investigate early oropharyngeal swallowing impairments in older adults with primary sarcopenia using a multimodal instrumental assessment and to evaluate the utility of combining different objective measures. In this cross-sectional study, swallowing safety and efficiency were assessed using the Dyn

Dynamic motion analysis of the cervical spine during normal swallowing.

This study aims to clarify the dynamic changes in the cervical lordotic angle (CLA) during normal swallowing using an automated motion analysis method. Physiological cervical lordosis is crucial for spinal alignment and musculoskeletal function. While previous studies have noted the relevance of cervical curvature in clinical contexts, its dynamic modulation during swallowing has not been well studied. Videofluoroscopic swallowing examinations were conducted in 39 healthy individuals without cervical spine disease. A 2-dimensional template matching technique was applied to automatically track

Swallowing Guidance with FEES May Alleviate Symptoms in Functional Dysphagia.

Dysphagia is a common concern. In an outpatient phoniatric and otorhinolaryngology clinic, approximately one fourth of dysphagia patients are classified as having non-organic, or functional dysphagia. This study aimed to evaluate symptom severity, health-related quality of life (HRQoL) and disability among dysphagia patients. Additionally, it focused on assessing the impact of flexible endoscopic evaluation of swallowing (FEES) and swallowing guidance particularly for patients with functional dysphagia. We recruited 60 consecutive dysphagia patients from our phoniatrics clinic. They completed

Test-Retest Reliability of Swallowing Outcome Measures in Huntington's Disease.

BACKGROUND: Dysphagia in Huntington's disease (HD) has been described using instrumentation. However, no research has evaluated reliability of swallowing measures in this population. OBJECTIVES: This study evaluated the test-retest reliability of swallowing measures in HD. METHODS: Ten participants with HD and dysphagia were assessed at three intervals over 1 week. Functional measures of swallowing included the Timed Water Swallow Test (TWST) and Test of Masticating and Swallowing Solids (TOMASS). Swallowing biomechanics were measured using manofluoroscopy and ultrasound. RESULTS: Nine part

A swallowed denture leading to misdiagnosis with esophageal neoplasm: a case report.

INTRODUCTION: Foreign body ingestion can lead to esophageal complications, including perforation and impaction, in up to 20% of cases, making it a critical situation. Misdiagnosis or delayed diagnosis can cause severe complications. CASE PRESENTATION: We present the case of a 78-year-old female who swallowed an acrylic partial denture leading to progressive dysphagia and a vegetative ulcerative lesion on endoscopy. The lesion was initially misdiagnosed as a neoplasm of the esophagus. CT scan and a repeat endoscopy revealed the presence of a denture in the esophagus. The denture was successful

Airway and Swallowing Outcomes in Children With Combined Laryngomalacia and Laryngeal Cleft.

Laryngomalacia (LM) and laryngeal cleft (LC) can independently cause dysphagia but rarely can occur concomitantly. We discuss the presentation, decision-making, and swallow outcomes following surgical correction of combined LM and LC. We present four patients with combined LM and an LC who underwent both primary supraglottoplasty (SGP) and laryngeal cleft repair (LCR). Each patient presented with recurrent choking or coughing with feeds. Stridor was only present in two patients. Patients with SGP saw the resolution of stridor when present, but dysphagia persisted in all four cases. LCR clinica

Serotonin therapies for opioid-induced disordered swallow and respiratory depression.

Opioids are well-known to cause respiratory depression, but despite clinical evidence of dysphagia, the effects of opioids on swallow excita … sults largely confirmed the following hypotheses: 1) swallow-related EMG activity was larger during swallows elicited by esophageal distension plus oral water infusion than by either stimulus alone. 2) Buprenorphine depressed swallow in both sexes, but females were more susceptible to total swallow suppression. 3) Female animals were also more vulnerable t …

An accelerometry and gyroscopy-based system for detecting swallowing and coughing events.

Measuring spontaneous swallowing frequencies (SSF), coughing frequencies (CF), and the temporal relationships between swallowing and coughing in patients could provide valuable clinical insights into swallowing function, dysphagia, and the risk of pneumonia development. Medical technology with these capabilities has potential applications in hospital settings. In the management of intensive care unit (ICU) patients, monitoring SSF and CF could contribute to predictive models for successful weaning from ventilatory support, extubation, or tracheal decannulation. Furthermore, the early predictio

Spatial relationships of oropharyngeal structures during respiration, chewing, and swallowing.

Spatial relationships between oropharyngeal structures and their coordinated dynamics ensure proper adaptations in functions such as respiration, chewing, and swallowing. Thus, the aim of this study is to analyze spatial changes in the normal oropharynx during respiration and mastication. For this purpose, eight 7-8 months old Yucatan minipigs, four of each sex were used. X-ray fluoroscopy was recorded with the field of view focused on the oropharyngeal region. The x-ray video clips showing respiratory cycles and masticatory sequences were digitized and traced. Points to be digitized and trace
