In 1997, over four thousand people on organ waiting lists lost their lives. The current organ shortage has led to the need to find alternative sources of organs for transplantation. Advances in immunosuppressive therapies and genetics have made xenotransplantation, the transplantation of animal organs into humans, an increasingly viable technology. However, the promise of xenotransplantation is tempered by the unspecified amount of risk that zoonotic pathogens could mutate and infect humans with potentially deadly diseases.

Prader-Willi syndrome (PWS) is associated with an insatiable appetite and (often) other maladaptive behaviours (self-injury, sleep disorders, insistence on routines, and temper tantrums). Psychoses are not a recognised feature. Most affected people have a chromosome 15 abnormality (deletion, disomy, structural rearrangement, etc.). Three people with PWS who developed psychotic disorders in early adult life are described. The nature of the psychoses and the significance of the association are discussed.

The clinical manifestations of sarcoidosis seen at the Lagos University Teaching Hospital are presented. The clinical features and management were in the main similar to those described in the temperate countries, but the patients tended to be older at presentation and were seen at a more advanced stage of the disease.Erythema nodosum and ocular lesions were relatively rare. The rarity of sarcoidosis in Lagos may be more apparent than real. Increased physician awareness of the disease is leading to the detection of more cases.

Blood products are scarce resources requiring prudent and reasoned allocation. The utilization of red blood cells and platelets in terminally ill patients can be complicated and requires guidelines tempered by individualized considerations. Representative cases are discussed in which blood products are requested or utilized by patients at the end of life. Relevant literature is reviewed and ethical issues pertaining to each case are discussed. A practical approach to blood product utilization at the end of life is suggested.

We report the first case of viscerotropic syndrome in Ecuador. Because of similarities between yellow fever and viscerotropic syndrome, the incidence of this recently described complication of vaccination with the 17D yellow fever vaccine is not known. There is a large population in South America that is considered at risk for possible reemergence of urban yellow fever. Knowledge of potentially fatal complications of yellow fever vaccine should temper decisions to vaccinate populations where the disease is not endemic.

Pyomyositis has become an increasingly recognized disease in temperate climates. A patient receiving chlorambucil for progressive systemic sclerosis, in whom streptococcal pyomyositis developed, is presented. Magnetic resonance imaging led to the early diagnosis. To our knowledge, this is the first case report of pyomyositis in a patient with scleroderma, and the first association with group G streptococcus infection in an adult. Connective tissue diseases and immunosuppression may emerge as risk factors for the development of this entity.

Despite the wide application of aids for cardiac auscultation, their diagnostic efficacy has not been comprehensively evaluated. The most commonly used auscultatory aids, including respiration (Carvallo's maneuver), Müller and Valsalva maneuvers, squatting, standing, handgrip, intrinsic cycle length changes, and amyl nitrite, are discussed. A review of the literature suggests that enthusiasm for these aids must be tempered because of limitations in sensitivity and specificity as well as weaknesses in the methods used in the validation studies.

The human immunodeficiency virus (HIV) pandemic has disproportionately affected tropical regions of the world, where dermatoses, such as leprosy and leishmaniasis, rarely encountered in temperate climates, are endemic. Although the introduction of highly active antiretroviral therapy (HAART) has been lifesaving, a few patients undergoing HAART experience clinical deterioration caused by immune reconstitution inflammatory syndrome (IRIS). This article explores the range of tropical dermatoses that are reported to date with associated IRIS events.

Pulmonary edema is a rare complication in preeclampsia patients without pre-existing cardiac disease, tocolytic therapy, infection, or multiple gestations. We present a preeclamptic patient complicated by overwhelming pulmonary edema just prior to cesarean section and the massive edematous fluid production made it an exceptional challenge to the anesthesiologist on airway management. Emergent classic laryngeal mask airway (cLMA) placement was an effective alternative means for ventilation to provide temperate oxygenation in this case.

A previously undescribed marine bacterium, Vibrio damsela, was isolated from naturally occurring skin ulcers on a species of temperate-water damselfish, the blacksmith (Chromis punctipinnis). Laboratory infection of the blacksmith with Vibrio damsela produced similar ulcers. Vibrio damsela was pathogenic for four other species of damselfish but not for members of other families of fish. The bacterium has also been isolated from water and from two human wounds and may be a cause of human disease.

Changes in serotonin function and disturbances in tryptophan availability have been implicated in many psychiatric disorders, including alcoholism. In the present study we took serum free tryptophan samples from 31 healthy volunteer controls and from 42 DSM-III-R alcohol-dependent subjects who had abstained from alcohol for at least 2.5 weeks (range 2.5-104 weeks). Although chronic alcoholics frequently present with gastrointestinal (GI) symptoms, the precise prevalence of GI symptoms in this group is unknown. Accordingly, we compared the frequency and severity of GI symptoms in 48 male alcoholics with those of 48 nonalcoholic controls. Cytochromes P450 (P450) are a family of isozymes which play an important role in xenobiotic metabolism. The concentration of three P450 isozymes, namely P450-IIE1(A1c),-IIIA(NF) and -IIC8-10(MP) has been measured in human liver biopsies of patients with different alcohol and drug intake status.
